1.A Study of the Isolation of Ureaplasma Urealyticum in Nongonococcal Urethritis.

A group of 180 men who visited Urology Department of Severance hospital, including 115 patients with nongonococcal urethritis (N.G.U.), 27 patients with prostatitis, 13 patients with gonococcal urethritis (G.U.) and 25 healthy medical student controls were investigated for the isolation of Ureaplasma urealyticum (T-strain mycoplasma) from the specimen of urethral discharge, urine and semen. Taylor-Pobinson media of T-broth and T-agar was used for the isolation of Ureaplasma urealyticum. To the best of our knowledge, the study on the culture of Ureaplasma urealyticum was reported for the first time in Korea. The following results were obtained: 1. The isolation rate of Ureaplasma urealyticum in nongonococcal urethritis (53.0%) revealed highest of those in the other three groups of prostatitis, gonococcal urethritis and control (40.7%, 38.4% and 16.0% respectively). 2. As for the specimens, urethral discharge revealed higher isolation rate of Ureaplasma urealyticum (54.6%)than first voided urine (50.0%). 3. The more consorts patients had, the higher positive culture rate of Ureaplasma urealyticum were revealed. The isolation rate in case of more than one causal in nongonococcal urethritis (71.1%) revealed much higher than in case of marital only (42.8%), one regular (41.1%) and one causal (41.0%). 4. 18.7% of isolation rate of Ureaplasma urealyticum revealed in patients with nongonococcal urethritis who visited the clinic in later than 4 weeks after the symptoms developed. However, the isolation rate in patients who visited within 4 weeks revealed 58.9%. The lower isolation rate of Ureaplasma in the late treatment seekers might be probably due to the suppression effect against Ureaplasma urealyticum from the possible previous self antibiotic treatment. 5. Attachment of Ureaplasma urealyticum mostly to the neck and head portion of the spermatozoa seemed to play a role to affect the motility of sperms.

2.Analysis of scientific literatures on the oral &maxillofacial surgery from the view point of informational science: Korean, Japanese, American, and International joms.

In order to examine a way of utilizing the papers published in the field of oral &maxillofacial surgery, the subjects and the references from total 3,392 papers. ; 263 papers in the KJOMS(Journal of Korean Association of Oral &Maxillofacial Surgeons. vol. 17-21), 237 papers in the KJMPRS(Journal of Korean Academy of Maxillofacial Plastic and Reconstructive Surgery. Vol 13-17), 1,233 papers in the JJOMS (Journal of Japanese Association of Oral &Maxillofacial Surgeons. vol 37-41), 1,212 papers in the AJOMS(Journal of American Association of Oral &Maxillofacial Surgeons. vol 49-53), and 447 papers in the International JOMS(Official publication of the International association of Oral &Maxillofacial Surgeons vol 20-25) have been analyzed. The results were as follows: 1. The subjects of KJOMS and KJMPRS were composed of various 28 items. Among 15 main subjects excluding the minor subjects below 1%, there were Aquired deformity &-Reconstruction (17.4%), Jaw deformity (14.2%), Trauma (14.2%) and Malignant tumor (10.8%). 2. The main subjects of JJOMS were Malignant tumor (22.8%) and Benign tumor (20.6%), In AAOMS, Trauma (13.0%), Jaw deformity (13.0%), and malignant tumor 12.4%) were main subjects. In IJOMS Malignant tumor (16.1%), Acquired deformity &Reconstruction (13.0%), and Trauma (10.3%) were main subjects. 3. IN KJOMS and KJMPRS, Scientific and Clinical articles was 63.5% and Case reports was 36.5%. But scientific and Clinical articles was 43.4% and Case reports was 56.6% in JJOMS, 47.1% and 52.9% in AAOMS, 50.1% and 49.9% in IJOMS. 4. The number of institutes in KJOMS and KJMPRS was 46. The main 6 institute published the 57.4% of total articles. In IJOMS, Nationality of the author was composed of 42 nations. West Europe Area published 52.8% of total articles, and The dominant nation of the author were West Germeny (16.3%), U.K. (12.1%), and Japan (11.6%). 5. Authors cited 30.4 references per articles in average, but only 2.7 domestic articles was cited in KJOMS and KJMPRS. It was 17.4 references, but 9.3 domestic articles was cited. In AJOMS and IJOMS, were 19.5 and 18.0 references.

4.Quantitative Measurement of Membrane Na+-K+ ATPase Activity using Thallium-201: Comparison with Rubidium-86.

