1.Nonvisuallzing Kidney on Radiography.

Although recently, there have been introduced many new diagnostic tools such as renal scanning and renogram, intravenous urogram remains to be one of the most useful method of urological diagnosis. Therefore it seems warranted to review 363 cases of intravenous urograms performed in the department of urology, St. Mary's hospital, Catholic Medical College during the period of January, 1962 to June, 1964. Among others, especial attention was paid to 60 cases of nonvisualizing kidneys. Of 60 nonvisualizing kidneys, renal tuberculosis occupied 27 cases(45%); ureteral and renal stones 13(21.6%); hydronephrosis 9(15%) (metastatic carcinoma 6(10%)and surgical manipulation 3(5%); Wilms' tumor 5 (8.3%) chronic atrophic pyelonephritis 3(5%) ; One case each of renal cell carcinoma, cystic kidney and thrombosis of the renal artery. Inasmuch as the present study revealed renal tuberculosis to be the most common cause of nonvisualizing kidneys, it is felt that one should always consider the possibility of tuberculosis in the diagnosis of nonvisualizing kidneys in Korea. This fact should be stressed because nontuberculous pathology such as chronic pyelonephritis has been implicated to be the most frequent cause of nonvisualizing kidneys by American authors.

2.Clinical experience with protermycin in genitourinary diseases.

Protermycin, the first molecular synthetic of tetracycline and chloramphenicol, has been given trial in 28 patients: acute gonococcal urethritis 10, nongonococcal urethritis 7, acute cystitis 6, chancroid 1 and postoperative cases 4. The doses ranged from 300 to 600 me daily. From this study, it was found that Protermycin proved itself to be most effective in nongonococcal urethritis which could not be treated satisfactorily by other antibiotics. Of 28 cases, 25 showed good result. The remaining 3 cases failed to respond.

3.Air Shadow Containing Mass Lesion in the Chest.

A 70-year-old female Patient was admitted due to the impression of lung abscess via local clinic. Chest P-A showed air shadow containing huge mass lesion in the right lower lung field, pericardio phrenic angle. But the patient complained of only mild upper respiratioy tract infection symptoms and the laboratory tests were within normal limits. Barium enema and chest C-T taken right after barium enema were performad in suggesting of the diaphragmatic hernia. Barium tilled transverse colon and associated omentum were in the right anterior hemithorax surrounded by the hernial sac. The mass lesion shown in the chest P-A was compatible with the diaphragmatic hemia, Morgagni type. Operative reduction of the herniated bowel and simple closure of the diaphragm was performed.

4.Comparison of Clinical and Structural Outcomes of Open and Arthroscopic Repair for Massive Rotator Cuff Tear.

BACKGROUND: Management of massive rotator cuff tears can be challenging because of the less satisfactory results and a higher retear rate regardless of the use of open or arthroscopic repair technique. METHODS: We retrospectively analyzed 102 cases of massive rotator cuff tear treated with either open or arthroscopic repair. Open repair was performed in 38 patients; and arthroscopic repair, in 64 patients. The mean age at the time of surgery was 59.7 years in the open group and 57.6 years in the arthroscopic group. RESULTS: The Constant score increased from the preoperative mean of 55.9 to 73.2 at the last follow-up in the open repair group and from 53.8 to 67.6 in the arthroscopic repair group (p<0.001 and <0.001, respectively). The University of California at Los Angeles (UCLA) score increased from a preoperative mean of 17.7 to 30.8 at the last follow-up in the open group and from 17.5 to 28.7 in the arthroscopic group (p<0.001 and <0.001, respectively). No statistically significant difference in the Constant and UCLA scores was observed between the two groups at the last follow-up (p=0.128 and 0.087, respectively). Retear was found in 14 patients (36.8%) in the open group and 39 patients (60.9%) in the arthroscopic group (p=0.024). CONCLUSIONS: Open and arthroscopic repairs of massive rotator cuff tears may provide satisfactory clinical results with no significant difference. However, a significantly lower retear rate was observed for the open repair group compared with the arthroscopic repair group.

5.Metabolism of C(14)-glucose by plerocercoid of Diphyllobothrium sp..

The glucose uptake rate by plerocercoid of Diphyllobothrium sp. was a mean value of 5.35+/-0.80 micro-mole/hr/g of wet wt, and total CO(2) production rates by the plerocercoid larva averaged 7.54+/-0.73 micro-mole/hr/g of wet wt. The relative specific activity into respiratory CO(2) showed a mean value of 7.30 +/-0.90 per cent. The rate of CO(2) production derived from medium C(14)-glucose was a mean of 0.58+/-0.13 micro-mole/hr/g of wet wt. Therefore, the average value of 1.92+/-0.38 per cent of glucose utilized by the larvae from the medium C(14)-glucose was oxidized to respiratory CO(2). The tissue concentration of glycogen in plerocercoid larva was a mean of 46.28 +/-2.23 mg/g or 4.63+/-0.22 per cent/g of wet wt., and the turnover rate of glycogen pool was a mean of 0.049 +/- 0.012 %/hr or 0.010 +/- 0.003 mg/hr/g of wet wt. The average value of 2.76+/-1.00 per cent of glucose utilized by the larvae from the medium C(14)-glucose was incorporated to the glycogen. These data accounts for that only 5 per cent of the utilized glucose by the plerocercoid larvae participated in furnishing the oxidation into respiratory CO(2) and the synthetic process into glycogen.

