1.A Case of Slipped Femoral Capital Epiphysis

Slipped femoral capital epiphysis is the condition in which the femoral head begins to slip gradually off the femoral neck. After first discription of Ambrose Pare(1572), the world literature has presented many cases, usually more than one hundred in each clinic. Its incidence in Korea, however, is extremely rare and particularly it is difficult to find out the report of typical case. The author reports here a typical case of billateral slipped femoral capital epiphysis in a 17 year old boy with symptom duration of 2 years on right side and 6 months on left side. Physical examination showed his body stature to be normal and his genital organs to have no abnormalities. However, both hips revealed severe pain and limited range of motion, especially in internal rotation and abduction, which restricted his normal gait. Operative reduction of epiphysio-cervical distorsion was successful in the left hip. After skeletal traction of the left hip, an anatomical repositioning by intraarticular wedge rection osteotomy was maintained by 3 Knowles pins. 3 months postoperatively he resumed partial weight bearing on his left hip with little pain and improved range of motion. Further restoration of function can reasonably be expected.

3.A Rapidly Skeletonized Dead Body in Late Summer: A Case Report.

In the following report, we describe the case of a rapidly skeletonized corpse discovered in late summer. On the first day of September, a 57-year-old man was found almost completely skeletonized on a hill behind his house. His body lay prone, accompanied by a brush cutter at the entrance to the graveyard where his friend's parents were buried. His cap, clothes, and rubber boots were all intact and undamaged. Upon autopsy, no definite abnormalities were identified except for an old linear fracture on the frontoparietal bone. According to his friends and neighbors, he had likely died 8 or 9 days prior to the discovery of his body.

5.Beta Dosimetry in Intraperitoneal Administration of 166Ho-chitosan Complex.

Intraperitoneal adminstration of radioisotopes is suggested to treat the metastatic ovarian cancer in the pertioneal cavity. Administering beta-emitting radioisotopes into the pertioneal cavity allows the maximum energy delivery to the cancerous cells of the pertioneal wall surface while sparing the normal cells located in deep site of the peritoneal wall. In this study, dose estimates of the peritoneal wall are provided to be used for prescribing the amount of 166Ho-chitosan complex administered. The 166Ho-chitosan complex diffused in the peritoneal fluid may attach to the peritoneal wall surface. The attachment fraction of 166Ho-chitosan complex to the peritoneal wall surface is obtained by simulating the ascites with Fischer rats. Both volume source in the peritoneal fluid and the surface source over the peritoneal wall surface are counted for the contribution to the peritoneal wall dose. The Monte Carlo code EGS4 is used to simulate the energy transfer of the beta particles emitted from 166Ho. A plane geometrical model of semi-infinite volume describes the peritoneal cavity and peritoneal wall. A semi-infinite plane of 10 micrometer in thickness at every 1 mm of depth in the peritoneal wall is taken as the target in dose estimation. Greater han 98 percents of attachment fraction has been observed from the experiments with Fischer rats. Given 1.3 microcurie/cm2 and 2.4 microcurie/ml of uniform activity density, absorbed dose is 123 Gy, 8.59 Gy, 3.00 Gy, 1.03 Gy, and 327 Gy at 0 mm, 1 mm, 2 mm, 3 mm, and 4 mm in depth to the peritoneal wall, respectively.

6.Congenital Laryngeal Atresia: An autopsy case.

Atresia of larynx is a rare fatal anomaly that should bring an immediate medical attention for proper managenent. We reported a case who died in neonatal period because of respiratory difficulty. His first problem was difficulty of inserting tracheal tube through the larynx. It was of interest in this case that he was presented with generalized edema and also massive lung edema. The lung was characterized by total absence of squamous and amniotic debris in the alveolar spaces and massive inflation of the alveoli by clear fluid that was thought to be amniotic fouid produced by the lung per se. Because there was no connection between oral cavity and the lungs, there would be no way the amniotic fluid outside the fetus. The laryngeal atresia was of infraglottic type and was complete with dispalced cricoid cartilage. Associated anomalies were left persistent supperior vena cava, perimembranous ventricular septal defect, spina bifida and focal cerebellar heterotopia.

7.Clinical experience with protermycin in genitourinary diseases.

