Eccrine spiradenoma is a txnign, painful tumor of the skin, which probably originates in the coil of the eccrine sweat glands and presents a characteristic clinical and pa.hological picture. However recently, the weakness and inconsistency of the enzymes histochemican, actions and electron microscopic findings, and the presence of numerous undifferentiated and indetirn, inate cells suggest a rather low degree of differentiation. We present herein four cases of eccrine spiradenoma which show livrse histopat.hologic characteristics including typical rosette-like structure, ductal structure, vacuar structure and mixed type, respectively. This report may support the premise that eccrine spiridinoma is defived not only from the eccrine duct but also from pluripotential stem cells.

3.Experimental Aconitine Optic Neuropathy.

Effects of aconitine on the retina and optic nerve were studied in rabbits by means of visual evoked potential(VEP), electroretinogram(ERG), and electron microscopy. Aconitine(0.4mg/kg) was administered intra peritoneally. The effects of aconitine were observed at 1 day, 3 days, 1 week, 2 weeks, 3 weeks, and 3 months after injection, respectively. The amplitude of VEP was decreased significantly at 1 and 2 weeks after injection. However ERG b-wave, c-wave, and the latency of VEP were not significantly changed during the experimental periods. Histopathological changes in the optic nerve and visual streak were characterized by disorganization of the lamellar structures of the myelin sheath, diffuse mitochondrial swelling, and vacuolization in the myelinated nerve fibers, which were increased in degree with time. No perceptible change in the retina including retinal pigment epithelium was observed in the experimental periods. Schwann cells and unmyelinated nerve fibers appeared unaffected, except for mild swelling of mitochondria. Above mentioned electrophysiological and electron microscopic studies indicated that the effect of aconitine on the rabbit eye was toxic myelo-optic neuropathy.

4.A Case of Bowen's Disease Partially Responded to Photodynamic Therapy.

Photodynamic therapy(PDT) is a treatment modality by highly reactive oxygen intermediates generated through the interaction of light with a photosensiziter. It has been shown to be an effective treatment for various cutaneous and noncutaneous malignancies. It is efficient for the curative and palliative treatment of epithelial skin tumor in situ or early invasive lesions. In effect, it is a useful alternative treatment for the lesions located on anatomically difficult areas or the large-sized lesions. We treated a case of Bowen's disease arising on the plantar area and 3rd and 4th toewebs of left forefoot in a 61-year-old man with PDT using the hematoporphyrin derivative, porfirmer sodium(Photofrin, Russia) as a photosensitizer and gold vapor laser as a visible light source. The outcome showed partial clinical improvement after about 2 months' follow-up.

5.Doppler echocardiographic assessment of left ventricular function in Kawasaki syndrome.

To investigate the effect of Kawasaki syndrome on left ventricular function, we studied 52 patients with Kawasaki syndrome at initial visit and after 3 months (36 patients). Using Pulsed Doppler echocardiogram,we obtained aortic velocity (peak and mean), acceleration time(AT),ejection time(ET), ratio of AT to ET(AT/ET), acceleration (peak and mean) and velocity time integral and mitral velocity of E and A waves(peak and mean) and velocity time integral. Mitral time for peak velocity time integral. Mitral time for peak velocity was significantly prolonged in Kawasaki syndrome,being a mean(+/-SD) of 66.2(+/-14) msec in the control group, 79.2(+/-13)msec at initial vist(p<0.05) and 79.4(+/-13) msec after 3 months (p<0.05). Aortic peak acceleration was significantly decreased in Kawasaki syndrome being a mean(+/-SD) of 2590(+/-785) cm/sec2 after 3 months (P<0.05).Aortic mean acceleration was also significantly decreased in Kawasaki syndrome being a mean(+/-SD) of 1575( +/-542)cm/sec2 in the control group, 1198(+/-351)cm/sec2at initial visit(p<0.05)and 1124 +/-275cm/sec2 after 3 months(p<0.01). Aortic acceleration time was significantly prolonged in Kawasaki svndrome being a mean(+/-SD) of 62(+/-13) msec in the control group, 72(+/-13) msec at initial visit(p<0.05) and 76(+/-16) msec in the control group, 72(+/-13) msec at initial visit(p<0.05) and 76(+/-16) msec after 3 months (p<0.01). We conclude that early abnormalities of left ventricular function, as assessed by echocardiograpy,gencrally persist after 3 months of onset.

