3.Malignant Fibrous Histiocytoma of the Heart: A case report.

A case of a 58-year-old man with malignant fibrous histiocytoma (MFH) of the right ventricle is reported light and electron microscopically. This is the first case of MFH of the heart in the Korean literature. A tendency for malignant fibrous histiocytoma of the heart to occur in the left atrium of young women is suggested; this sarcoma's usual location is in the soft tissue of elderly men. The tumor consisted of spindle cells arranged in a focal storiform patterns, clusters or sheets of histocyte-like cells, benign and malignant giant cells, inflammatory cells, scattered mitotic figures and anaplasia of stromal cells. In ultrastructure the constituent of cells of the tumor are primitive mesenchymal cells, histiocytoid cells and fibroblast like cells including giant cells.

5.HAIR TRANSPLANTATION FOR MALE PATTERN BALDNESS AND OTHER ALOPECIAS.

The increasingly successful results of hair restoration surgery in the last years have developed the interest and the confidence of patients and it is the most frequently performed esthetic surgery among male patients. Currently, various kinds of techniques for hair restoration surgery including hair transplantation, scalp reduction, and scalp flaps are performed in our clinic. Among these, hair transplantation is the basic and the most popular procedure, which can be performed not only by itself but also in conjunction with other procedures. 123 consecutive cases of hair transplantations were peformed from Jan. 1995 to Feb. 1996 for male pattern baldness, traumatic alopecia, and female pattern baldness under out patient base. The authors introduced a new classification for male pattern baldness, which are the type M, O, C, U, M-O, and C-O after alphabet to make simple and easy for clinical application. The ancillary procedures were scalp reduction, preauricular flap, and scalp expansion. An ellipsis of hair bearing scalp taken from the occipital area was sliced into slit-, mega-, mini-, and micro-grafts. The survival rate of the grafts was over 90% with minimal complications. This hair transplantation technique enabled us to achieve a good density and more natural looking hair with avoidance of cobble stoning and apparent scar.

6.Malignant Mixed Germ Cell Tumor and Contralateral Gonadoblastoma in Turner's Syndrome, 45, X0/46, XY Karyotype: A case report.

Turner's syndrome results from complete or partial monosomy of the X chromosome and is characterized by hypogonadism or related other congenital anomalies in phenotypic females. In these patients, there are failure to develop normal secondary sex characteristics, amenorrhea, or short stature at puberty and the ovaries are reduced to atrophic fibrous strands devoid of ova and follicles(streak gonads). Individuals with this condition are particularly prone to the development of gonadoblastoma. For this reason, the gonads should be early removed and supplemental estrogen therapy given. We experienced a case of Turner's syndrome, 45, XO/46, XY karyotype in a 20-year-old phenotypic female complained an amenorrhea. On the exploratory laparotomy, the right gonadal mass is sevearly adhered to the adjacent organs and measures 8 x 5 x 5 cm in dimension and 75gm in weight and shows multiple foci of hemorrhage with necrosis. The left streak gonad measures 3.5 x 2 x 1.5 cm in dimension and shows multiple foci of calcification. Microscopically, the right gonadal mass reveals malignant mixed germ cell tumor, composed of endodermal sinus tumor, composed of endodermal sinus tumor with dysgerminoma and gonadoblastoma. The left streak gonad consists of mainly dense fibrous connective tissue and shows some foci of calcification associated with gonadoblastoma. On immunohistochemical and special stainings, the cytoplasm and hyalin droplets of the endodermal sinus tumor component reveal strong positivity to the a-fetoprotein and PAS. After removal of both gonads, the serum level of the a-fetoprotein is markedly down from 1742ng/ml to 2.6 ng/ml.

7.Comparisons of cholera bacilli positive and negative patients in symptoms and laboratory findings among 1991 cholera patients.

