3.Malignant Lymphoma of Mucosa-associated Lymphoid Tissue Arising in the Conjunctiva.

We report two cases of malignant lymphoma of mucosa-associated lymphoid tissue arising in the conjunctiva. Case 1, an 18-year-old girl, showed her left lower conjunctival swelling for four months and Case 2, a 29-year-old man, showed similar lesions on both his eyes for one year. Histologically, both cases revealed monomorphous dense lymphoid infiltrates invading the overlying conjunctival epithelium with diffuse patterns. The infiltrates were composed of small lymphocytes admixed with centrocyte-like cells within the conjunctival epithelium and stroma in association with B-cell lineage. No relapse was noted during the period of about 1½ years follow-up in case 1 treated by local excision only and in the follow-up period of 2 months in case 2 treated by local excision and subsequent chemotherapy.

5.Clinical Survey of Pstients in the Intensive Care Unit .

The ICU at Severance Hopital was opened with 7 beds on October 18, 1968 and expanded to 19 beds on February 2, 1981. From 1970-977, We statistically analyzed 3,072 ICU patients. During the period of years from March 1975 to February 1982, We have analyzed clinically 4,348 ICU patients. The Results were as Follows: 1) During the 7 years the total number of patients was 4,383. ln 1981, 1063 patients were admitted to the ICU: this number being almost twice that of in 1980. 2) During the 7 years, the overall mortality was 15.1%, highest in 1976(30.1%) and lowest in 1979(11.5%). It has decreased annually, 12.4% in 1980, 12.3% in 1981. 3) Total admission days in ICU were 18,148 days, the average patient stay in ICU being 4.17 days. The majority of the patients(81.2%) stayed in ICU less than 5 days. The number of patients staying in ICU more than 6 days increased every year from 14.2% in 1975 to 22% in 1981. 4) About 50% of the patients admitted were under the forties-age group. The number under the teens old group was 19%, this group representing the highest number of patients admitted to the ICU. The highest mortality(18.5%) occurred in the 1 to 10 yearold-age group. 5) The number of patients admitted to the department of internal medicine was 1,574 cases, which was the highest among all departments. The mortality rate was highest in the neurosurgery department(41.9%). In 1981, the number of ICU patients in cardiac surgery and pediatrics was increased three time compared with that of 1975. 6) All ICU patients have been divided into two categories, operative and non-operative cases. The operative cases were 2,150, non-operative cases 2,198 cases. Mortality rate was 10% in operative cases, 20% in non-operative cases. 7) Case expired from march 1981 to February 1982: The number of expired parients including those with a poor prognosis was 183 cases. The number of patients that expired under the 10-year-age group was the highest, 78 cases. The cases that expired in internal medicine and pediatrics were 63 and 58 cases respectively. The causes of death were due to damage of central nervous system, cardiovascular system, respiratory system in that order. From the above results, it can be concluded that in spite of an increase in patients, the mortality rate has decreased annually.

8.The Clinical Value of the Serum and Urinary Levels of beta-HCG in Transitional Cell Carcinoma of the Bladder.

It has been reported that beta-HCG is produced in the transitional cell carcinoma of the bladder and measuring of its level can be used as a tumor marker. We measured the serum and urinary levels of beta-HCG in the patients with the transitional cell carcinoma of the bladder and analysed the relationship to the clinical stage and histological grade and its significance as a tumor marker. The levels of the beta-HCG in the serum and early morning urine were measured in 40 cases of transitional cell carcinoma of the bladder and 37 cases of control group. Double-antibody immunometric assay (Coat-A-Count HCG IRMA) using I-labeled anti HCG antibody and monoclonal anti HCG antibody were used. The mean levels of the serum and urinary f-HCG in control group were 1.2mIU/ml, 1.4mIU/ ml and 3.6mIU/ml, 6.0mIU/ml in patient group, which showed a statistically significant difference between the two groups(p<0.05). In patient group, the levels of the serum and urinary beta- HCG were 2.9mIU/ml and 3.5mIU/ml in the superficial tumor, 2.6mIU/ml and 7.0mIU/ml in the invasive tumor and 7.5mIU/ml and 9.9mIU/ml in the metastatic tumor, showing a statistically significant increase as the clinical stage was advanced(p <0.05). The levels of the serum and urinary beta-HCG were 3.3mIU/ml and 1.3mIU/ml in grade I, 2.9mIU/ml and 5.9mIU/ml in grade II and 4.7mIU/ml and 7.6mIU/ml in grade III, which did not demonstrate a significant relationship to the histological grade(p>0.05). We think that the serum and urinary levels of beta-HCG in the transitional cell carcinoma of the bladder are related to the tumor stage but not to the histological grade and in the metastatic tumor urinary beta-HCG level can be used as a reliable tumor marker.

10.Experimental Study on Representation of Flow on the Bifurcated Carotid Arterial Phantoms Using Magnetic Resonance Angiography.

PURPOSE: A common finding of carotid artery on magnetic resonance angiograms(MRAs) is a signal dropout along the posterior wall of carotid bulb due to reverse flow. The purpose of this study is to evaluate variable flow patterns on bifurcated carotid arterial phantoms using steady-state flow. MATERIALS AND METHODS: We designed phantoms of a bifurcated carotid artery with acrylic materials. Flow patterns were evaluated with axial and coronal imaging of MRA(2D-TOF, 3D-TOF), color Doppler imaging, and computational fluid dynamics (CFD) within the phantoms constructed of an automated closed-type circulatory system filled with 4% sugar solution. These findings were compared with findings obtained from normal volunteers. RESULTS: Axial 3D-TOF MRA images exhibited closer resemblance to the contour of the inner wall of phantoms when compared to coronal 2D-TOF MRA imaging. However, 2D-TOF MRA showed good contrast difference of signal intensities between forward flow area and reverse flow area. Dark zones with reduced signal intensities due to reversed flow were separated from the outer wall of the internal and external carotid arteries by a thin layer of forward flow along the wall on the source slice image of MRA. The general hemodynamics of the phantoms on MRA were identical to hemodynamics on color Doppler imaging and CFD. The results obtained with the phantoms matched the findings on normal volunteers. CONCLUSION: Although representations of bifurcated carotid arterial phantoms on axial 3D-TOF MRA were excellent if ideally desinged, the zone of reversed flow could be a significant factor in creating distorted image when the zone of reversed flow contacted directly with curved or deformed arterial wall.
