2.Two Cases of Annular Skin Defects Repaired with Quadruple Fan Flaps (O-X Flap).

Nowadays, patients have high expectations when it comes to minimization of postoperative scarring after dermatologic surgical procedures. When an annular lesion is being excised, normal skin should be excised, as well, to prevent a dog ears resulting in a long scar. We introduce a new flap reducing the scar length in annular defects. In order to avoid a long scar, we designed a quadruple fan flap (O-X flap) that is a variation of the rotation flap. It consists of four rotation flaps arranged like fans, with open and closed configuration. We suggest that the quadruple fan flap (O-X flap) is a viable option for treating annular skin defects, because it shortens the scar line, preserves normal tissue, and provides a cosmetically favorable outcome.

3.Two Cases of Squamous Cell Carcinoma of the Bulbar Conjunctiva.

Squamous cell carcinoma of the conjunctiva is a rare disease and usually arises at the limbus and spreads to the cornea, adjacent bulbar conjunctiva and sclera. We experienced two cases of squamous cell carcinoma of the cornea and bulbar conjunctiva. One patient was a 36-year old male with progressive loss of visual acuity and discomfort in the nasal side of the left eyeball for about 7 months. The other patient was a 67-year old male with excessive tearing, recurrent conjunctival injection and elevated mass on inferonasal side of the limbus of the left eye. Excisional biopsy was carried out and histopathologic examination revealed bulbar squamous cell carcinoma in both patients.

4.Three cases of Cornelia de Lange syndrome.

We have experienced 3 cases of Cornelia de Lange syndrome in Korean female infants. They showed typical clinical features of a microbrachycephaly, hirsutism and characteristic face with low forehead bushy eye brows, antimongolian slant, large and depressed briedge of nose, thin protruding lips with down turning of the corners and micrognathia. They also had skeletal abnormalities and chromosomal abnormalities. The diagnosis was established by clinical, laboratory and X-ray findings. The brief review of literature was made.

5.A Case of Hepatocellular Carcinoma with Visceral Peritioneal Metastasis.

Although extrahepatic metastasis of hepatocellular carcinoma are frequent as about 64%, implants involving the peritoneum are rare. We presented a cases, whieh is hepatocellular carcinoma with metastasis to visceral peritoneum revealing multiple, hard, variable-sized and nodular mass including large mass over 10 cm in size. The presence of tumor was diagnosed by serum alpha fetoprotein, abdaminal CT, celiac and SMA angiography and biopsy with laparoscopy.

Alopecia areata is infrequently associated with other diseases, such as, atopic dermatitis, Down syndrome and various autoimmune diseases including thyroiditis, vitiligo, connective tissue diseases, Addison's disease and pernicious anemia. However the association of alopecia areata with myasthenia gravis, a known autoimmune disease, accompanied by thymoma has been rarely reported. We describe herein such a case.

7.Clinical Results of Selective Thoracic Fusion by Segmental Pedicle Screws in King type II Adolescent Idiopathic Scoliosis ( AIS ).

STUDY DESIGN: This study analyzes the results of selective thoracic fusion by segmental pedicle screw fixation in King type II adolescent idiopathic scoliosis(AIS). OBJECTIVES: To verify the efficacy of selective thoracic fusion by segmental pedicle screw fixation in King type II adolescent idiopathic scoliosis. SUMMARY OF BACKGROUND DATA: Decompensations following selective thoracic fusion in King type II adolescent idipathic scoliosis have been attributed to relative inability of the lumbar curves to accommodate correction of the thoracic curves. However, with segmental pedicle screw fixation, selective thoracic fusions yielded satisfactory results in most of the patients regardless of the lumbar curve characteristics. MATERIALS AND METHODS: Eighty patients subjected to selective thoracic fusion by segmental pedicle screw fixation were analyzed after a minimum follow-up of 2 years for deformity correction and spinal balance. Deformity correction was performed by derotation of rod contoured to the normal sagittal curve without additional compression or distraction. RESULTS: Mean preoperative thoracic curve of 53.3 was corrected to 20.3 showing a correction of 63.2%. The mean preoperative lumbar curve was 34.4 with a flexibility of 97.2% and mean lumboapical distance(LAD)of 18.0mm, L4 tilt of 8.0, and lumbar apical rotation (LAR) of 10.9. It was corrected to 16.5 showing a correction of 55.5%. All the patients were balanced clinically and radiologically. CONCLUSIONS: Selective thoracic fusion with segmental pedicle screw fixation and rod rotation offers a satisfactory correction of the deformity and maintenance of spinal balance in King type g AIS regardless of lumbar curve magnitude, LAD, L4 tilt or LAR.

8.Changes in Vertebral Rotation Following Segmental Pedicle Screw Instrumentation and Rod Derotation in Idiopathic Thoracic Scoliosis : Part I - CT Evaluation.

The employment of segmental pedicle screw instrumentation has greatly enhanced the correction of scoliotic deformity. However there is still much controversy on the ability of the system to correct the rotational deformity. This prospective study with 12 idiopathic throacic scoliosis patients subjected to segmental pedicle screw instrumentation was performed to determine the effect of the system on the rotational deformity of idiopathic throacic scoliosis. In addition to the standing films to measure the magnitude of scoliotic curvatures, preand postoperative CT were utilized for the measurement and comparison of angle of rotation relative to the sagittal plane(RAsag.), relative rotation with reference to the upper and lower end vertebrae(RAend), relative rotation with reference to the sacrum(RAsac.), frontal translation(TF) and sagittal translation(TS) for the apical vertebrae. Despite the high correction rate obtained in the frontal plane(average 71.1%) and normalization of sagittal contour, the rotational corrections were statistically insignificant in all parameters and showed no significant correlation to the correction of the curvatures nor horizontal plane displacement as represented by the frontal and sagittal displacement. In conclusions, the segmental pedicle screw instrumentation with rod derotation does not produce significant derotation by itself.

9.A Case of Nail Changes Secondary to Docetaxel.

Docetaxel is a new taxoid antineoplastic drug widely used for advanced breast cancer , ovarian cancer . Skin and nail toxicity are one of the more frequent nonhematologic adverse reactions. Especially, nail changes include dark pigmentation, subungual hemorrahage, acute paronychia, onycholysis, Beau s line etc. We have observed a striking example of multiple Beau s lines from all fingernails and toenails in a man receiving a docetaxel infusion for treatment of metastatic cancer with unknown-origin.

10.A Case of Cavernous Hemangioma of Stomach.

A 44-year-old woman who comlpained of dizziness and generalized weakness was admitted. The hemoglobin was 6.6g/dL, hematocrit 25.5%, and serum ferritin 2.14 ng/mL Stool occult blood was positive and microcytic hypochromic anemia was found on periyheral blood smear. Gastroscopic examination showed about 2 x 1 cm sized hemispherical sebmucosal tumor on antrum. The patient underwent operatioh for confirmatory diagnosis and treatment. The final pathologic diagnosis of the resected lesion was hemangioma of stomach. Cavernous hemangioma of stomach is a rare disease.Mostly, it has a benign course clinically, but early diagnosis is important because massive hemorrhage and anemia by chronic blood loss can occur. We report a case of hemangioma of stomach with review of literature.
