2.Efficacy and safety of budesonide turbuhaler in Korean asthmatic patients.

A controlled study was carried out in 50 patients with perennial bronchial asthma to assess the efficacy and safety of budesonide turbuhaler. Subjects have suffered from cough, wheezing, dyspnea and chest tightness and showed either 15% of reversibility in FEV after bronchodilator inhalation or airway hyperresponsiveness to methacholine(PC20 < or = 25mg/ml.) Patients were randomized to treatment with budesonide turbuhaler or terbutaline turbuhaler for 8 weeks after 2 weeks of run-in period. Budesonide turbuhaler was effective for cough, wheezing, dyspnea and chest tightness. It improved peak expiratory flow rate and FEV1. Budesonide turbuhaler was tolerated well and the laboratory tests showed no abnormality. It is suggested that budesonide turbuhaler is effective and safe in the management of bronchial asthma.

5.Arthroscopic Adhesiolysis of Partial Knee Ankylosis

Partial knee ankylosis is a recognized complication of following open operative procedure, trauma and disease processes about the knee. Intraarticular trauma or surgical procedures such as TKR, synovectomy or patellar fracture were main source of intraarticular or extraarticular soft tissue adhesion and contracture were resulted from distal femoral and proximal tibial fractures. The purpose of this study is to report the value of arthroscopic adhesiolysis of partial knee ankylosis and to evaluate the clinical results of its application in various conditions. From October 1982 to December 1992, 56 cases in 54 patients who had severe limitation of motion in the knee joint following open operative procedure or trauma about the knee were treated by fibroarthrolysis under the arthroscopic control. For release of thick fibrous tissue, we used our designed blunt metal bar. The following results were obtained. There were 30 males and 24 females and the age at the time of release ranged from 20 to 69 years old(average 38.7 years old). The interval between last open operative procedure or trauma and arthroscopic adhesiolysis ranged from 3 months to 6 years(average 16 months). The interval between arthroscopic adhesionlyssis and the last follow up evaluation ranged from 1 year to 9 years(average 49 months), The average preoperative range of knee motion was 42.2 and the postoperative range of motion under the anesthesia was 115.5 and the average final knee range of motion at follow up was 101.3. So the average loss of motion between postoperative and final motion was 14.2. Complications were one case of patellar and tibial condylar fracture respectively. In the author's experience, arthroscopic adhesiolysis seems to be applied to the various condition of the partial knee ankylosis and the results are better than the other surgical procedures when it was performed early enough16).

6.A Case of Acanthoma Fissuratum Occurring on the Mucosa of the Upper Lip.

Acanthoma fissuratum is a localized thickening of the skin in response to pressure by an eyeglass frame and usually occurs on the retroauricular fold, superior auricular crease, or bridge of the nose. The definitive diagnosis can be confirmed by characteristic clinical and histopathological features in association with anamnestic data. Herein we report a case of acanthoma fissuratum developing on an unusual site-the mucosa of the upper lip and it was successfully treated with simple excision.

8.Safety of Intravitreal Ciprofloxacin in Phakic Rabbit Eyes Determined with Electroretinography.

This study was conducted to determine the safe intravitreal dosage of ciprofloxacin. Twenty-four phakic eyes of New Zealand pigmented rabbits were used. Each group(4 eyes) received midvitreal ciprofloxacin of 100, 200, 400, 600, 800 micro gram in 0.1ml BSS Plus, or 0.1ml BSS Plus only as control. We evaluated retinal function by measuring the electroretinograms for a graded series of flash intensities and fitting b-wave amplitudes to the Naka-Rushton equation. At a dose of greater than 600 micro gram, Rmax decreased signifantly and log K increased signifantly. N-value decreased slightly. B-wave amplitude decreased as a toxic response of intravitreal ciprofloxacin in a dose dependent manner, and this response was best detected using lower luminance stimuli. Lower luminance electroretinography revealed a significant decrease in b-wave amplitude in eyes injected with a dose of 400 micro gram or more. We concluded that 200 micro gram will be the safe intravitreal dosage of ciprofloxacin in phakic rabbit eyes.
