1.The Study of Cell Killing Mechanism by Membrane Attack Complexes of Complement in the Nucleated Cells.

The mechanism of cytolysis by complement attack of nucleated cells(NC) is of special interest in comparison to that of red blood cells. It is known that NC death by membrane attack comples, C5b-9, is caused by many factors, i.e., efficiency of complex assembly, activation of intrinsic metabolic pathway by signal transduction, cytotoxic effect of the channel itself and natural repair ability. These factors suggest that colloid osmotic lysis, known in red blood cells, does not fully explain the complement-mediated cell death of NC. In this study, the authors investigated correlation between biochemical and morphological changes to prove "Ca2+-mediated metabolic death"8~13) representing a mechanism of NC death caused by C5b-9 attack. The L1210 cells, mouse leukemic cell line carrying small complement channel(TAC5b-91) were used in the experiments. The amounts of intracellular adenine nucleotides to extracellular Ca2+, ouabain, KC1 and dextran were analyzed by bioluminescence method using luminometer. Cell viability was checked by 0.4% trypan blue dye and LDH release. Morphological observation of TAC5b-91 was done by immunocytochemical staining and electron microscope. The results were as follows: 1) The release of ATP, ADP and AMP followed by cell death was rapid and progressive along the incubation time at 37 degrees C and it was accelerated in 1.5 mM of [Ca2+]0. 2) There was no evidence of ATP repairment in the TAC5b-91. 3) Extracellular KC1(150 mM), dextran(0.66 mM) and ATP supplement(0.2 microM) could not effectively inhibit ATP depletion and cell death. Ouabain(27 and 100 microM) enhanced cell death and could not completely prevent ATP loss. 4) Most of the mitochondria showed swelling, loss of cristae and Ca2+ deposit in matrix in the electron microscopic observation. Rapid, sustained and irreversible depletion of adenine nucleotides was due to Ca2+ deposit with destruction of mitochondria and also the leakage through transmembrane channels. Moreover this energy depletion was accelerated by high extracellular Ca2+ concentration. These results indicate that Ca2+-mediated, energy exhaustion is one of the mechanisms of the metabolic cell death by C5b-9 attack of NC.

3.Dysplasia Epiphysealis Hemimelica: A Case Report

A case of dysplasia eplphyseaalis hemlmelica (Fairbank, 1956) in a 8 years old boy, showlng typical clinical and roentgenographical appearance, is reported together wlth a brief review of literature in this paper. This condition is a rare developmental disorder of childhood in which there is a asymmetrical cartilaginous overgrowth of one or more than one epiphysis, or of a tarsal or carpal bone. Thls disorder is usually Iimited to elther the medial or lateral half of a single extremity. The disease is characterised radiographically by delay in the appearance of the epiphyseal nucleus, which is mottied and of irregular density. The most common deformities were genu valgum or varum and valgus or equinus deformity of the ankle. These deformities tended to increased wlth growth but the size of the lesion increased in proportion to the growth of the normal bone. Concomitant involvement of the knee and ankle is the rule, and in those cases the affected limb Is usually longer than Its mate. The lesion Is often microscopically indistingulshable from an osteochondroma.

4.Acute Pyogenic Osteomyelitis in Innominate Bones Simulating Septic Hip in Children: A Report of Seven Cases

Acute hematogenous osteomyelitis adjacent to hip, though uncommon, should be included in the differential diagnosis of the patients presenting symptoms suggestive of septic arthritis of hip. When it occurs in children, it is particularly serious, because the diagnosis at first may be very difficult to be made. Our observations on seven cases reported here suggest the following findings to be helpful for early differential diagnosis of osteomyelitis of innominate bones from septic hip. 1. Among 7 cases, 4 had lesions in ischium, and 3 in os ilii. 2. The initial diagnosis on admission in 3 cases out of 7 was wrongly made to be septic arthritis of hip. 3. Early accurate diagnosis is essential to initiate early treatment, and there by shortens the convalescent time and minimize sequelae. 4. The diagnosis could be made in the cases by clinical and roentgenographic findings, such as fever, pain nature, antalgic posture of hip together with range of hip motion, tender point around hip and positive soft tissue and bony X-ray findings, but K-ray taken on early stage of infection could not provide any clue until bone lesion appears. 5. Gentle passive motion of hip is more easily permitted without eliciting severe pain and restriction of motion in the infection of innominate bone than in the septic hip. This also can be another clue to differentiate the acute osteomyelitis of innominate bone from septic arthritis of the hip.

