3.The Application of Endoscopic Mucosectomy in Various Mucosal Lesioss of Upper Gastrointestinal Tract.

In case of submucosal tumor, adenoma and atypism, we can't frequently use the snare polypectomy for treatment of them. Adenoma and atypism are premalignant, so other therapeutic modalities are necessary for resection of them. We have perfortned endoscopic mueosectomy such as strip biopsy, and "O"ring ligation on various mucosal lesions of upper GI tract in 23 patients(11 men, l2 women) from June 1993 to December l994. Fourteen patients wbo had adenoma were followed up endoscopically for 6 months to 24 months(mean: 14 months). The results were as follows: 23 patients(27 lesions) were enrolled in this study. Mean age was 50.3(range 32-74 years). The method of mucosectomy was strip biopsy in 19 cases(21 lesions), and "O"ring ligation in 4 cases(6 lesions). The rate of complete resection was 88.9%. If adenomas were resected incompletely, we added a laser therapy. There was no recurrence except a case in which carcinoma was detected on the other site during follow-up. No serious compication occurred such as massive bleeding or perforation. Based on the above results, the endoscopic mucosectomy is considered to be a safe and effective modality in the treatment of the flat polyps of upper GI tract and could be used for prevention of gastric cancer.

4.A Case of Gastric Anisakiasis Causing Severe Gastric Ulcer Bleeding.

Anisakiasis is a parasitic infectious disease caused by eating raw, infected fish. It causes severe abdominal pain-like acute abdomen, but not fatal. Anisakiasis was firstly reported by van Thiel et al. in 1960 in Netherlands and it has subsequently been reported in the Netherlands, Japan, the United states, Korea as well as other nations. Because Koreans like eating raw fish, the anisakiasis of the stomach was occasionally reported, but the case of severe gastric bleeding was very rare. So we report one case of gastric ulcer bleeding due to gastric anisakis larva proven by endoscopy and gastric resection.

6.Diagnostic Significance of Telomerase Activity in Breast Cancer.

PURPOSE: This study was performed to confirm the differences in telomerase activities between breast cancer and benign breast disease and to evaluate the clinical significance of telomerase activity in the diagnosis of breast cancer by its application to cytologic specimens. METHODS: Thirty-six breast-cancer samples and 23 benign-breast-disease samples were analyzed to detect telomerase activity by using a modification of the telomeric repeat amplification protocol (TRAP). In 27 breast-cancer specimens, both the cytologic samples obtained by fine needle aspiration (FNA) and the tissue samples from the primary tumor were analyzed using the same method for comparison. RESULTS: Telomerase activity was detected in 7 of 23 (30.4%) benign breast diseases and 30 of 36 (83.3%) breast cancers and was found not to correlate with the prognostic or pathologic variables. In the diagnostic accuracy of breast cancer, the resultant sensitivity, specificity, and positive and negative predictive values for telomerase activity were 83.3%, 69.6%, 81.1%, and 72.7%, respectively. Telomerase assays of cytologic and tissue samples of 27 breast cancers detected telomerase activity in 20 of 27 (74.1%) cytologic specimens and in 23 of 27 (85.2%) tissue specimens. CONCLUSION: The results of this study suggest that the detection of telo merase activity in tissue or cytologic samples is useful in the diagnosis of breast cancer.

7.In vivo31P MR Spectroscopy of Breast Tumors: Preliminary Results.

PURPOSE: To evaluate the various phosphorus metabolism' of breast tumors with use of in vivo phosphorus-31 (31P) M R spectroscopy (MRS) MATERIALS AND METHODS: Five patients with breast tumor (benign in two, malignant in three) and three normal healthy volunteers participated in this study. All in vivo31P MRS examinations were performed on 1.5 Twhole-body MRI/MRS system by using a Free Induction Decay (FID) pulse sequence. Tl-weighted MR images were used for localization of tumors. Peak areas for each phosphorus metabolite were measured using a Marquart algorithm. RESULTS: Breast carcinoma had a substantially larger phosphomonoester (PME) and a smaller phosphocreatine (PCr) peak intensity than normal breast tissue. This was reflected in the relatively higher PME/PCr ratio of breast carcinomas as well as phosphodiester (PDE)/PCr, inorganic phosphate (Pi)/PCr, and adenosine triphosphate (ATP)/PCr ratios, compared with normal controls. The mean pH value of breast tumor demonstrating the alkaline nature was higher than that of normal controls. Spectral patterns between benign breast disease and normal breast tissue were quite similar, and differentiation was not established. CONCLUSION: Our preliminary study suggests that in vivo 31P MRS is a noninvasive examination which may be useful in the early differentiation of malignant breast tumors from normal and benign conditions. However, normal control and benign conditions could not be characterized on the basis of the phosphorus metabolite ratios.
