2.A Case of Eosinophilic Myocarditis associated with Hypereosinophilia showing Myocardial Edema.

Chusid et al proposed diagnostic criteria of hypereosinophilic syndrome (HES) that remain valid today. These were, (1) a sustained peripheral blood eosinophil count of more than 1500/L present for longer than 6 months ; (2) no evidence of other apparent causes for eosinophilia, and (3) presumptive signs of parenchymal organ involvement. Any organ system may be affected in HES, but the most severe clinicopathological involvements are of the heart and nervous system. Although multiple organ systems may be involved, the most common cause of morbidity and mortality is cardiac involvement with extensive fibrous thickening of the endomyocardium and overlying thrombus. We report a case of acute peri-myocarditis with eosinophilia, which was confirmed as eosinophilic myocarditis by endomyocardial biopsy, with literature review.

4.Subungual Exostosis.

Subungual exostosis are not uncommon, however, they are infrequently mentioned in the dermatologic literature. We report herein a rase of subungual exostosis in the great toe of 16-year-old female student which was confirmed by histopathologic and radiologic findings. Histopathologic examination showed that the mass was ooeered by a dense fibrous tissue which merged into a fibrocartilage cap and bone. Computed tomography showed a well defined, oval shaped, radiopacity of bony density capped by a radiolucency.

6.A Case of Cervical Tab.

Cervical tab is one of the congenital anomalies, originated from an accessory tragus or a remnant of branchial cartilage on the head a.nd neck. A 41 year old male patient was seen with 1.1 x0.8cm sized cartilage consistent intredermal nodule on the left supraclavicular area. Histopathologic findings showed the oval shaped cartilags surrounded with connective tissue and interstitial substance stained black with Verhoeff van Gieson stain.

7.Development of a quality of life questionnaire for Korean asthmatics.

BACKGROUND AND OBJECTIVE: The outcome of asthma treatment has traditionally been assessed by measuring the conventional clinical parameters such as symptoms, expiratory flow rates and needs for medications. But none of them tells us whether the patients are actually able to function properly in their lives. Only assessment of the quality of life (QOL) can reflect the parts of patients lives affected by the disease. However, there is no measure to assess the quality of life of patients with any chronic diseases in Korea yet. We developed the QOL Questionnaire for Korean Asthmatics and evaluated its measurement properties. METHOD: The questionnaire was composed of 18 items in four domains : i.e. activity limitation (6 items), symptoms (5 items), emotional function (3 items) and environmental stimuli (4 items). At our out-patient clinic, 32 adults with moderate to severe persistent bronchial asthma were enrolled. The study design consisted of 4 week unblinded single cohort with visits at 0 and 4 weeks, and the patients were asked to answer the questionnaire by self administration method at the two separate visits. Forced expiratory volume for 1 second (FEV1) and peak expiratory flow rate (PEFR) were recorded at each visit. According to the patients and physicians global assessment, the patients were divided into two groups, i.e. stable and changed (improved). The responsiveness, reproducibility and validity of the questionnaire were evaluated. RESULTS: The total QOL scores at the second visit in the changed group were significantly higher than those at the first visit. The score differences between the two visits in the changed group were higher than in the stable group. In the stable group, the responses to the questionnaire were reproducible. The QOL scores showed good longitudinal correlation with the expiratory flow rates, whereas FEV1 did not correlate with the QOL in cross-sectional analysis. CONCLUSION: The questionnaire proved to be reproducible, responsive and valid in the subjects. It could be recommended for the outcome evaluation of Korean asthmatics.

8.Adenoid Cystic Carcinoma of the Male Breast: A case report.

A 65 year-old male patient presented with a large palpable mass beneath the areola of the right breast for 7 years. The resected breast tissue was almost totally replaced by a round large solid mass (9 6 cm) with a pink-gray to yellow firm, partly nodular cut surface. Microscopically, the tumor revealed the diagnostic biphasic cellular pattern of adenoid cystic carcinoma, which consisted of both cribriform pattern of myoepithelial cells and tubular pattern of epithelial cells. On immunohistochemistry, the tumor revealed immunoreactivities for alpha-smooth muscle actin and S-100 protein in the myoepithelial cells and for AE1/AE3 in the epithelial cells. Mitoses were scarce. Multifocal lymphatic permeation and foci of perineural invasion were also found. Underlying resection margins and overlying skin were invaded by the tumor. We diagnosed this tumor as grade II adenoid cystic carcinoma according to the system utilized for the salivary gland tumors.

9.Effects of Scleral Buckling on Eyeball in Young Rabbits.

There has been a series of reports indicating that scleral buckling[SB] surgery may induce high myopia in advanced retinopathy of prematurity. The mechanism of SB on refraction, however, has not been clearly known. We investigated the effects of SB on eyeball in young rabbits, including mechanism of refractive change. In 9 rabbits with 5 weeks old[Group A]and 11 rabbits with 8 weeks old[Group B], SB was performed for right eyes of all rabbits with encircling band, 2.5 mm in width.Left eyes of these were followed up for control group. Spherical equivalent, corneal power, and axial length were measured before SB and at 2, 4, and 8 weeks postoperatively. We compared these measures between experimental and control groups, and analyzed the influence of age at operation. In control group, corneal refractive power was decreased, axial length was increased, and spherical equivalent developed emmetropization in aging. High myopia was developed in all eyes with SB, and the increase of axial length was the major factor in myopia.These changes were greater in group A than group B.In experimental group, the degree of myopia was increased at 4 and 8 weeks postoperatively, but corneal power and axial length were not statistically changed. In conclusion, the SB in younger rabbits induced higher degree of axial myopia.These findings may suggest that SB in infant could induce significant axial myopia and secondary amblyopia, especially younger age.
