1.Granular Cell Myoblastoma of the Cecum: Report of a case.

Granular Cell Myoblastoma is typically encountered in the skin, subcutaneous tissue nr oral cavity, but was rarely been found in the large intestine. In a series of 110, the two were in the cecum(18). Both patients, 36-and 43-year-old women, underwent radial excision of the lesion because of the clinical impresaion of the malignancy. The other Three patients with granular cell myoblastoma of the cecum, all asymptomatic and found incidentally, have been reported. From the available data, one was a man(22) and two were women (26,27). They were 17 (27), 40(26) and 71(22) years of age. One lesion, in the cecum, was excised at laparotomy that was performed under the irnpression of acute appendicitis, and disclosed a small polypoid submucosal nodule in the cecum incidentally(27). Another lesion, in the cecum, was excised at laparotomy following the discovery of a small cecal defect on barium enema examination 9260. The other lesion, in the cecum, was found at autopsy(22), This paper reports a case of granular cell myoblastoma of the cecum, including its colonoscopic finding.

2.Clinical experiences in Thumb reconstruction

The reconstruction of a mutilated thumb is one of the most fascinating aspect of hand surgery. The hand deprived of the thumb loses approximately 40 per cent of its function. it corresponds to a total body loss of 22 per cent, which is compatible with the loss of an eye. Recently reconstructive operations were performed for the restoration of the mobility, sensibility, and length of thumb. 4-flap Z-plasty were performed for the skin contracture in the first web space. For the sensibility, we transferred the neurovascular island flap from the middle or ring fingers. And we reconstructed the loss of thumb by pollicization, microsurgical reattachment, and toe-to-thumb transfer operation. We selected 11 cases who were followed over one year after operation.

4.Artificial Corneal Endothelial Dystrophy due to Corrective Surgery for Myopia.

A 46 years old male patient had visited to our Ophthalmology department due to abrupt onset of decreased visual acuity in his left eye. He had taken an operation for correction of high myopic astigmatism on both eyes 23 years ago in Japan by Dr. Sato. His visual acuity was: (R)0.3(0.7 x Sph. + 0.75D. = Cyl. - 5.50D.A10 degrees) (L)0.15(0.2 x Sph. - 2.50D. = Cyl. - 2.25D. A180 degrees). On slit lamp examination. hoth cornea had 3mm. (length) radiated linear scars in a number about 50 resulted from myopic operation. In his left cornea, several small vesicles and mild corneal central opacity were observed. We have diagnosed this symptom as artificial corneal endothelial Dystrophy.

5.Capillary Plexus and Vasa of the Rat Lung as Revealed by Scanning Electron Microscope of Corrosion Casts.

The pulmonary microvasculatures of rats were studied by injection replicas prepared from Mercox. This medium enabled us to easily obtain consistent, stable, and complete injected replicas of the pulmonary vasculature. In order to investigate the three dimensional structures of the tributaries of the bronchial artery, such as the capillary plexus and vasa vasorum, we performed a scanning electron microscopic(SEM) study of the vascular casts, using Mercox CL-2B as a media. The alveolar capillaries revealed hexagonal or pentagonal rings of vascular networks. In some areas, the vascular rings composed a square network, The bronchial tree was supplied by the bronchial arteries which form a coarse capillary extending as far as the terminal bronchioles. Occasionally the capillary plexus was connected with adjacent capillary networks in and around the alveolar walls. The walls of the pulmonary artery revealed only a single layer of the vasa vasorum, but those of the pulmonary vein were surrounded by more complicated and well developed vasa vasorum than the pulmonary arterial side. The mean diameters of the venous vasa vasorum are greater than the arterial vasa vasorum.

6.Treatment of the Hemospermia.

To evaluate the etiology and treatment of options in patients with hemospermia, we retrospectively reviewed 21 patients who had both transrectal ultrasound and MRI and have been followed for more than 6 months after initiation of treatment. Mean age was 40 years (range 20 - 50 years) with duration of infliction being 24 months (range 1 week to 16 years). Among the 14 patients with prostatic cyst, 9 patients underwent transurethral unroofing (TUUR) of cyst. All 9 patients were completely cured, but one patient developed postoperative epididymitis. Among the 6 patients with prostatitis, hemospermia was resolved in 4 patients after mean treatment of 12 weeks with antibiotics. In DES (Diethylstilbestrol) group, 50 % showed resolution of symptoms among 6 patients after 4 weeks of treatment, and two patients complained breast discomfort which were developed after medication. In conclusion, the antibiotic treatment is advisable in hemospermia patients with prostatitis. Prostatic cystic lesion is believed to be a factor in hemospermia, thus TUUR of cyst wall and removal of stone and hematoma in cyst can be a viable option.

7.Surgical Correction of Congenital Epiblepharon: Lower Eyelid Crease Reforming Technique.

Epiblepharon is a commonly encountered congenital anomaly in Asian infants and children. It causes symptoms of ocular irritation and inferior punctate corneal epithelial erosion. Surgical correction may be needed if ocular irritation symptom and corneal pathology persist with age. A series of 185 lower eyelid epiblepharon in 98 Korean children underwent lower eyelid crease reforming technique. Surgical treatment included excision of redundant skin and pretarsal orbicularis muscle with lid everting suture by anastomosis of the subcutaneous tissue and inferior tarsal border including capsulopalpebral fascia. With a minimum follow-up of 12 months, a total of 37 [20%]eyelids developed recurrence of cilia touch, and then 9 [4.8%]eye-lids needed further surgery.

10.The Relationship between Psychological Characteristics and Academic Achievement in Medical Students.

OBJECTIVE: Several psychological characteristics of medical students were investigated, the relationhip between these characteristics and academic achievement was identified, and futhermore some significant predictors of their academic achievement were explored. METHOD: Subjects were a total of 489 medical students, 1st, 2nd, 3rd year in Chosun university as of in 1998. We administered self-report questionnaires including age, sex, grade failure, and transfer experience. And Beck Depression Inventory, Achievement Self-Discrepancy Scale, Self-Efficacy Scale, State-Trait Anxiety Inventory(state) Test Anxiety Scale, and the Inventory of Performance Anxiety were handed out to them on April 1998. After the semester, we examined the academic achievement according to grade point average. Pearson's correlations were calculated to explore the relationships between grade failure and other psychological variables, as well as the relationships between academic achievement and other psychological variables. The amount that academic achievement was explained by the psychological variables was obtained using multiple regression analysis. RESULT: 1)In 489 students, grade failure was 168(34.3%) non-failure was 321(65.7%) and transfer was 33(6.7%) 2)While G.P.A., performance anxiety, and self-efficacy, respectively, were negatively correlated with the failure, test anxiety was positively correlated with the grade failure. There were no statistically significant correlations between grade failure and depression, self-discrepancy, or state anxiety. 3)Academic achievement was positively correlated with both gender and self-efficacy, whereas it was negatively correlated with depression, self-discrepancy, test anxiety, and state anxiety, respectively. Female students were better than males in academic achievement. 4)The amount that age, gender, grade failure, transfer, depression, self-discrepancy, test anxiety, state anxiety, performance anxiety, and self-efficacy accounted for academic achievement was 10.86% totally. CONCLUSION: Gender, grade failure, and depression in medical students affected their academic achievement significantly.
