1.Observation about Dapsone-Resistant Cases of Leprosy Partients by Clinical and Bacteriological Aspects.

Fifteen hundred seventy-one leprosy patients registered in the leprosy Mission Hospital from Mar. 1957 to Sep. 1980 were investigated for dapsone resistance by clinical and bacteriological aspects. The results are as follows: There were 29 suspected dapsone-resistant cases out of a total 1571 registered leprosy patients (1. 85%), 26 of 650 of lepromatous type(4. 00%), and 3 of 164 of borderline type (l. 83%). In 1969, we observed the first case if suspected dapsone-resistant leprosy. Since then, suspected dapsone-resistant cases increased and we observed as many as 8 cases in 1977. But frequency of cases has decreased since 1978. (countinued..)

3.Two Cases of Bullous Dermatitis Herpetiformis.

Dermatitis herpetiformis occurs rarely in childhood and certain features of the disease in children differ from its manifestation in adults. The eruptions are commonly bullous, show little predilection for the typical regional pattern of the adults, may be nonpruritic, have a shorter course, and often are resistant to the usual therapy. A review of literature shows that in patients over 50 years of age or older the tendency is to manifecst predominantly, although not as strikingly for children, a vesicobullous type of dermatitis herpetiformis with large bullae, varying 1 to 5cm or more in diameter. The authors present two cases of bullous dermatitis herpetiformis confirmed by histology; the first case is a 5 year-old boy who has tense bullae, pustules, and pigmentations on the inguinal region, lower abdomen, buttocks, face, and extensor surfaces of the extremities of 1 yars duration; the second case is a 44 year old housewife who has eruptions of vesicles, bullae, and erosion on the buttccks, back, abdomen, ext;remities, oral cavity, and vulva of 15 days' duration. The authors reviewed the relevant literature and discussed the clinicaI feat ures of both cases thoroughly.

4.Tuberous Sclerosis Occuring through 3 Generations.

Tuberous sclerosis is a complex neurocutaneous disease inherited as an autosomal dominant pattern, which is characterized by facial angio-fibroma, mental retardation and seizures. Many cases have been reported in the literatures, but familial cases are relatively rare. We present 9 cases of tuberous sclerois occuring througb 3 generations, which are 3 cases of angiofibroma, mental retardation and seizures, 2 cases of facial angiofibroma and seizures, 1 case of mental retardation and seizures and 3 cases who showed facial angiofibroma only. Two brothers in this family reveald icthyosis vulgaris without tuberous sclerosis.

5.Osteosarcoma-Thirteen-Year Experience

Osteosarcoma is the most common primary malignant tumor in the orthopaedic field. Recently, the management of osteosarcoma was evolved in many aspects and the improved results were reported in many articles. The purpose of this study is to evaluate the changes of clinical findings and management modalities in osteosarcoma since 1980. We reviewed 127 osteosarcomas managed from 1980 to 1992. The Enneking surgical stage was as follows; stage I 12 cases, stage II 98 cases and stage III 17 cases. They were subclassified into classic(97 cases), parosteal(12 cases), telangiectatic(5 cases), secondary(4 cases), periosteal(2 cases), gnathic(2 cases), high grade surface(1 cases) and undetermined(4 cases). The disease-free survival rate was evaluated for the patients of stage II classic osteosarcomas treated with amputation(27 cases) or limb-salvage operation(23 cases), and average follow up period of them was 49 months. In 27 cases of amputation adjuvant chemotherapy was administered in 23 cases. And in 23 patients treated with limb-salvage operation, neoadjuvant and adjuvant chemotherapy were done in 19 cases and only adjuvant chemotherapy in 2 cases. The clinical changes since 1980 were as follows: (1) The mean duration from symptom onset to diagnosis was decreased gradually from 6.5 months(1980) to 3.1 months(1992). (2) The proportion of curative surgery was increased from 40%(1980) to 87%(1992) in stage I and II. (3) The proportion of limb-salvage operation was rapidly increased upto 88% since 1985. The 5 year disease-free survival rate of stage II classic osteosarcoma was 36% with amputation and 67% with limb-salvage operation.

6.Two Cases of Acute Encephalopathy with Thalamotegmental Involvement and Remained Motor Dysfunction in Infants.

