2.Chiari Osteotomy in Old Congenital Dislocation of the Hip

Twelve neglected congenital dislocations of the hip in ten patients over the age of eight years were treated by open reduction and chiari osteotomy. Reduction was aided by preoperative traction only in two hips, by femoral shortening with preoperative traction in three and by femoral shortening only in seven. The average follow up period in our series is 3 years, ranging from 1 year and 2 moths to 4 years and 5 months. The overall results were good but one case of avascular necrcsis and one case of redislocation were seen. By our experience it is thought that all old and congenitally dislocated hip in the growing age can be reduced by femoral shortening with soft tissue release, and preoperative traction for its reduction is not undispensable. The most problem in the treatment of the old congenital dislocation of the hip is the postoperative partial ankylosis in the affected hip and this problem would be overcome by delicate operative technique and meticulous hemostasis.

7.Results of Treatment in Childhood with High Hyperopic or Astigmatic Amblyopia.

We analized the results of treatment in 28 children with high hyperopic amblyopia or astigmatic amblyopia among 65 amblyopic children ranging from 3 to 10 years of age from Jan. 1985 to Dec. 1989. The results are as follows: 1. The final corrected visual acuities of both eyes above 0.8, according to Han's visual acuity chart, were attained in 7 cases (50.0%) among children with high hyperopic amblyopia and 4 cases (28.6%) among children with astigmatic amblyopia. 2. As the degree of hyperopic errors increased, initial corrected visual acuity decreased (p<0.05). 3. The initial corrected visual acuity was not associated with the degree of hyperopic errors, but could be correlated with the initial visual acuity examination under 5 years of age (p<0.025). 4. The final corrected visual acuity recovered was above 0.8, (with Han's visual acuity chart) in 11 eyes (78.6%) among 14 eyes with high hyperopic amblyopia under 5 years of age However above 6 years of age, only one eye (12.5%) among 8 eys was correctable. 5. As the degree of astigmatic errors increased, the final corrected visual acuity decreased (p<0.025).

8.A Case of Chordoma in the Sacro: Coccygyeal Area

Chordoma is a rare distinctively malignant tumor thought to be histogenetically related to the primitive notochord, It is locally infiltrative but does not metastasize until late. The usual initial symptom is pain or some symptom caused by involvement of the nervous system. Involed in the order of frequency are the sacro-coccygeal area, the spheno-occipital area, and the other vertebral area. This case is a sacro-coccygeal chordoma that is confirmed by histologic examination. The lesion was treated by complete excision.

9.A Case of Congenital Solitary Morphea Profunda.

A 4-year-old boy has had a solitary sclerotic depressed plaque on the right anterior chest since birth. The histopathologic findings are consistent with morphea profunda: thickening, hyalinization, and homogenization of collagen bundles in the dermis and subcutaneous tissues, admixture with a prominent lymphocytic and plasma cell infiltrate, and sweat glands en-trapped between the thickened collagen bundles. We report a case of congenital solitary morphea profunda.

10.The Expression of Vascular Endothelial Growth Factor, Kinase Domain Region, and Transforming Growth Factor-beta 1 in Cervical Neoplasia.

OBJECTIVE: Angiogenesis is a critical factor in the progression of solid tumors. The mechanisms responsible for angiogenesis in cervical neoplasia, however, are not well defined. Our study was aimed to determine the expression of VEGF(Vascular Endothelial Growth Factor), its receptor(KDR), and TGF-beta1(Transforming Growth Factor-beta1) in cervical neoplasia, to determine the role of these angiogenic factors in preinvasive(dysplastic) process and the progression of cervical cancer and to investigate the progression of angiogenesis in the transition from normal cervix to invasive squamous cell carcinoma of the uterine cervix. METHODS: The cervical lesions of 76 patients were punch biopsied and paraffin embedded. Among these, 5 were normal cervix, 36 were cervical intraepithelial lesion I-III, and the other 35 were invasive squamous cell carcinomas. The tissues were immunostained with antiVEGF, antiKDR, and antiTGF-beta1 polyclonal antibody. RESULTS: The expression of VEGF, KDR, and TGF-beta1 in CIN III was stronger than those of CIN I(p<0.01). Their expression were not significantly different among the each staged cervical cancers(p>0.01). CONCLUSIONS: These observations suggest that VEGF, KDR, and TGF-beta1 are important angiogenic factors in cervical neoplasia, especially in an early event to neoplastic transformation of cervical tissues, but these angiogenic factors are not associated with the progression of cervical cancer.