Among 130 cholera patients treated in Seohae Hospital in 1991, 86 were cholera bacilli positive and remaining 44 were negative. All cholera bacilli 'positive' patients were confirmed bacteriologically by National Institute for Health team. Cholera related symptoms and laboratory findings were gathered by interviews and medical records surveys. Symptoms and some serological laboratory findings are compared between cholera bacilli 'positive' and 'negative' patients in 1991 cholera epidemic. Results are as follows: 1. There were no differences in symptoms distribution and in mean values of all serological laboratory tests done between two groups. These facts support that both cholera bacilli 'positive' and 'negative' patients are persons who have cholera bacilli infection. 2. The age distribution of the cholera bacilli 'negative' group tended to be much younger than that of 'positive' group. The infection source of this group is believed to be the environmental reservoir. This findings suggests that cholera bacilli 'negative' patients have the possibility of endemic characteristics. We think it is worth investigating Vibrio cholera antibody titers of people in area with frequent cholera epidemics in Korea.

8.Scalp Extender for Hair Replacement Surgery and Treatment of Male Pattern Baldness.

Scalp extension involves the continuous nonvolumetric expansion of the hair-bearing scalp, via biologic creep, using implanted surgical device consists of cutting a 5 cm wide strip of plain silastic (0.02 inch thick) from a larger 16x8 inch sheet, then gluing dacron-reinforced silastic strip (5x1 cm, 0.015 inch thich) to the distal ends. A stainless steel plate from hooks is then secured to each end of the extender. During the period from March 1996 to September 1997, 21 patients including 16 patients of male pattern baldness were treated for alopecia of the scalp using internal scalp extender in the department of plastic surgery Korea university hospital. All cases were successfully reconstructed without major complications. It not only reduces the number of scalp reductions and time necessary to totally excise bald vertex scalp, but also appears to decrease the amounts of stretch back and atrophic scarring that may accompany some reductions. Scalp extension appears to be a useful adjunctive technique in hair replacement and reconstructive surgery, and has distinct advantages over standard scalp reduction and tissue expansion techniques.

9.A Comparative Study of Eccentric and Concentric Isokinetic Exercise Testing.

The understandings of the characteristics of eccentric exercise are very important because all sports and daily activities under the gravity force should be well coupled between concentric contraction and eccentric contraction as accelerator and decelerator respectively. We tested 40 knee flexors and extensors of 20 normal young persons at different anglular velocities of 60o/sec and 120o/sec using Cybex 6000 (A division of Lumax, New York) isokinetic dynamometer in order to know the characteristics of eccentric isokinetic contraction parameters by comparing with concentric isokinetic parameters. Compared to the concentric isokinetic contraction, eccentric isokinetic the peak torque and average power values of knee extensor and flexor muscles showed negative values which means that eccentric contractions were coupled with concentric contractions as decelerator and energy absorptioner. The peak torque values of eccentric contraction were 38 - 61%, 44 - 79% higher in flexor and extensor, respectively than those of concentric contraction and not so decreased as the peak torque of concentric contraction decreased in high angular velocity. The average power values of knee flexor and extensor muscles were definitely low in eccentric contracton than those of concentric contraction but were increased simultaneously with high angular velocity contraction. Further studies about the velocity dependency, muscle soreness, methodology to measure real eccentric activities and other parameters of eccentric contraction are needed imminently.

10.Diagnostic Value of Spinal Computed Tomography Scan.

Since Di Chiro et al. first reported on computed tomography(CT) in syringomyelia, spinal CT has slowly but steadily gained in importance. Recently, application of advanced scanning equipment has led to considerable progress in the evaluation of spinal canal and disc diseases. Since April to December 1982 we have evaluated 96 spine patients by a high-resolution GE 8800 CT scanner. 1) High-resolution non-enhanced CT images provide clear visualization of soft tissue as well as bony structures of the spine, particularly of the lumbar spine. 2) Unenhanced CT of the lumbar spine is highly effective in the diagnosis of herniated disc or the evaluation of bony entrapment syndrome. 3) High-resolution CT aids in the evaluation of destructive neoplastic disease and of burst fracture of the spine. 4) So high-resolution CT is considered the most important noninvasive primary screening test in the diagnosis of herniated lumbar disc or spinal stenosis as well as in the evaluation of primary or metastatic spinal lesion.
