3.Surgical Results in Constant Exotropia According to Fixation Patterns.

Surgical results of 59 patients with constant exodeviation having at least 2 months of follow-up were reviewed. Fifty nine cases of constant exodeviation were classified by fixation patterns into alternating exotropia 38 cases and monocular exotropia 21 cases. The final surgical results and success rates were compared between the 2 groups and were analyzed according to the length of postoperative follow-up period. The surgical results were as follows: in alternating exotropia, 63.2% was satisfactory, 28.9% undercorrected and 7.9% overcorrected, in monocular exotropia, 85.7% was satisfactory, 9.5% undercorrected and 4.8% overcorrected. The mean follow-up period was 5.3 months in alternating exotropia and 5.9 months in monocular exotropia. Final surgical results were not affected by the length of postoperative follow-up period in alternating exotropia. However, in monocular exotropia, there was a strong tendency to become exotropic with a long term follow-up period(2-17 months). We suggest that the fixation patterns could have an influence on postoperative stability in exodeviation.

6.Medical policy for suicide prevention

Suicide is a serious problem worldwide, especially in Korea, but the factors affecting suicide are complex and diverse. Therefore, a medical policy should be developed to help establish an effective national suicide prevention policy in Korea. To reduce suicide rates effectively, some of the current suicide prevention policies need to be improved. This study proposes the following steps to do so: 1) standardizing the suicide attempt management system, 2) strengthening effective counseling programs in the psychiatric field, and 3) strengthening early psychiatric interventions in patients with a physical illness. These improvements should be considered high-priority measures for ensuring successful suicide prevention at the national level.

9.Pheochromocytoma and Renal Artery Stenosis.

In hypertensive patients it is very important to detect renal artery stenosis or pheochromocytoma, since both diseases are curable causes of hypertension. However, renal artery stenosis can be induced by pheochromocytoma, when the diagnosis of the two simultaneous disease is very difficult. We experienced two cases of pheochromocytoma presented as renal artery stenosis. Pheochromcytoma was overlooked when renovascular hypertension was diagnosed. Pheochromocytoma was found during surgery in one patient and after angioplasty in the other. In both cases, BP returned to normal after surgical removal of pheochromocytoma without repair of the stenosis. Prevention of ineffective and unnecessary renal artery angioplasty of surgery requires knowledge of this unusal association between pheochromocytoma and renal artery stenosis and a high degree of clinical alertness for pheochromocytoma.
