We reviewed clinically the 56 patients, who had anterior interbody fusion of the cervical spine due to various causes, from 1975 to 1984. The result of the studies are as follows; 1. Inawaisting sign of the grafted level is the one of the most important prognostic factors which indicates the favorable fusion. 2. The mechanism of the inwaisting sign is not confirmed, however plausible explanation is the flexure drift law, which cause the inwaisting phenomena. 3. The time of clinical, radiological union and the appearance of the inwaisting sign of the grafted bone in the fusion of the lower cervical spine are displayed earlier than in the upper cervical spine. 4. Fusion at the one level is obtained one or two weeks earlier than the two level fusion. At the two level fusion, one large bone block operation achieved the earlier bony union than the two separate bone blocks fusion; it could shorten the fusion time by 4 weeks radiologically, although there were no significant difference in clinical aspect and inwaisting sign. 5. In the anterior interbody fusion of the cervical spine, clinical fusion took place in 6–7 weeks, and radiological one in 10-14 weeks after the fusion operation. And it was not influenced by the type of injury or neurological complications. The inwaisting sign appeared from the 4th to 7th week postoperatively. 6. There are no problems in the case of posterior instability initially. The reason is that we have performed the cervical traction and absolute bed rest for 4 to 6 weeks before the anterior interbody fusion in order to take the healing of the poterior ligament complex.

BACKGROUND: This study was undertaken to compare nosocomial infection rates between intensive care units of military and civilian hospitals. METHODS: From July to December 2000, we surveyed the intensive care unit of Armed Forces Capital Hospital (AFCH). We compared device use ratios and device-day infection rates with those of Korean Society for Nosocomial Infection Control (KOSNIC) and National Nosocomial Infections Surveillance (NNIS) system. RESULTS: During the period of study, 185 cases were admitted and 24 nosocomial infections were detected: 7 cases of pneumonia, 6 urinary tract, 3 blood stream, 3 cardiovascular system, 3 surgical site infections, 1 skin and soft tissue, and 1 central nervous system infection. Ventilator, urinary catheter and central venous catheter use ratios were 0.14 (95% confidence interval, 0.12-0.16), 0.58 (0.56-0.60) and 0.33 (0.31-0.35). The ratios of NNIS were 0.41, 0.67 and 0.50. Ventilator-, urinary catheter- and central venous catheter-day infection rates were 18.69(11.36-53.32), 6.65 (3.36-14.20) and 1.95 (1.44-9.92). However, the rates of KOSNIC were 9.93, 5.29 and 3.62. The rates of NNIS were 11.24, 6.14 and 5.55. CONCLUSIONS: In AFCH ventilators were used less frequently than NNIS, but more ventilator-associated pneumonia were developed than KOSNIC and NNIS.

OBJECTIVES: Fatigue is a primary human factor for decreased job performance in the workplace. It is well documented that drowsiness is a typical symptom of fatigue and is closely associated with commercial drivers'safety and well-being. However, few studies have been conducted to develop or validate fatigue symptom instruments for a working population. The main purpose of this study was to develop a general fatigue checklist and a driving fatigue checklist for Korean commercial drivers. METHODS: A total of 287 bus drivers in a commercial transportation company participated in a self-administered survey that was designated Study 1. Based on the statistical results of Study 1, a focus meeting with 16 professional consultants was conducted to revise the fatigue symptom instrument for Study 2. In Study 2, 288 commercial drivers (156 bus drivers and 132 truck drivers) participated in the revised questionnaire survey. All collected responses were entered into a SPSS worksheet and the data analysis was conducted using SPSS software 11.1. The exploratory factor analysis used in this study followed the principle component factoring rule and the varimax rotation method for factor extraction. The criteria for item selection were an Eigen value of 1.0 or greater, a communality score of .50 or greater, and no 'fence rider'property over the extracted factors. RESULTS: Through Study 1, the consultant meeting, and Study 2, a general fatigue checklist was developed with a total of 3 factors and 11 items, and a driving fatigue checklist was developed with 2 factors and 10 items. The 3 factors of the general fatigue checklist were physical fatigue, psychological fatigue, and chronic tiredness. The two factors of the driving fatigue checklist were physical fatigue and perceptive and functional fatigue. CONCLUSIONS: The primary contents of general fatigue were different from those of driving fatigue according to the two fatigue instrument factors developed in this study. The primary fatigue symptoms of the commercial driving population were identified as physical fatigue and perceptive and functional fatigue.