Protermycin, the first molecular synthetic of tetracycline and chloramphenicol, has been given trial in 28 patients: acute gonococcal urethritis 10, nongonococcal urethritis 7, acute cystitis 6, chancroid 1 and postoperative cases 4. The doses ranged from 300 to 600 me daily. From this study, it was found that Protermycin proved itself to be most effective in nongonococcal urethritis which could not be treated satisfactorily by other antibiotics. Of 28 cases, 25 showed good result. The remaining 3 cases failed to respond.

8.A case of delayed radiation-induced lumbosacral radiculoplexopathy after 30 years

Lumbosacral radiculoplexopathy in colon cancer treatment is a very rare but serious complication
after radiation. We report here a 66-year-old man with slowly progressive lower limb weakness which
arose 30 years after local radiation treatment for colon cancer. Electrophysiological studies revealed
signs of denervation confined to the lower limbs. Other causes were excluded by clinical presentation,
serological, cerebrospinal fluid and imaging studies. This case shows that delayed radiation-induced
lumbosacral radiculoplexopathy can occur 30 years after the initial treatment.

9.A Radiological Study of the Elbow in Professional Baseball Players

Bony changes about the elbow in the baseball players are common problem. Pure bony injuries occur in the elbow of the throwing arm, but most often they have some associated soft tissue counterpart. The importance to understand the elbow lesion in baseball players is that many of this lesion seem mild at onset and can often be easily managed, but permanent disability can result from the delayed or inadequate treatment. The authors have performed radiological study in 32 professional baseball players and obstained following result. 1. Among 32 baseball players we studied, twenty-four plsyers(75.0%) had 41 bony changes in the throwing elbow. 2. Forty one bony changes were classified into three groups by Slocum classificstion,;twenty nine medial tension overload injury, eleven extensor overload injury and one lateral compression injury. 3. Incidences of bony changes in each age group were 63.6#% in 20~24 years, 71.4% in 25~29 years and 100% in 30~35 years of age. 4. Incidences of bony changes in each csreer group were 63.6% in 10~14 years, 69.2% in 15~19 years and 100% in 20~25 years of career. 5. Incidences of bony changes in each position were 72.7% in the piteher and 76.2% in the fielder. 6. In conclusion, the best treatment is prevention, namely, exercise to increase the stren- gth, flexibility, and endurance of elbow musculature, which should be coupled with attention to proper mechanics of throwing. If an injury does take place, prompt treatment consisting rest, physical therapy and appropriate medicstion when indicated-should lead to a successful result in most cases.

10.An Experimental Study for The Biomechanical Properties and Histologic Reactions of Carbon Fiber Reinforced Epoxies(CFRE) * Implant

Histologic reactions of bone and soft tissue to CFRE were explored, and biomechanical properties of both CFRE plate and stainless steel plate were tested and compared. Six achilles tendons of albino rabbits were replaced with CFRE implant after artificial rupture and serial histologic examination of neotendon and histologic response in vivo were done during 8 weeks. Proximal tibia of twelve albino rabbit was selected, subjected the tibia a single osteotomy and internal fixation with CFRE plate in six cases, and stainless steel plate in the others. And then, consequent radiographic and histologic change was observed during 8 weeks. Objectives in this paper is to provide basic data of CFRE in its biomechanical and biologic response in vivo. The results were as follows :1. Foreign body reaction and fibroblast were decreased at 7-8 weeks after operation. Early formation of collagen, which is similar to that found in the normal side, was shown. So, CFRE seems to be ideal material in the aspect of tissue tolerance among biocomaptibility factors. 2. In biomechanical test, CFRE plate is lower than stainless steel plate in flexural strength and flexural modulus. This means that CFRE plate is semirigid in nature. 3. While external callus formation is marked after fixation of CFRE, internal callus for mation is marked after fixation of stainless steel plate. In CFRE plated tibia, the resorption-cavity is much smaller than stainless steel plated, and there is some formation of woven bone beneath the plate. In conclusion, CFRE is ideal material in the aspect of its biomechanical compatibility and tissue tolerance. Further efforts on production and varying design of CFRE make it to use widely in clinical application, Such as internal fixator of fracture and temporary replacement of tendon or ligament.
