One hundred fifty bacteriae isolated in significant numbers from 96 specimens of urine of the 45 in-patients with long-term indwelling catheter were evaluated from January, 1978 to July. 1979 and the following results were obtained; 1. In 20 patients (44.4%), positive urine culture was obtained within 10 days after indwelled catheter in the urinary tract and 97.9% of the patients revealed the positive urine culture within 20 days after that. 2. Of 150 bacterial strains isolated by cultures of the urine, 96% was gram-negative bacteria and 4% was gram-positive. Isolated organisms included klebsiella(27.4%), E.coli(23.3%). proteus(14.7%), pseudomonas(12.0%). coliform bacilli(10.0%). other gram-negative organism (8.6%), staphylococcus(2.7%) and streptococcus (l.3%) in order of frequency. 3. In 4 patients(8.9%), bacteremia was developed. Isolated organism from blood culture and those from urine culture were same. The mortality rate was 50%.

We developed an animal model to recreate the condition of an open fracture in communication with the maxillary sinus. We then studied wound healing of the sinus wall structures following fracture in the presence of autogenous bone and alloplastic implant. This model is designed to simulate the repair of an orbital floor fracture in humans. The New Zealand White rabbit was used as the animal model. Standardized 8mm defects were made bilaterally in the maxillary sinuses to include bone and mucosa in 36 rabbits. Two different implants and autogenous calvarial bone graft were placed in the soft-tissue pockets to obturate the defects, exposing one surface of the implant to the open sinus. Medpor porous polyethylene, silicone and calvarial bone implant were compared. Animals were killed at 1, 2 and 8 weeks after implantation. Gross examination of the specimens for the amount of mucosal closure and implant tissue fixation was performed. Histological sections were evaluated for bone and soft-tissue morphology juxtaposed to the implant. Complete closure of the mucosal defect was demonstrated with each type of implant. Medpor implants showed both vascular and soft-tissue ingrowth into pores by week 1. Bone ingrowth was seen by week 2. Closure of the Medpor obturated defects occurred more rapidly than in the silicone group. The Medpor implants and calvarial bone demonstrated bone and soft-tissue fixation, callus formation and maturation, while mature overlying mucosa was reconstituted over the defects. Silicone implants demonstrated a fibrous tissue reaction within 1 week of implantation and they never became fixed to bone or soft tissue. Maxillary sinus wall regeneration occurred in all defects. This study supports clinical observations of maxillary sinus wall regeneration in humans.

4.A Clinical Observation of the Renal Angiography.
Korean Journal of Urology 1980;21(6):566-575
Renal angiography provides the ultimate in detailed vascularization of the renal vessels and vascular architecture of kidney. It is used in diagnosis and treatment of renal diseases e.g. renal injury, tumor, cyst and inflammation. A clinical observation of renal angiography was made on 49 cases of renal diseases which had been admitted to the Department of Urology, National Medical Center from 1975 to 1979. The diagnostic accuracy of renal angiography was 87.8%.

Dermatofibrosarcoma protuberans (DFSP) is a rare, distinctive cutaneous tumor, which consists of spindle shaped ceils arranged in densely packed interlacing bundles with the storiform or cartwheel pattern. Histologically, it resembles deep growing dermatofibroma, nodular fasciitis, neurofibroma and neural sheath tumors. DFSP is one of t.he connective tissue tumors which is difficult. to diagnose histologically as well as clinically. Recently, the immunochemical staining with a monoclonal antibody to CD34 is reported to give assistance in the clear differential diagnosis of DFSP from other fibrous or neural tumors. Herein, three cases of DFSP were stained by immunohistochemical staining with S-100 protein, vimentin, factor VIII and anti-CD34 antibody in order to assess the use of anti-CD34 in the differential diagnosis of DFSP.

8.Clinical Characteristics of Polypoidal Choroidal Vasculopathy Associated with Chronic Central Serous Chorioretionopathy.
Korean Journal of Ophthalmology 2012;26(1):15-20
PURPOSE: To investigate the clinical characteristics of polypoidal choroidal vasculopathy (PCV) associated with chronic central serous chorioretinopathy (CSC). METHODS: We retrospectively reviewed the medical records of 246 PCV patients (283 eyes) between July 2004 and August 2009 and investigated the clinical characteristics of the PCV patients who had specific fundus findings of chronic CSC. RESULTS: Among PCV patients, 13 eyes (4.6%) of 13 PCV patients (5.3%) had fundus findings of chronic CSC. All of the PCV lesions had a solitary polyp located outside the atrophic retina, predominantly in the macular area (84.6%), most showed an exudative pattern (69.2%) and there were a few that showed a hemorrhagic pattern (30.8%). All of the lesions were smaller than 1 disc diameter. Most of the PCV lesions (76.9%) were cured with less than two treatments in a short period of 6.4 +/- 1.9 months; however, visual acuity deteriorated (61.5%) or was not changed (30.8%) in most of the cases. CONCLUSIONS: The PCV associated with chronic CSC had several clinical features such as a small exudative retinal lesion with a solitary polyp and frequent involvement of the macular area. Even though there was poor visual outcome due to the atrophic change, all of the PCV lesions were easily resolved in a short period with a simple treatment course and no recurrence.

9.Urologic Aspect in the Management of VATER Anomalies.

Korean Journal of Urology 1997;38(11):1202-1209
Ten cases of VATER syndrome were reviewed to determine the type of anomalies, genitourinary anomalies, complications of treatment and prognosis. None of the four cases that were born in our hospital and six cases that were referred to us were diagnosed prenatally. Eight patients were born at full term with normal birth weight, two were premature and one showed pre-eclampsia at IUP 34 weeks. The male to female ratio was 6 to 3 with one patient having ambiguous genitalia. Tracheo-esophageal fistula was found in five patients. Three of these five patients died after the parents refused treatment, and one died the next day after birth even with treatment. Imperforate anus was observed in seven patients. Unilateral renal agenesis, crossed renal ectopia and dysplastic kidney were found in four, three (one pelvic kidney, one crossed renal ectopia with fusion, one crossed renal ectopia) and one patients, respectively. There was one hypospadias. Six of the eight patients with vertebrae anomaly survived; four of the survivors had vesicoureteral reflux (VUR) and neurogenic bladder. Two of the four patients with VUR received ureteroneocystostomy then clean intermittent catheterization (CIC), sympatholytic and antibiotics. The other two patients with neurogenic bladder received clean intermittent catheterization (CIC) and vesicostomy. Complications of recurrent urinary tract infection (UTI) and acute epididymitis occurred when descending loop colostomy was performed due to imperforate anus and urethro-rectal fistula. Separated colostomy instead of descending loop colostomy is recommended for recto-urinary fistula since infections of the upper urinary tract in such cases required ureteroneocystostomy before the correction of imperforate anus and continued antibiotic administration was needed because of the possibility of delayed wound healing and recurrent UTI. A strong will by the patient`s parents, protection of renal function through early detection of genit-ourinary anomalies and other appropriate treatments are required for the high success rate in the surgical correction of VATER syndrome.
