2.The Research on the Breast Feeding and its Related Factors of Premature Infant.

PURPOSE: The purpose of this research is to provide basic informations for the encouragements of premature infants' breast feeding. METHOD: From August 10 to October 9, 2002, we have carried out a statical research which surveyed 148 mothers of premature infants registered in NICU. The sample had generated cluster-randomly from 25 General Hospitals, all over the Korea peninsula and being surveyed with 74 questionnaires. RESULT: Mean hospitalized day of premature infants was 27.9 days. The mean total feeding period was 19.1 days and continuous breast feeding period 12.4days. They were interested in breast feeding education-they answered that they would join the breast feeding education if they were given the chance 87.8%. There was significant relation between babies fed only breast milk and the body weight of birth(p < .05). The reasons why mother gave the baby her breast milk include 'for her baby's health' and 'people said breast milk is good for babies' with a portion of 99%. The main reason why mothers could not execute breast feeding was 'the deficit of breast milk volume' 50.0% and other reason were 'because of start to support more nutritions '18.2%, 'difficulty to carry out the breast milk to hospital'13.6%. The reasen why mothers could not try breast feeding at first were 'deficit of breast milk volume'37.0%, 'not to be prepared for breast feeding because of unexpected delivery'32.6%. CONCLUSION: We need a program to inform importance and excellent of the breast feeding and a plan to increase the premature infants' breast feeding through the importation of fortifier.

3.A Case of Hydranencephaly Caused by Internal Carotid Artery Stenosis: Diagnosis with Doppler Sonogram.

Hydranencephaly is congenital absence of the cerebral hemispheres which are replaced by a large fluid-filled cavity. The brain stem and basal ganglia are well formed and rudiments of frontal k occipital cortex may be present. We experienced a case of hydranencephaly caused by both internal carotid artery stenosis. We diagnosed it through the brain CT sonogram and doppler sonogram. A brief review of the related literatures was made.

6.The Accuracy of Pulse Oximeter in Predicting the Arterial Oxygen Saturation.

We studied 21 neonates who required mechanical ventilation during study period in NICU OF Il Sin Christian Hospital with diagosis of prematurity, IRDS, pneumothorax and diaphragmatic hernia to evaluate the accuracy of pulse oximeter in predicting the arterial oxygen saturtion, hypoxia and hyperoxemia. We also studied whether the changes of birth weight, hematocrit, blood pressure and body temperature affect the accuracy of pulse oximeter. The results are 1) Linear regression analysis revealed a close correlation between in vivo pulse oximeter readings and in vitro SaO2 measurements in patients (Y=0.008X+12.34, r=0.946) 2) The changes of birth weight, hematocrit, blood pressure and body temperature did not affect the accuracy of pulse oximeter. 3) SpO2-PaO2 relationship was similar to the SsO2-PaO2 relationship. 4) When Nellcor N-100 pulse oximeter was setted at alarm limit 96% to avoid hyperoxemia, it identified 16 of 22 hyperoxemic instances (sensitivity 73%) and alarmed falsely in 48 of 92 nonhyperoxemic instances (specificity 59%).

7.Schilder’s disease in a young child with tumefactive demyelinating brain lesion

Schilder’s disease is a rare sporadic demyelinating disease of the brain. We report a girl with Schilder’s
disease who had undergone Kasai operation for biliary atresia. The patient presented with acute right
hemiparesis. Brain magnetic resonance imaging (MRI) showed a single large tumefactive white
matter lesion. A diagnosis of Schilder’s disease was based on clinical features and MRI findings. The
patient showed dramatic clinical improvement and significant regression of the lesion in response to
high-dose intravenous methyl prednisone, and remained free of relapse of other neurologic problems
during the 3-year follow-up.

V. parahaemolyticus was isolated from blood culture of a 34-year old female patient with HCV viral hepatitis and liver cirrhosis. V. parahaemolyticus is one of the frequent causative agents of gastrointestinal infection, but rarely causes septicemia. This case is thought to be the 3rd report of V. parahaemolyticus septicemia in Korea.

10.Comparison of spread of subarachnoid sensory block and incidence of hypotension in early and late second trimester of pregnancy.

BACKGROUND: Obstetric or non-obstetric surgery can be performed in pregnant women during the second trimester. We evaluated maximal sensory block level (MSBL), and other sensory block characteristics after spinal anesthesia in early and late second trimester pregnant women. METHODS: Forty-four pregnant women scheduled for cervical cerclage under spinal anesthesia in the early second trimester (Group E) or in the late second trimester (Group L) were enrolled in this study. Spinal anesthesia was performed at the L3/4 and 7 mg of 0.5% hyperbaric bupivacaine was injected into the subarachnoid space. Hemodynamic variables, incidence of nausea/vomiting, ephedrine dose, and sensory block were recorded every 2.5 min during the first 15 min and 20 min after the injection. The MSBL, the time at which MSBL was achieved, and the time to the two-segment regression of the sensory level were also recorded. RESULTS: The maximum number of segments blocked was significantly greater in the Group L than in the Group E. The incidence of nausea/vomiting and hypotension, and dose of administrated ephedrine were significantly higher in the Group L than in the Group E. The mean arterial pressure during the 15 min after subarachnoid injection was significantly lower compared to the baseline value in the Group L. CONCLUSIONS: The MSBL of spinal anesthesia with hyperbaric bupivacaine 7 mg were T9 in the early and T5 in the late second trimester groups. Pregnant women in the late second trimester exhibited increased incidence of hypotension and need for ephedrine than women in the early second trimester.
