3.Early Extubation after Open Heart Surgery for Congeaital Heart Disease.

We have experienced early extubation after 20 consecutive open heart surgeries for congenital heart disease between May 19 th and May 30 th this year. Eighteen of those 20 had the tracheal tube removed in the operating room immediately after operation. The reanlts were good and satisfactory. Two required postoperative ventilatory support for several hours at RICU. Sufentanil (Sufenta) 10-15 ug/kg was used intravenously as a major narcotic anesthetic for 18 podiatric patients and Fentanyl 50 ug/kg used for 2 adult Patients. The advantages of early extubation are as follows ; It is more physiologic, more comfortable, psychologically leas anxiety provoking for the patients and has less chance of pulmonary infection and less chance of ventilator mishaps arid it also shortens RICU stay and hospitalization. It is concluded that early extubation after open heart surgery for congenital heart disease has minimal risk and definite advantages in carefully selected patients.

4.The Internationalization of the Korean Radiological Society.

Toward the beginning of the twenty-first century, the world experiences the dramatic changes in politics, economics and culture, and it is evident that the Korean medical field will not be able to survive provided it doesn't prepare ourselves to adapt to those changes. The Korean Medical Society held a forum for the active operation of the medical society, inviting several leading affiliated societies, to meet the needs of the times. This review describes the summary of the presentation that the authors made on behalf of the Korean Radiological Society in the forum, including the organization, current status of academic activity, current status of international communication, and problems encountered in the internationalization of the Korean Radiological Society.

5.Subclavian Vein Catherization for Cardiac Surgery in Children .

Subclavian vein catheterization is a well estab1ished technique in adults for central venous pressure monitoring and the infusion of irritant solutions. Its use in small children is less common, preaumably because of technical difficulties in inserting the catheter and the disk of major complications. During cardiac surgery heparinization potentially adds to the risk of hematoma forma-tion. But it is often preferred becauae of the greater stability of the catheter on the anterior chest wall and allows a greater freedom of the neck and upper limb movement. We have experienced 62 open heart surgeries for congenital heart disease between April 14th and Oecember 31th 1986. In 47 children ranging in age from 11 months to 15 years, there was a high succes rate and no morbidity. It is concluded that infraclavicular subclavian vein catheterization is a useful means of measuring central venous presaure and establishing a central infusion line in children undergoing open heart surgery.

6.Clinical experience of cartilage allograft.

Bony defect is one of the most common problems in craniomaxillofacial surgery. Although aurogenous bone graft is the best choice for the treatment of bone defect, it provides many problems such as donor site morbidity, irregular absorption, and limited amount of harvest. To overcome the shortcomings of autogenous bone graft many bone substitutes have been introduced. The ideal bone substitution is to have characteristics such as cheap, easy to obtain, rapid fusion to recipient bone, hard structure, long maintenance of shape and volume, low infection rate, and low exposure rate. Among those bone substitutes which have been widely used we chose lyophilized cartilage allograft because of low antigenecity, low resorption rate, easiness of carving and ling term preservation. From August 1993 to August 1997, 66 patients had been performed craniomaxillofacial reconstruction with lyophilized cartilage allograft. Orbital wall reconstruction and correction of enophthalmos were 24, correction of cleft lip and nose deformity were 19, temporal augmentations were 7, and others 16. Complications such as infection, exposure were not common. And partially removed cartilage was proved some calcification. Radiologic follow-up presented well positioned lyophilized cartilage allograft. Two radiologic works revealed haziness of bone density at the site of cartilage allograft. This suggests the ossification of lyophilized cartilage allograft. Together with liw infection rate, low exposure rate, and good framework for osteoconduction, lyophilized cartilage allograft are regarded as one of the good bone substitutes.

10.Fine structure of Toxoplasma gondii.

The importance of Toxoplasma gondii in human disease stimulated a number of electron microscope studies on the structure of this protozoan parasite. Gustafson et al. first studied the fine structure by means of thin sections in 1954. Many other papers havs subsequantly appeared. It is well known that Toxoplasma gondii has two stages in its life cycle-the proliferative forms and the cyst. The purpose of the electron microscopical work reported here was to study the fine structure of Toxoplasma gondii with recent techniques clarifying the correlation between the proliferative forms and cyst. RH strain and KM strain as proliferative forms on the one hand and Beverley strain as a cyst form of Toxoplasma gondii on the other hand were used throughout this study. The conoid, toxoneme, nucleus, nucleolus, osmiophilic granules, mitochondria and vacuoles were found in RH strain as wsll as in KM strain and Beverley strain. The endoplasmic reticulum was found in the cytoplasm of RH strain and KM strain. It was better developed in KM strain than in RH strain. The outside contour of the organism of Beverley strain was somewhat irregular and toxoneme of this organism was better developed than in the other two strains. Vacuoles were found in RH strain, KM strain and Beverley strain. Furthermore, tube-like bodies were observed in the vacuoles of the organism of RH strain. In KM strain, two organisms of the same size were demonstrated in the leucocytes. It was presumed that they were products of longitudinal division.