OBJECTIVES AND METHODS: The presence of aneuploidy or high proliferative activity in cytologic specimens is considered as complementary for the diagnosis of malignancy. To evaluate the diagnostic usefulness of DNA ploidy and cell cycle analysis in lung cancer, we compared the diagnostic yielding rates of DNA ploidy test by brushing specimens using flow cytometry with bronchoscopic forceps biopsy and brushing cytology. RESULTS: Of the seventy-six cases, 55 cases proved to have malignant diseases(squamous cell cancer: 27, adenocarcinorna: 7, large cell cancer: 1, undifferentiated: 4 and small cell cancer: 16). The incidence of aneuploidy in lung cancer..patients was 32.y %(18/55), as opposed to no cases in benign disease. And the proportion of high proliferative activity(S+GEM>22%) in lung cancer patients was 42.9% (15/35), but none in benign diseases. In (iffy-six of 75 cases(74.7%), cytology of brushing specimens and DNA analysis(either aneuploidy or high proliferative activity vs. diploidy and low proliferative activity) were in concordance. The sensitivity with only brushing cytology was 41.8%(23/55), but with the addition of DNA analysis, it was increased to 56.4%(31/55), without decreasing the specificity(100%). And there was a case whose clue for malignancy was absent except aneuploidy, and he was confirmed to have squamous cell cancer following open thoracotomy There were no differences in the frequency of aneuploidy or high proliferative activity between histologic subtypes of bronchogenic malignancy. CONCLUSIONS: The diagnostic detection rate of lung cancer was improved with the addition of DNA ploidy and cell cycle analysis, and the presence of aneuploidy or high proliferative activity was a relatively specific indicator of malignant disease. It would be useful to test DNA ploidy and cell cycle analysis with brushing specimen for the diagnosis of bronchogenic malignancy particularly in patients whose biopsy specimen could not be obtainable.

The laryngeal mask as an new form of airway has recently been described. This mask is introduced blindly into the hypopharynx to form a seal around the larynx, so permitting spontaneous or positive pressure ventilation without passing the larynx, therefore associated with much reduced incidence of laryngeal damage. But endotracheal intubation is associated with a significant rate of layngeal damage and subsequent vocal cord pathology. This study was designed to investigate the effects of postoperative sore throat related to insertion of laryngeal mask airway and to compare with these effects after endotracheal intubation. One hundred randomly selected female patients who had gynecological procedures under general anesthesia were surveyed to ascertain the incidence of sore throats. The results were as follows; l) The incidence of sore throat after endotracheal intubation with the Portex tube was 48% but 12% after insertion of laryngeal mask. 2) The average volume of air required to inflate the cuff of laryngeal mask sufficiently to form air-tight seal around larynx was 11.4+/-2.8ml and cuff pressure was 25.6+/-13.2cmH2O when the laryngeal mask 3 was used for Korean adult female.

Ochrobactrum anthropi, a rarely encountered organism in humans, is usually associated with infections in immunocompromised hosts and patients with indwelling catheters. We report a case of bacteremia associated with O. anthropi in a 80-year-old male with intrahepatic duct carcinoma. After insertion of biliary stent with endoscopic retrograde cholangiopancreatography, fever and neutrophilic leukocytosis developed in the patient. Blood cultures were performed three times, and O. anthropi was isolated from an aerobic bottle. Although the patient did not receive effective antibiotic treatment, fever and neutrophilic leukocytosis were improved.

Brunners gland adenoma is a benign epithelial tumor of the duodenum originating from submucosal Brunners gland. This is an extremely rare entity that account for only 10.6% of benign duodenal tumor, which are themselves relatively rare, representing 0.008% of all surgical and autopsy specimens. The clinical manifestation are nonspecific gastrointestinal complaints, such as bloating or epigastric pain, and the tumor gives rise to melena or anemia, due to the ulceration or erosion of the tumor. The diagnosis is usually made by radiologic studies and gastroduodenal endoscopy which can also provide definitive treatment. The aim of treatment is complete removal of the lesion and exclude malignancy. We report on 4 cases of Brunners gland adenoma which was confirmed by operation or endoscopic polypectomy.
