2.A Study of Psychiatric Accidents in Korea.

Followings are results from analyzing 263 responses out of 1,000 survey questionaires, which were mailed to Korean psychiatrists, for purpose to discover current status of psychiatric accidents in Korea, and to find appropriate solutions. 1) More accidents were experienced by 30-40 years-old psychiatrists than any other age level. More experience in practice they had, more accidents were experienced. 2) 74.5% of responding psychiatrists reported that they had experienced accidents. The rate has steadily increased. 3) Quite a few accidents were involved with schizophrenic patients, who were between 20-30 years of age level 4) 58% of reported accidents were suicides, which was the highest rate. In legal suit regarding malpractice, the highest rate of 47.9% were suicides, as well. It calls special attention to careful evaluation and treatment of suicidal patients. 5) More accidents tended to be dealt in private rather than in legal. Accidents tended into legal suits asked for more compensation. 40.4% of accidents upset patient's family to take actions. It is suggested that Special Case Settlement Policy be established and Accident Dispute Intervention Committee be actively involved for efficient solutions against accidents.

3.Relationship between Health Behavior and Subjective Unhappiness in High School Students.

OBJECTIVES: We studied the relationship between health behavior and subjective unhappiness in high school students. METHODS: Using 27,097 responses from the 2015 Korea Youth Risk Behavior Web-based Survey for general high school students. we analyzed by multiple logistic regression based on the complex sample design. RESULTS: Unhappiness was positively related with the low economic status, smoking, drinking, fast foods intake, and negatively related with fruit intake in results of multiple logistic regression. CONCLUSIONS: Health behaviors have a significant impact on the unhappiness of high school students. Therefore, in-depth research and policies to decrease unhappiness of high school students through health promotion are required.

5.A Study of Murder in Schizophrenic Patients Characteristics of the murders of family and nonfamily homicides.

Sixty schizophrenia patients who committed homicide and detained after January 1, 1993 were divided in two groups, depending on whether their victim was a family member or not. Below conclusions were deduced from the comparison of the two groups in terms including demographic characteristics, onset and duration of illness, IQ score, suicidal attempts, history of alcohol/drug abuse or past crime, neuropsychiatric ward admission history, living circumstance, conflict with the victim, and adolescent conducting behavior. 1. More in the family homiciding patients (FHP) group lived alone due their unmarried, divorced, or separated marital status than those in the non-family homiciding patients (NFHP) group. More in the FHP group were in married state than those in NFHP group. 2. More in the NFHP group stayed alone apart from their family, while more in the FHP group lived with their family. 3. Those in the NFHP group show more frequent suicidal attempts than those in the FHP group, implying they have difficulties in the control of their impulsive behaviors. 4. To prevent the crime, it is essential for the government to practice the systematic support and care of the schizophrenia patients who are living alone, especially those without relative.

6.Treatment of the Humeral Shaft Fracture: Comparison between Functional Bracing Technique and Closed Intramedullary Fixation with Functional Bracing Technique

It has been generally agreed that most fractures of the humeral shaft are best treated non-operatively although occasionally there are indications for primary operative treatment. Recent advances in internal fixation techniques and instrumentation have led to an expansion of surgical indications, and operative treatment produces higher rates of nonunion, infection and other associated complication when compared with closed treatmet techniques. Because of the significant morbidity of the operative treatment, closed intramedullary fixation techniques that provide stability of the fracture site without opening and reduce the postoperative complications, were developed. In this paper we document forty two fractures of the humeral shafts that were treated by functional bracing(Group I: 24 cases) and intramedullary fixation with functional bracing(Group II: 18 cases) between October, 1982 and August, 1986. And the obtained results were as follows: 1. The everage healing time was 9 weeks in the Group I, and 11 weeks in the Group II . 2. The non-union has not been encountered in the Group I, but non-union and delayed union were 4 cases (22.2%) in the Group II. 3. The complication rate was 9 cases(37.5%) in the Group I, and was 12 cases(66.7%) in the Group II, but deep infection did not occured in all cases. 4. The residual angulation more than 5 degrees was 20 cases(83.3%) in the Group I, and was 2 cases(11.1%) in the Group Il. However the results of the group Il were inferior ot the group I, of more proper selection of the patients, more accurate technique and adequate type of nails have been used, the better results will be taken.