PURPOSE: Na+-K+ ATPase Activity has beem estimated by the degree of inhibition of cation transport by cardiac glycosides (ouabain) using Rb-86 as a substrate. The biological characterist-Isc of T1-201 is known to be simiIar to those of potassium as a transport substrate in the presence of glucose, insulin or phobol myristate acetate (PMA). The purpose of this study was to measure ouabain sensitive Na+-K+ ATPase activity using T1-201 and compare with that using Rb-86. MATERIALS AND METHODS: Smooth muscle cells isolated from rat aorta or human placental umbilical artery were cultured, and used to measure cellular Na+-K+ ATPase activity. Na+-K+ ATPase activity was measured as a percentage decrease in cellular uptake of T1-201 or Rb-86 by ouabain under the presence of glucose, insulin or PMA in media. RESULTS: Na+-K+ ATPase ase activity measured with T1-201, as a transport substrate, was not different from those measured with Rb-86 in rat or human smooth muscle cell preparation. Incubation with high concentration glucose resulted in about 30% decrease in enzyme activity. In contrast, insulin or PMA resulted in 50-70% or 28% increases from baseline activity, respectively. CONCLUSION: These results suggests that T1-201 could replace Rb-86 in measurement of ouabain sensititive Na+-K+ ATPase activity in vitro. High level of glucose concentration decreased cellular Na+-K+ ATPase activity, but insulin or PMA increased it.

6.Comparison of Na, K between arterial whole blood and venous serum electrolyte analyzer: For clinical use of arterial whole blood analysis of electrolytes.

BACKGROUND: Recently, there are so many newly developed equipments to detect blood gas analysis with serum electrolytes. And then, so many important parameters are quickly reported from laboratory department. So, many clinicians use the data to manage the critical patient. We attempted to detemine the correlation coefficient and adjustment equation. METHODS: Total 68 patients results of Na, K that are cheked using arterial whole blood(288 blood gas system) and venous serum electrolytes analyzer(NOVA 6) were evaluated and we detemined the coefficient of two methods from April to September, 1995. RESULTS: The correlations between the results obtained with NOVA 6 and that of 288 blood gas system were excellent for both Sodium and potassium(r>0.95, p<0.001 in both). In this study, we make the regression equation to adjust the differences between the two methods. Na : y=1.004190X+7.656643(r=0.9637, p<0.001) y=venous serum Na, x=arterial whole blood Na K: y=0.994831X+0.489631(r=0.9545, p<0.001) y=venous serum K, x=arterial whole blood K. CONCLUSIONS: Specimen(whole blood, serum), kind and content of heparin also affect the results of electrolytes. And then, the report of the result after adjustment of them using above equation will provide reinforcement of the efficacy of the results using arterial whole blood specmen. We recommend it especially for emergency laboratories for critical test.

Tumors involving the proximal humerus confront the surgeon with the problem of both eradicating the osseous lesions and restoring shoulder joint function. This problem is magnified because of the biomechanical complexity of the shoulder, its anatomic configuration, and its inherent lack of stabillity. With the increased interest in limb-sparing resection for lesions in the area, efforts are being made to improve the reconstructive techniques. These include leaving a flail shoulder, an being made to improve the reconstructive techniques. These include leaving a flail shoulder, an arthrodesis using intercalery graft between the scapula and the remaining humerus, or if the glenoid is preserved, an arthroplasy. From January 1984 to December 1993, 23 patients with a malignant or locally aggressive bone tumor in the proximal humerus were treated by limb-sparing surgery in Seoul National University Hospital. Histologic diagnosis included osteosarcoma(6), chondrosarcoma(2), Ewing's sarcoma(1), metastatic bone tumor(11), and recurred giant cell tumor(3). Options for reconstruction following limb-sparing resection in our cases were 10 prosthetic arthroplasties, 3 arthrodeses with an living fibula graft, and 10 arthroplasties with intramedullary nailing and cementization. We performed a retrospective analysis in view of tumor eradication and performance status with modified functional evaluation system of Musculoskeletal tumor Society. In 7 cases(74%), good or fair performance status was acquired with average 3.6 years follow-up(6 months-11 years). Each option for reconstruction resulted in relatively good outcome but rather better result was acquired in giant cell tumor and chondrosarcoma. Estimated survival rates by Kaplan-Meier's method, 5 year survival rate was 81% in primary malignant bone tumors(12), 76% in stage Ⅱ B malignancies(5) with 4 years and 4 months follow-up. There was no regional recurrence in primary malignant bone tumors but there were three cases in which distant pulmonary metastases were found in follow-up. Complicationss were one soft tissue infection, two nonunions in arthrodesis with a living fibula graft and one dislodgement of prosthesis in prosthetic arthroplasty. In summary, each option for limb-sparing surgery in malignant bone tumor of proximal humerus might result in rather satisfactory outcome in performance and survival rate when effective preoperative and postoperative chemotherapy were performed in adequately selected cases.
