4.Expression of bcl-2 and p53 Protein in Primary Gastric Lymphomas.

The bcl-2 gene is a proto-oncogene which extends cell survival by blocking apoptosis. Bcl-2 expression has been detected in many types of nodal and MALT lymphoma. The p53 gene is a tumor suppressor gene and p53 mutation is the most common genetic alteration in human malignancies. The relationship between the expression of bcl-2 and p53 protein in primary gastric lymphoma has been rarely reported. The authors investigated the expression of bcl-2 and p53 protein in 37 cases of primary gastric lymphoma by immunohistochemical method using bcl-2 and p53 monoclonal antibodies. There were five cases of low grade B-cell MALT lymphomas and thirty two cases of high grade B-cell lymphomas. Fifteen of 37 cases (41%) showed bcl-2 protein expression in the cytoplasm of tumor cells and 26 cases (70%) showed p53 protein expression in the nucleus of tumor cells. Bcl-2 protein was detected in 4 of 5 (80%) low grade MALT lymphomas, and in 11 of 32 (34%) high grade lymphomas. There was no significant correlation between bcl-2 expression and histologic grade of primary gastric lymphomas (p>0.05). p53 protein was positive in 25 of 32 (78%) high grade lymphomas, and in 1 of 5 (20%) low grade MALT lymphomas. The expression of p53 protein is significantly higher in high grade lymphoma than in low grade MALT lymphoma (p<0.05). The p53 expression in the bcl-2 negative cases (86%) was significantly higher than in the bcl-2 positive cases (47%). There was an inverse relationship between bcl-2 and p53 expression in primary gastric lymphoma. These results suggest that bcl-2 and p53 expression in primary gastric lymphoma may be involved in the transition from low grade MALT lymphoma to high grade lymphoma.

6.A Case of Trichophyton rubrum Infection showing Unusual Clinical Manifestation.

Trichophyton rnbrum is a.nthropophilic, and the most common and widely distributed dermato phyte of man. Characterisic lions are circular, usually sharplyn ginated, slightly erythematous, dry, scaly patches with a raised edge. Progressive central clearin produces annular outlines. Other variants are reported, such as granulomatous, bullous or verucius. We reported a case of Trichophyton rubrum infection showing an unusual clinical manifest tin. The skin leison showed multiple discrete brownishouni macules with a fine scale on both hi lower legs, dorsum of feet and buttocks. Central clearing and inflammed border were not not. On KOH mount, fungal hyphae were noted and Techophyton rubrum was isolated on the funu culture.

7.Cutaneous Infection by Fusarium solani in a Patient with Burger's Disease.

Fusarium species are known as ubiquitous soil saprophytes and human skin comtaminants. Occasionally, however, they have been reported to cause a variety of infections in humans. We report herein a case of cutaneous infection by Fusarium sokmi in a patient with Burger's disease involving his right lower leg. He was treated with itraconazole 200 mg p.o. per day for 5 weeks and the skin lesion resolved without any noticeable side effects. Furthermore, there has been no evidence of recurrence of the disease 9 months after discontinuing the itraconazole. This is the first reported case of cutaneous infection by Fusarium sokmi in a patient with Burger's disease and itraconazole was an effective drug in our case.

9.A Clinical Observation on Injuries of Genito-urinary Tract.

This is to report a clinical observation on 81 cases of genito-urinary tract injuries treated at the urology service. Capital Army Hospital, during the period from January, 1960 to July, 1963. Of 81 Cases which stood for 0.82% of all genito-urinary out patients, 58 cases (or 71.6%) occurred in the urethra, the most common site of involvement, and 6 cases (or 7.2%) showed multiple injuries. Age ranged between 21 and 10 years with the highest incidence of 58.4% in the ages between 21-25 years. In lateralyzation of injury while the kidney showed nosignificant difference the ureter was more frequently involved in the left than in the right with a ratio of 4:1. Traffic accident was listed as high as 56.9% to be most frequent cause of injury and contusion, fall, gun-shot and explosion were listed less frequently. Of associated major injuries, pelvic fracture was seen in 20 cases (33.9%) and all the urethral injuries were accompanied with injuries to gastro-intestinal tract. Of all cases herein reported, 62.3% of cases were treated surgically and the remaining 37.7%. non-surgically. Surgical intervention was necessary in 33.3% of kidney injury, 100% of ureteral injury, 67.8% of urethral injuries and 40% of genital injuries. Average hospitalization required 20.2 days for renal injury and 67.2 days for urethral injury.

10.Surgical gems in dermatologic surgery I.

A bolster, simply defined, is any device used to support or cushion. In surgery, bolsters have been used under sutures to reduce skin and subcutaneous tension. Bolsters function to reduce the potential for necrosis and laceration by distributing the wound closure tension over a broader surface area. Pexing stitich is placed between the mobilized tissue and the deep fascia and periosteum to prevent the secondary motion of closure from disrupting anatomic landmarks and moveable structures. The technique aids in obtaining linear closures, prevents distortion of free margins, prevents tenting across concavities, and fixes the closure line at the junction of cosmetic units. Standard treatment of lipoma commonly includes incision with extrication or elliptical excision. Unfortunately, these treatments often elicit a large scar. Using pressure technique, risk of hemorrhage and infection may be minimized secondary to the decreased degree of invasion. In cosmetically sensitive areas, this technique is an effective and aesthetically pleasing method for removal of lipoma.