7.Effect of Lumbar Extensor Strengthening in Chronic Low Back Pain Patients.

OBJECTIVE: To evaluate the effects of strengthening exercise for isolated lumbar extensor muscles on chronic low back pain patients. METHOD: 28 patients with chronic low back pain in back school program participated in this study. Initially, isometric lumbar extensor strength, 10 point of Visual analog scale (VAS) and questionnaires related to self-experienced symptoms and daily activity (ADL) were checked. Isometric lumbar extensor strength test and strengthening exercise were performed with MedXTM lumbar extensor machine, which was designed to stabilize the pelvis. After 8 weeks of training program, isometric lumbar extensor strength & other pain related variables were checked again in the same way. RESULTS: The results were as follows: 1) After 8 weeks of lumbar extensor strengthening program, isometric lumbar extensor strength was increased at all the measured points significantly (p<0.01). 2) VAS and ADL were improved from 6.7 to 3.7, and from 6.9 to 9.1 respectively (p<0.05). CONCLUSION: 8 weeks of lumbar extensor strengthening program with pelvic stabilization was effective therapeutic tools for patients with chronic low back pain.

8.An Epidemiologic Investigation of Shigellosis Outbreak in Gangnung 1998.

An epidemiologic investigation was carried out to identify the distribution and the cause of Shigellosis outbreak, which was occurred in Gangnung city from August to November, 1998. This study carried out from September, 14, to November, 20, 1998. The total number of Shigellosis confirmed patient is 87. We surveyed and obtained the stool sample from all suspected patients and their members of household. The major findings of this study are as follows. 1. The first patient appeared in September 11 and the number of patients increased to September 13, that is peak time, and the patient number tailed down apparently from September, 18, 1998. 2. The incidence rate per 1,000 is largest in Wangsan-Myeon Doma-Ri(32.9), and second largest in Wangsan-Myeon Mokgea-Ri(31.6), The total incidence rate in Gangnung-shi is 0.4(0.4 for man and 0.3 for woman). The age group with the largest incidence rate per 1,000 is 5-14 age group whose incidence rate is 1.4. 3. The secondary generation attack rate in confirmed patient family is 20.6%, and the secondary attack is 7.8%. 4. We considered the meal of school, environmental factor, milk, carrier of Shigellosis as the cause of outbreak, but we were not able to identify the bacteria of shigellosis. But we suspected the meal of school that played important role in outbreak, because outbreak occurred explosively after students ate the meal of school. 5. We considered three hypotheses for the cause of the outbreak. First, a propagation from external regions; Second, an outbreak from contaminated foods; Third, a propagation from carriers in Gangnung. In order to obtain the further information for first and third hypothesis, we should perform PFGE(Pulsed-field gelectrophoresis) with the shigella bacteria of each epidemic region.

10.Crystalloid Resuscitation in Experimental Hemorrhagic Shock in Dogs.

Although crystalloid solutions such as Ringer's lactate have become the standard for initial volume replacement after hemorrhage, it is controversy whether this fluid alone restores and maintains the depressed physiologic function after severe hemorrhage. Six anesthetized dogs were bled(35ml/kg) and resuscitated with Ringers lactate solution (100 ml/kg). Physiologic variables are measured initialy, after hemorrhage, after resucitation. Compared with initial value, after resuscitation data show marked reduction in hemoglobin, oxygen carrying capacity, vascular resistance, and electrolyte. But cardiac output and blood gas analysis data was not changed. Prothrombin time and partial thromboplastin time was prolongated. From this study crystalloid resuscitation in severe hemorrhagic shock restore cardiac output and perfusion, but fail to maintain adequate oxygen carrying capacity.