5.Fracture of the Femoral Shaft and an Associated Ipsilateral Fracture of the Femoral Neck

The femoral neck fracture having concomitant fracture of the femoral shaft occurs predaminantly in young man and have known to associate the serious complications. One of such factures is sometimes overlooked and is possibly due to a more obvious nature of the shaft fracture than that of the ipsilateral neck. Six cases of this type of combined fractures in ipsilateral femur are reported with the review of 1 iteratures. In six cases of the femoral neck fractures, three which were unrecognized at initial examiaation in early phase, resulting in serious residue as much as deformity and avascular necrosis of the femoral head, and other three were recognized in early at initial examination healed in good anatomic position and in due time. Through this observation it is stressed that early discovery and adaquate treatment of the fracture is the only solution to minimize the sequelae.

6.Assessment of Left Ventricular Diastolic Function by Doppler Echocardiogram in Hypertensives with Disproportionate Septal Thickening.

Hypertensive left ventricular hypertrophy(LVH) was considered to be a physiologic adaptation to the increased afterload of left ventricle, but recent studies revealed that LVH was one of the most important target organ damage in essential hypertensive patients & cardiovascular morbidity was increased in patients with hypertensive LVH. Hypertensive LVH could be classified into three types : concentric LVH, disproportionate septal thickening(DST), and left ventricular dilatation. Relatively high incidence of DST in hypertension has bee reported after clinical introduction of echocardiogram. But, the mechanisms for the development of DST and its clinical significance have not been elucidated exactly. In order to assess left ventricular diastolic function in hypertensives with DST, the authors performed phonocardiogram, M-mode, and pulsed Doppler echocardiogram in 15 normotensive control(group A : 5 male, 10 female, 44.4+/-7.7 years), 15 hypertensives without LVH(group B : 5 male, 10 female, 45.5+/-8.6 years), 85 hypertensives with DST(group C : 9 male, 16 female, 47.5+/-8.6 years) and 15 hypertensives with concentric LVH(group D : 8 male, 7 female, 47.7+/-6.1 years). The obtained results were as follows : 1) Left ventricular ejection fraction was 71.6+/-6.3% in group A, 71.9+/-7.5% in group B, 731+/-7.0% in group C, and 70.3+/-10.3% in group D. PURPOSE: We evaluated both the patients and the normal volunteers to determine the diagnostic criteria of subacromial impingement syndrome. MATERIALS AND METHODS: We analyzed the radiologic finding of Thirty degree of caudal tilt view (TCTV) and Supraspinatus outlet view (SOV) of 100 shoulders from 85 patients with clinically proved subacromial impingement syndrome and normal 100 shoulders from 60 volunteers. RESULT: In TCTV, the protrusion of acromion below the line of extension from inferior surface of clavicle was shown in 94% of the patient group and 48% in normal group. Sharp tip of acromial protrusion was detectable in 55.3% of the patient group and 10.4% in normal group. In SOV, curved type of acromion was seen in 53% of the normal and 50% in patient group. Hooked type of acromion was detected in 3% and 31% of the normal and patient group, respectively. CONCLUSION: Protrusion of acromion at TCTV itself was not a criteria of subacromial impingement syndrome, but more than 7 mm below the line of extension from inferior surface of clavicle was meanigful. In SOV, hooked type of acromion was a criteria of subacromial impingement syndrome but curved type is was not a finding of diagnostic significence. Acromial spur formation on TCTV and SOV was important criteria of subacromial impingement syndrome.