Acute encephalopathy with thalamotegmental involvement occurs in infants and young children and is thought to be a postviral or postinfectious brain disease. This disease is preceded for several days by fever and symptoms of upper respiratory infection. The symptoms are followed by the rapid evolution of stupor and coma, associated with generalized seizures and decerebrate rigidity. In this disease, the computed tomography (CT) or brain magnetic resonance imaging demonstrate symmetrical focal lesions in the thalami and brainstem tegmentum. We report two cases of acute encephalopathy with thalamotegmental involvement and remaining motor dysfunction in infants.

8.Treatment of Fixed Lumbosacral Kyphosis by Posterior Vertebral Column Resection: A preliminary report.

STUDY DESIGN: This is a retrospective study evaluating the efficacy of new surgical technique for treatment of fixed lumbosacral kyphosis. OBJECTIVES: To report a new method of vertebral column resection and to determine its efficacy in the treatment of fixed lumbosacral kyphosis. SUMMARY OF BACKGROUND DATA: The treatment of fixed severe spinal deformity is very difficult and only a few surgical methods are reported. The vertebral column resection from anterior and posterior is a radical method to treat the severe deformity but it has many problems with anterior and posterior approachs. MATERIALS AND METHODS: A new surgical method of vertebral column resection from posterior approach only(PVCR) was carried out in 5 patients of fixed lumbosacral Tb kyphosis from October 1997 to March 1998. The mean age was 43.4 years(range 35-61 years), and four were female and one male. The average postoperative follow-up period was 5.5 months(ranged from 2 months to 12 months). An average of 2.6 vertebrae was resected. The degree of sagittal curves was measured using the Cobb technique preoperatively and postoperatively. the rejional lumbosacral kyphotic angle and compensatory thoracic curvature from 74 to 712. The sagittal imbalance was measured by distance from C7 plumb line to postero-superior corner of 51 using 14 x 36 inch standing lateral radiograph. RESULTS: The sagittal deformities were corrected from an average of kyphosis 37.4 to lodosis 8.4 at lumbosacral level and from an average of lodosis 31.6 to lodosis 8.2 at thoracic level. The sagittal imbalance was improved from an average of +11.7cm to +2.2cm, for a posterior immigration of 9.5cm. The mean operating time was 334 minutes and with an average blood loss of 4338m1. Complication comprised of motor weakness with deep wound infection in one case and superficial wound infection in another case. The transient hip flexor weakness was seen in all the patients. The hip flexor power recovered in 3-4 weeks. The patient of motor weakness and deep infection showed partial motor improvement and complete wound healing at last 2 month follow-up. CONCLUSION: one-stage posterior vertebral column resection is a promising technique for treatment of the severe fixed lumbosacral kyphosis.

9.Influence of the Posterior Slope of the Tibial Component on the Maximal Flexion after Total Knee Arthroplasty.

To evaluate the effect of tibial cut with posterior slope in total knee arthroplasty(TKA) surgery on the flexion of the knee, 41 knees(32 patients) with varying degree of the posterior slope were retrospectively reviewed at more than 1 year after operation. Does sloping the tibial cut iurface rnore posteriorly promote increasing of maximal flexion of the knee by elimination of excessive tenaion of the posterior cruciate liga ment The degree of the posterior slope was radiographically measured, awi the change of the degree of the maximal flexion between preoperative and postoperative period was clinimlly measured with a goniometer at the latest follow up more than 1 year after operation. Comparison of the results demonstrated significantly larger degree of rqaximal flexion for the knees that had tibial cut with the degree of the posterior slope, 5 degrees or rnore (p<0.05). We concluded that 5 degrees or more posterior slope in proxirnal tibial cut is one of the important fact()r which could achieve increased flexion of the knee after TKA operation.

10.Intramuscular Hemangioma of the Mentalis Muscle: A Case Report.

Intramuscular hemangiomas are uncommon tumors of the head and neck, consistituting of 0.8% of all hemangioma, but often occur in the trunk and extremities. In the head and neck area, the masseter and trapezius muscle occur frequently, in which diagnosis is often difficult even with accurate preoperative imaging. The definitive diagnosis is made by histological study of the surgical biopsy specimen. Treatment of choice is complete surgical excision. This is, to our knowledge, the first case of intramuscular hemangioma of the mentalis muscle to be reported in korea.