6.Metabolism of C(14)-glucose by Fasciola hepatica.

The glucose uptake rate by Fasciola hepatica was a mean value of 9.62 +/- 0.54 micro-mole/hr/g, and total CO(2) production rate by the flukes averaged 24.28 +/- 4.26 micro-mole/hr/g wet wt. The relative specific activity of respiratory CO(2) showed a mean value of 79.89 +/- 1.78 per cent. The rate of CO(2) production derived from medium C(14)-glucose was a mean of 19.55 +/- 3.56 micro-mole/hr/g of we wt. Therefore, the average value of 32.72 +/- 4.8 percent of glucose utilized by the flukes from the medium C(14)-glucose was oxidized to respiratory CO(2). The tissue concentration of glycogen in F. hepatica was a mean of 38.36 +/- 2.91 mg/g or 3.84 +/- 0.29 %/g of wet wt, and the turnover rate of glycogen pool was a mean of 1.6+/-0.22 %/hr or 0.65 +/- 0.13 mg/hr/g. The average value of 37.26 +/- 3.86 per cent of glucose utilized by the fluke from the medium C(4)-glucose was incorporated to the glycogen. These data account for that approximately 70 per cent of the utilized glucose by the flukes participated in furnishing the oxidation into respiratory CO(2) and the synthetic process into glycogen.

7.Radiation recall dermatitis induced by tamoxifen during adjuvant breast cancer treatment.

Tamoxifen and radiotherapy are used in breast cancer treatment worldwide. Radiation recall dermatitis (RRD), induced by tamoxifen, has been rarely reported. Herein, we report a RRD case induced by tamoxifen. A 47-year-old woman had a right quadrantectomy and an axillary lymph node dissection due to breast cancer. The tumor was staged pT2N0; it was hormone receptor positive, and human epidermal growth factor receptor 2 negative. The patient received adjuvant chemotherapy followed by tamoxifen and radiotherapy. After 22 months of tamoxifen, the patient developed a localized heating sensation, tenderness, edema, and redness at the irradiated area of the right breast. The symptoms improved within 1 week without treatment. Three weeks later, however, the patient developed similar symptoms in the same area of the breast. She continued tamoxifen before and during dermatitis, and symptoms resolved within 1 week.

8.RECONSTRUCTION WITH METAL PLATE AND ILIAC BONE GRAFT ON AMELOBLASTOMA.

This is to report a case of immediate reconstruction after hemimandibulectomy by using of bicorticocancellous block bone harvested from the iliac crest in the case of an ameloblastomaon the mandible. Because the lesion involved condylar area, it was reconstructed with titanium artificial condyle attached to A/O metal plate. Three weeks after the operation, infection developed with suppuration and was well treated with adequate antibiotic therapy and drainage. The patient has been followed up over a four-year period and taken an orthopantomogram every three or six month for the examination of mandibular movement, the potentiality of recurrence and the remodeling of the grafted bone. At present, the patient is satisfied with her appearence and has a normal occlusion with proper masticatory function, and there is no sign of recurrence.

10.Air Pollution Has a Significant Negative Impact on Intentional Efforts to Lose Weight: A Global Scale Analysis.

BACKGROUND: Air pollution causes many diseases and deaths. It is important to see how air pollution affects obesity, which is common worldwide. Therefore, we analyzed data from a smartphone application for intentional weight loss, and then we validated them. METHODS: Our analysis was structured in two parts. We analyzed data from a cohort registered to a smartphone application in 10 large cities of the world and matched it with the annual pollution values. We validated these results using daily pollution data in United States and matching them with user information. Body mass index (BMI) variation between final and initial login time was considered as outcome in the first part, and daily BMI in the validation. We analyzed: daily calories intake, daily weight, daily physical activity, geographical coordinates, seasons, age, gender. Weather Underground application programming interface provided daily climatic values. Annual and daily values of particulate matter PM10 and PM2.5 were extracted. In the first part of the analysis, we used 2,608 users and then 995 users located in United States. RESULTS: Air pollution was highest in Seoul and lowest in Detroit. Users decreased BMI by 2.14 kg/m2 in average (95% confidence interval, −2.26 to −2.04). From a multilevel model, PM10 (β=0.04, P=0.002) and PM2.5 (β=0.08, P < 0.001) had a significant negative effect on weight loss when collected per year. The results were confirmed with the validation (βAQI*time=1.5×10–5; P < 0.001) by mixed effects model. CONCLUSION: This is the first study that shows how air pollution affects intentional weight loss applied on wider area of the world.
