A review of all 919 perinatal deaths occurring in II Sin Christian Hospital From 1985 to 1989 was carried out. The results of clinical analysis were as follows: 1) The overall perinatal mortality rate was 26.30 per 1,000 birth. 2) The perinatal mortality rate was lowest in the gestation group between 37~41 week and in the weight group between 2,501~4,000 gm. 3) The perinatal mortality rate was high in the age group under 20 and 40 and more. 4) The perinatal mortality rate was increased with an increasing number of parity. 5) The majority of neonatal deaths occurred within 24 hours of life, and the most common cause of death was prematurity. 6) The most common congesital anomaly was multiple anomaly, and the most common single anomaly was anencephaly. 7) In pregnancy, the most common maternal complication was anemia.

4.A clinical study of congenital anomalies in births associated with hydramnios.

A study was made in the congenital anomalies in 137 babies born to 120 mothers with hydramnios, delivered at Ilsin Christian Hospital between Jan. 1st 1981 and Dec. 31th 1990. The results were as follows; 1) The incidence of hydramnios was 1.6/1000 deliveries, 120 cases in total 73, 129 deliveries. 137 infants was born and of these 71 (51.8%) had congenital anomalies. 2) Hydramnios was assiciated with a high incidence of prematurity and low birth weight infant. 3) The incience of multiple anomalies was 54.9% (39 out of 71 infants). The most common system involved with the congenital anomalies was the musculoskeletal system, 28.9% (39 cases out of 135) and the most frequent anomaly was anencephaly, 15 cases. 4) The perinatal mortality rate was 759/1000 total (104 cases) and of these 45 cases, 43.3% were associated with congenital anomalies. 53 of the live birth had congenital anomalies and of these 27 cases (50.9%) died in the neonatal period.

5.The Effectiveness on Prevention of Rehospitalization with Long-Term Psychosocial Interventions for Patients with Major Psychiatric Disorders

OBJECTIVES: This study aimed to elucidate the effectiveness of long-term psychosocial intervention in reducing the disabling period of patients with major psychiatric disorders by their rehospitalization rate. METHODS: Of the 210 patients with major psychiatric disorders received psychosocial interventions in a Mental Health and Welfare Center, 192 patients (147 with schizophrenia spectrum disorders, 45 with mood disorders) who received interventions more than 6 months were selected. Review of case management records was conducted to obtain information. RESULTS: The number and length of hospitalization and the hospital days per year significantly decreased after psychosocial intervention. Additional analysis of 102 patients followed up for more than 5 years suggested that the effectiveness of the intervention persisted for a sufficient period. However, no significant difference was observed in the number of rehospitalization in 45 patients with mood disorders, though the length of hospitalization significantly decreased. In addition, the hospital days per year of 21 patients with mood disorder followed up for more than 5 years also showed no significant decrease. CONCLUSION: Long-term psychosocial intervention had a significant effect on reducing the number and length of hospitalization for patients with major psychiatric disorder and the effectiveness maintained for more than 5 years.

6.Protective Action of Purinergic and Cholinergic Agonists on the Ischemic Myocardium in the Rat.

BACKGROUND: Purinergic and cholinergic agonists elicit negative-inotropic and chronotropic effects, anticip-ating their protective action from the damage of overloaded myocardium. However, the actions of the agents during the ischemic insults are not yet clearly informed. The aim of this study was to investigate the role of the purinergic and cholinergic agonists on the simulated ischemic myocardium of the rat atrial fiber preparations. METHOD: Various action potential parameters (maximum diastolic potential MDP;action potential amplitude APA;velocity of phase 0 depolarization dV/dtmax;action potential duration APD90) were measured and compared in electrically paced, normal (NPSS) and modified physiological salt solution (MPSS) superfused rat atrial fibers in vitro, using conventional 3M-KCl microelectrode technique. Ischemia-simulated modified physiologic solutions were prepared by changing the solution's composition. RESULTS: Hypoxic-and/or hyperkalemic-MPSS decreased all the action potential (AP) variables. However, no significant changes of the AP variables were developed by the acidic-or glucose-free MPSS. Adenosine (Ado) and cyclopentyladenosine (CPA) only decreased the APD90 in a dose-dependent manner. Acetylcholine (Ach) and carbachol (Cch) hyperpolarized the MDP, increased the dV/dtmax with certain doses, and decreased the APD90 dose-depen-dently. The potency for APD90-decrease was greater in order, CPA>Cch>Ach>Ado. Ado and CPA did not affect the hypoxic, hypokalemic MPSS-induced dV/dtmax-decrease. On the other hand, Ach and Cch sig-nificantly inhibited the dV/dtmax-decrease by the hypoxic hypokalemic-MPSS. Ado, CPA, Ach and Cch sig-nificantly augmented the hypoxic, hypokalemic MPSS-induced APD90-decrease. The inhibition by the Ach and Cch on the MPSS-induced dV/dtmax-decrease was not affected by DPCPX, but atropine significantly attenuated the inhibition by the cholinergic agonists. DPCPX inhibited the augmentation by the Ado and CPA on the MPSS induced APD90-decrease, and atropine inhibited the effect of the cholinergic agonists. CONCLUSION: Both purinergic and cholinergic agonists not only shorten the AP duration by themselves but also enhance the AP-shortening effect elicited by the ischemia, and therefore, it is inferred that both agonists prevent further tissue damage from the ischemic insults.