6.Investigation of hemostatic disorders in patients with congenital heart diseases undergoing open heart surgery at Hue Central Hospital

Background: Open heart surgery with cardiopulmonary bypass (CPB) causes haemostatic abnormalities which result in postoperative excessive bleeding. Objectives: To investigate haemostatic disorders before - after CPB and postoperative bleeding. Subjects and methods: Sixty congenital cardiac patients with and without cyanosis were recruited consecutively. Hematology and coagulation tests were done 1 day before operation, 15 minutes after protamine administration, 2 and 6 hours after the operation in the intensive care unit (lCU). Mediastinal chest tube drainage (MCTO) was measured for the first 6h in the ICU. Results: Significant differences between 2 groups could be found for red blood cells, hemoglobin, hematocrit, fibrinogen, D-dimer, fibrinogen degradation products (FOP), von Kaulla and platelet aggregation to epinephrine before operation (p < 0.05). There were not significant differences in platelets, PT, APTT, platelet aggregation to adenosine diphosphate (AOP) in the acyanotic and cyanotic patients (p > 0.05). Most hematology parameters were decreased significantly and hemostasis measurements were prolonged after operation in both groups (p < 0.05). There was also the significant difference in MCTO between 2 groups (p < 0.05). 7.A Case of Clear Cell Sarcoma of the Tendon and Aponeurosis.

Clear cell sarcoma of the tendon and aponeurosis was first described by Enzinger in 1965, and has become a well-accepted clinicopathological entity as malignant melanoma of soft parts. Although it posesses the ability to produce melanin, it is quite different from conventional melanoma in that, it is more deeply located, usually associated with tendon and aponeurosis, displays pale-staining round to oval-shaped tumor cells and has a longer clinical course and better prognosis.Several cases have been reported in Korea, and we experiened an additional case of this rare tumor that showed ulceration.

8.Exploring medical educators’ readiness and the priority of their educational needs for online teaching

Purpose:
This study investigated medical educators’ readiness for online teaching by exploring their perceived ability and importance of online teaching competencies and identified the highest priority of their educational needs.
Methods:
In this study, 144 medical education faculty members from a university were invited to participate. The faculty online teaching readiness scale was virtually distributed at the end of the spring semester of 2020 and 38 faculty members responded for 2 weeks. The collected data were analyzed with descriptive statistics, paired t-tests, Borich Needs Assessment, and the Locus for Focus model.
Results:
The overall average perceived ability was 2.76, while the overall average perceived importance was 3.36. The course design and the technical competency categories showed the highest and lowest educational needs, respectively. Five competencies were given the highest priority of educational needs.
Conclusion
The results revealed that the medical educators are not ready for online teaching; thus, urgent educational needs for online teaching competencies exist.
9.Emergency Free Flap Transfer for Deep Burn and Severe Crushing Injury of Dorsum of the Hand.

The accepted method in treating deep burns and severe crushing injuries of dorsum of the hand is serial debridement and delayed closure. Delayed wound closure with skin graft, local flap or regional pedicle flap may produce joint stiffness, tendon adhesion, and immobility. Emergency free flap transfer suggests against these concepts by advocating radical debridement and early closure of these wounds. Early mobilization of the joints with emergency free flap transfer may produce better range of motion than delayed closure. The key to success in early coverage of these wounds is thorough debridement while sparing vital structures, such as nerves, tendons, and intact vessels. We discussed 3 cases, which were performed emergency free flap transfer using lateral arm flap in severe crushing injuries and deep burns of dorsum of the hand. All flaps survived without complications. Each joints had shown 95% range of motion of contralateral normal side on the average. Emergency free flap transfer allows early closure of acute soft tissue defect of dorsum of the hand, promoting early motion and possibly reducing the incidence of post-operative infection, flap failure and secondary operative procedures, and improving functional results.

10.Solitary Fibrous Tumor A clinicopathologic review of five cases.

We experienced five cases of solitary fibrous tumor; two in the pleura, two in the orbital soft tissue, and one in the lung parenchyma. Three patients were male, and the age of the patients ranged from 38 to 71 years (mean age: 53.6). Grossly, the masses were well circumscribed and had varying sizes from 2.5 to 30.0 cm. The cut surfaces were grayish-yellow firm with focal variegated hemorrhage, necrosis, cystic change, and myxoid area. Microscopically, these were characterized by a haphazard proliferation of spindle cells or polygonal cells separated by variable amounts of hyalinized collagen and showed a prominent vascular channels reminiscent of hemangiopericytoma in foci. Immunoperoxidase stains showed a strong reactivity for CD34, and were weakly positive for vimentin. Electron microscopical examination revealed features of fibroblast; spindle to round tumor cells were arranged in groups and surrounded by collagen. Nucleoli were seldom prominent. The cytoplasm contained many microfilaments and a moderate number of cisternae of rough endoplasmic reticulum.
