Flavimonas oryzihabitans is a gram-negative, glucose non-fermentative bacillus, and is rarely reported as a pathogen in human. In recent years, reports of infection due to F. oryzihabitans, especially in immunocompromised patients with indwelling catheter or peritoneal Tenckhoff catheter have increased. We report two cases of F. oryzihabitans peritonitis in patients undergoing continuous ambulatory peritoneal dialysis (CAPD).

3.Metallic Failure in Treatment of Comminuted Fracture of Femur: Clinical Analysis

The authors clinically analysed the causes of metallic failure in 21 cases after treatment of comminuted fracture of femur at Department of Orthopaedic Surgery, Kang-Nam St. Marys Hospital during 5 year period from May 1981 to December 1986. In this series, the nature of metallic property was not studied which may influence the result of failure. 1. Metallic failure occurred in seventeen self-compression plates, one Kuntscher nail, one I-beam nail, one Ender nail, and one Angle blade-plate. 2. Seventeen patients had fractures in the middle or distal one third of femur. 3. Metallic failure occurred from 3 to 19 months following open reduction and internal fixation. The time interval between internal fixation and failure was 9.6 months on an average. 4. Metallic failure of the self-compression plate was observe in the empty hole over the fracture site in 14 out of 17 patients. In the Kiintscher, failure developed at the interlocking hole corresponding with the fracture level. In the Ender nail and Angle blade-plate, failure developed at the fracture site, and in the I-beam nail plating, failure developed at the neck of the fixation screw, and not at the plate. 5. The cause of metallic failure in 17 self-compression plate-treated cases was improper plating such as varus plating in 4, absence of medial buttress due to cortical comminution in 10. Above mentioned two conditions were combined in 4 cases, in which one case of Angle blade-plate was included. Incorrect position of I-beam nail, inadequate postoperative management following Ender nailing and incorrect selection of implant in one case of Kiintscher nailing were the other possible causes of metallic failure.

4.A Case of Renal Vein Thrombosis in a Patient with Lung and Uterine Cervical Cancer.

Thrombosis of one or both renal veins occurs in a variety of settings such as trauma, dehydration, extrinsic compression, nephrotic syndrome, pregnancy, oral contraceptives or invasion by tumor. Nephrotic syndrome and certain carcinomas seem to predispose to the development of renal vein thrombosis. The most common carcinoma which is associated with renal vein thrombosis is the renal cell carcinoma. But, renal vein thrombosis associated with tumor other than renal cell carcinoma is extremely rare. Our patient was diagnosed as lung and uterine cervical cancer by histologic examination. The patient complained of left flank pain. Abdominal CT scan shows the dilated left renal vein filled with hypodense material and parenchymal infarction on the left kidney. Renal arteriography shows cut-off sign on a branch of the left renal artery. Inferior venacavography revealed contrast-filling defect in the left renal vein. To our knowledge, renal vein thrombosis with lung and uterine cervical carcinoma is the first report in English literature. So, we have presented a rare case of a renal vein thrombosis in a patient with lung and uterine cervical cancer.

8.Stress analysis of supporting tissues and implants according to implant fixture shapes and implant-abutment connections.

PURPOSE: Four finite element models were constructed in the mandible having a single implant fixture connected to the first premolar-shaped superstructure, in order to evaluate how the shape of the fixture and the implant-abutment connection would influence the stress level of the supporting tissues, fixtures, and prosthethic components. MATERIAL AND METHODS: The superstructures were constructed using UCLA type abutment, ADA type III gold alloy was used to fabricate a crown and then connected to the fixture with an abutment screw. The models BRA, END, FRI, ITI were constructed from the mandible implanted with Branemark, Endopore, Frialit-2, I.T.I systems respectively. In each model, 150 N of vertical load was placed on the central pit of an occlusal plane and 150 N of 40.oblique load was placed on the buccal cusp. The displacement and stress distribution in the supporting tissues and the other components were analysed using a 2-dimensional finite element analysis. The maximum stress in each reference area was compared. RESULTS: 1. Under 40.oblique loading, the maximum stress was larger in the implant, superstructure and supporting tissue, compared to the stress pattern under vertical loading. 2. In the implant, prosthesis and supporting tissue, the maximum stress was smaller with the internal connection type (FRI) and the morse taper type (ITI) when compared to that of the external connection type (BRA and END). 3. In the superstructure and implant/abutment interface, the maximum stress was smaller with the internal connection type (FRI) and the morse taper type (ITI) when compared to that of the external connection type (BRA and END). 4. In the implant fixture, the maximum stress was smaller with the internal connection type (FRI) and the morse taper type (ITI) when compared to that of the external connection type (BRA and END). 5. The stress was more evenly distributed in the bone/implant interface through the FRI of trapezoidal step design. Especially Under 40.oblique loading, The maximum stress was smallest in the bone/implant interface. 6. In the implant and superstructure and supporting tissue, the maximum stress occured at the crown loading point through the ITI. CONCLUSION: The stress distribution of the supporting tissue was affected by shape of a fixture and implant-abutment connection. The magnitude of maximum stress was reduced with the internal connection type (FRI) and the morse taper type (ITI) in the implant, prosthesis and supporting tissue. Trapezoidal step design of FRI showed evenly distributed the stress at the bone/implant interface.

10.Postoperative radiotherapy for endometrial cancer.

PURPOSE: To investigate the prognostic factors and effectiveness of postoperative radiotherapy alone for endometrial carcinoma. MATERIALS AND METHODS: Sixty four patients with stage I-III endometrial cancer (EC) treated with postoperative radiotherapy alone between January 1989 and December 2008 at the Keimyung University Dongsan Medical Center were chosen for the present study. Typically, total hysterectomy, salpingo-oophorectomy and lymphadenectomy were performed on the patient's pelvis. Total dose from 50.4 Gy to 63 Gy was irradiated at pelvis or extended field. Thirteen patients were treated with Co-60 or Ir-192 intracavitary radiotherapy. Follow-up periods were from 7 to 270 months, with a median of 56 months. RESULTS: Five year overall survival (OS) rate was 58.7%, respectively. Five year disease-free survival (DFS) rate was 59.2%, respectively. In univariate analysis for OS and DFS, stage, menopausal age, type of operation, serosal invasion, and lymph node involvement were found to be statistically significant. Histologic type was marginally significant. In multivariate analysis for OS and DFS, stage, types of operation, histologic type were also found to be statistically significant. Treatment failure occurred in 14 patients. The main pattern of failure was found to be distant metastasis. Time to distant metastasis was from 3 to 86 months (median, 12 months). There were no grade 3 or 4 complications. CONCLUSION: Stage, types of operation, and histologic type could be the predictive prognostic factors in patients. We contemplated postoperative radiation as effective and safe treatment method for EC. Additional treatment would be needed to reduce distant metastasis.