7.A Case of Neuronal Heterotopia.

Neuronal migrational disorders of the brain represent abnormalities in the formation of the neocortex caused by faulty migration of the subependymal neuroblasts. The neuroblasts normally migrate between the sixth and 15th gestational week and in doing so form the six-layered neocortex. When the migration does not occur in a normal fashion the resultant brain anomalies include lissencephaly, pachygyria, schizencephaly, hemimegalencephaly, heterotopia, and polymicrogyria. Neuronal heterotopia is a collection of nerve cells in abnormal locations as a result of arrest of their radial migration, improper formation, or destruction of the radial glial fiber. We reported a case of neuronal heterotopia with brief review of related literatures.

8.Ovarian Artery Rupture Presenting with a Spontaneous Retroperitoneal Hemorrhage of an Anticoagulated Patient: A Successful Treatment with Embolization

Retroperitoneal hemorrhage is a potentially lethal complication in patients on anticoagulant therapy and can be caused by ovarian artery bleeding, regardless of the patients’ age and obstetric history. This case illustrates the clinical presentation of ovarian artery bleeding in a postmenopausal female on anticoagulant therapy, the diagnostic utility of transcatheter angiography, and successful embolization.
9.Ultrasound-Guided Radiofrequency Thermal Ablation of Normal Kidney in a Rabbit Model: Correlation with CT and Histopathology.

PURPOSE: To assess the feasibility and safety of using a cooled-tip electrode to perform percutaneous radiofrequency ablation of kidney tissue in rabbits, and to evaluate the ability of CT to reveal the appearance and extent of tissue necrosis during follow-up after ablation. MATERIALS AND METHODS: Using ultrasound guidance, a 17-G, cooled-tip electrode was inserted into the right lower portion of the kidney in 26 New Zealand White rabbits. Radiofrequency was applied for 2 mins, and biphasic helical CT scanning was used to assess tissue destruction and the presence or absence of complications immediately after the procedure and at 24 hrs, 2 and 3 days, and 1, 2, 3, 4, 5, 6 and 7 weeks. The study had three phases: acute [immediately killed : N=10]; subacute [killed at 24 hrs (n=3), 2 days (n=3), 3 days (n=1) : N=7]; chronic [killed at 1 week (n=4), 2 weeks (n=2), 4 weeks (n=1), 7 weeks (n=1) : N=8]. After the animals were killed, their kidneys were histopathologically examined and the radiologic and pathologic findings of lesion size and configuration were correlated. RESULTS: In each instance, ultrasound-guided radiofrequency ablations of the lower pole of the kidney were technically successful. Contrast-enhanced biphasic helical CT revealed regions of hypoattenuation devoid of parenchymal enhancement, and these correlated closely with true pathologic lesion size (r=0.884; p>0.05). In subacute and chronic models, CT scanning revealed gradual spontaneous resorption of the ablated lesion and the presence of perilesional calcification. Histopathologically, in the acute phase the ablated lesions showed coagulative necrosis and infiltration of inflammatory cells, and in the chronic phase there was clear cut necrosis of glomeruli, tubules and renal interstitium, with diminishing inflammatory response and peripheral fibrotic tissue formation. CONCLUSION: Ultrasound-guided renal radiofrequency ablation is technically feasible and safe. In addition, the avascular lesion measured at contrast-enhanced helical CT closely correlated with the size of ablated tissue. Contrast-enhanced CT may therefore be used for serially monitoring the effect of radiofrequency ablation. In the future, RF ablation may offer an alternative treatment option for renal cancer.

10.Color Doppler Ultrasonography in the Evaluation of the Acute Scrotum.

Thirty seven patients with acute scrotal pain were evaluated by color doppler ultrasonography. The diagnosis was made by clinical finding in 26 cases and surgical finding in 11 cases. There were inflammatory diseases in 22 cases, testicular torsion in 5 cases, other diseases in 10 cases. 20 cases of inflammatory disease showed increased blood flow in color doppler ultrasonography and 4 cases of testicular torsion showed absent blood flow. Thus, color doppler ultrasonographic evaluation had diagnostic value that was 80% in sensitivity and 96% specificity in differentiation of testicular torsion from other acute scrotal diseases. We think that color doppler ultrasonography in acute scrotum is a valuable diagnostic method for differentiation of testicular torsion from other acute scrotal diseases.
