2.Research on the frequency of Terry's nail in the medical inpatients with chronic illnesses.

Terrys nail(TN) might be causally associated with cirrhosis or other disorders, but his criteria have been proved to be inadequate reflection of systcmic disorders. Modified criteria of Holzberg and Walker were introduced in TN examination in 1984. Using these criteria, fingernails were studied in 444 medical inpatients with chronic systemic diseases at Taegu Catholic Hospital from March 1991 to March 1992. There were 136(30.6%) patients with TN and 308(69.4%) patients without TN, respectively. Ninety-three patients with TN showed a signifiart association with cirrosis(49/86, 57%, p<0.01 by the x-test), congestive heart failure 18/35, 515, p<0.05), and diabetes mellitus(25/51, 49%, p<0.05), and otherwise not;ignificantly associated with chronic renal failure(19%), and cancer(18%). Male and female patients with TN were affected about equally. Age distribution in The patients with TN showed 35(25.8%) the age group from twenty to fortynine and 101(74.2%) in the age group from fifty and above with a mean age of 57.3+14.0, whereas the patients without TN had 178(57.8%), and 130(42.2%), with 47.4+16.9, respectively. The greater part was distributed in the older group(p<0.001 by student t-test), suggestive of an increased risk in older group. The average number of nails affected per patient was tending to be higher in frequency close to the thumb, with an equal frequency in six. Thirty-nine patients(28.7%) had all nails affected. The distal band with was 0.5 to 3.0mm averaging 1.0+0.5mm with no difference, in sex. The distal band was pink in 43(31.6%) of the 136 patients with TN, brown in 62(45.6%) and intermediate in color in 13(22,8%). The results of routine laboratory test such as hemogram, eiythrocyte sedimenation rate, liver enzyme, triglyceride, protein, albumin, blood urea nitrogen and creatinine were statistically insignificant between the patients with and without TN.

3.Effect of High-dose Intrevenous Immune Globulin in the Treatment of Neonatal Immune Hemolytic Jaundice Unresponsive to Phototherapy.

OBJECTIVE: Immune hemolytic jaundice is caused by the destruction of antibody-sensitized erythrocytes and is associated with antibody-dependent cellular cytotoxic effects mediated by Fc receptor-bearing cells of the reticuloendothelial system. Intravenous immune globulin(IVIG) may have exerted its effect through Fc receptor blockade. We studied the effect of high-dose intravenous immune globulin(HDIVIG) in neonatal hemolytic jaundice unresponsive to phototherapy. METHODS: We selected only those with Coombs test(+) immune hemolytic jaundice who had admittcd at the NICU of the Dcpartment of Pediatzics of Dongsan Medical Center, Keimyung University between January 1995 and December 1998. They were unresponsive to phototherapy. Ten newborn infants(9 ABO incomplatibilities, l minor group incompatabillity due to anti-E) received HDIVIG therapy combined with phototherapy. IVIG was given as a dose of lg/kg for 6 hours, and serial hemoglobin, reticulocyte count, and bilirubin levels were evaluated. If the serum bilirubin level went up and reached the level above 22mg/dl, we conducted exchange transfusion for the patient. RESULTS: HDIVIG induced a significant decrease of serum billirubin levels in 8(80%, group I, HDIVIG responsive poup) of 10 cases and only 2 cases(group II, HDIVIG unreponsive group) required exchange tnnsfusions. No side effect was observed after HDIVIG therapy. CONCLUSION: We suggest HDIVIG may be effective in the treatment of phototherapy-resistant hyperbilirubinemia due to blood group incompatibility. More studies are needed to confirm the optimal dosage and therapeutic indication of HDIVIG in the therapy of neonatal immune hemolytic jaundice.

7.Arthroscopic Adhesiolysis of Partial Knee Ankylosis

Partial knee ankylosis is a recognized complication of following open operative procedure, trauma and disease processes about the knee. Intraarticular trauma or surgical procedures such as TKR, synovectomy or patellar fracture were main source of intraarticular or extraarticular soft tissue adhesion and contracture were resulted from distal femoral and proximal tibial fractures. The purpose of this study is to report the value of arthroscopic adhesiolysis of partial knee ankylosis and to evaluate the clinical results of its application in various conditions. From October 1982 to December 1992, 56 cases in 54 patients who had severe limitation of motion in the knee joint following open operative procedure or trauma about the knee were treated by fibroarthrolysis under the arthroscopic control. For release of thick fibrous tissue, we used our designed blunt metal bar. The following results were obtained. There were 30 males and 24 females and the age at the time of release ranged from 20 to 69 years old(average 38.7 years old). The interval between last open operative procedure or trauma and arthroscopic adhesiolysis ranged from 3 months to 6 years(average 16 months). The interval between arthroscopic adhesionlyssis and the last follow up evaluation ranged from 1 year to 9 years(average 49 months), The average preoperative range of knee motion was 42.2 and the postoperative range of motion under the anesthesia was 115.5 and the average final knee range of motion at follow up was 101.3. So the average loss of motion between postoperative and final motion was 14.2. Complications were one case of patellar and tibial condylar fracture respectively. In the author's experience, arthroscopic adhesiolysis seems to be applied to the various condition of the partial knee ankylosis and the results are better than the other surgical procedures when it was performed early enough16).

10.The Study on the Menisci of Korean Adults

The menisci or semilunar cartilages of the knee provide the function of mechanical lubrication and they are particularly important in allowing rotational movements of the knee joint. The study was carried out on 123 Korean adult knees for measurement of the menisci and observation of surrounding ligamentous structures through anatomical dissection of formalin embalmed cadavers. Detection of abnormal meniscal lesions was also included in this study. The following results were obtained: 1. A-P diameter, transverse diameter, maximal width and thickness were measured in 123 knees of 62 cadavers. 2. Among 62 cadavers,' the anterior menisco-femoral ligament was present in 11 subjects (17.7%), the posterior menisco-femoral ligament in 59 (95.2%), and the transverse ligament in 36 (58.0%). 3. Discoid lateral meniscus was found in 5 knees (4.1%), and tear was associated in 3 of them. 4. Meniscus tear was found in 13 knees (10.6%), and the site of tear was medial in 9 knees and lateral in 4 including 3 torn discoid menisci. The oblique pattern of tear was most common.
