To 48 adult patients general and spinal anethesia was administered for elective orthopedic (intra-thoracic and extremity) and abdominal(general and gynecological) operations. Radial artery was cannulated and postoperative change of PaO2 was observed for 7days. The results are as follows: 1) General anesthesia administered for operations on the legs results in a reduction of PaO₂ which is maximal immediately after discontinuation of anesthesia, gradually returns toward normal in a 3-hour period, and becomes normal on the first postoperative day. 2) General anesthesia administered for thoracomy and laparotomy is followed by the same early changes, which do not return toward normal in the first 3 hours. Reduciton of PaO₂ persists, and PaO₂ continue to deteriorate for several days, not completely returning to normal even 7days post-operatively. 3) When spinal anesthesia is administered for laparotomy, PaO₂ does not begin to fall untill several hours after the end of operation. The subsequent course follows as that in 2, above. 4) When spinal anesthesia is administered for operation on the legs, PaO₂ does not change significantly throughout the postoperative period. 5) Thus, it is concluded that late forms of postoperative hypoxemia is influenced primarily not by the method of anesthesia, but by site of operation.

Recently there has been remarkable improvement in the treatment of chondrosarcoma, accompanied with the development of diagnostic tools, operative technique, replacement materials and designs, anticancer-chemotherapy and radiotherapy. In well-deferentiated chondrosarcom, wide excision is the only treatment required. Nevertheless, in dedifferentiated chondrosarcoma, anticancerchemotherapy or radiotherapy is usually recommended after the wide excision. From 1984 to 1994, 22 patients diagnosed as chondrosarcoma had been treated at Seoul National University Hospital. We analysed anatomic distributions, size, methods of treatment and their results. Simple bone cyst was first described by Virchow over a century ago and has been the subject of numerous articles. Nevertheless considerable confusion still exists regarding its etiology. clinical behavior and optimal management. Eighteen cases of pathologically proven simple bone cyst were treated in the Department of Orthopedic Surgery, Seoul National University Hospital during the thirteen year period from June 1966 to May 1979, and following results were obtained. 1. Age distributions were between five and forty-three years (Average ; seventeen years), and 72.2% (thirteen cases) were during first two decades of life. 2. Male to female ratio was 2.6 to 1 3. The most frequently involved location was proximal humerus followed by proximal femur. 4. Pathologic fracture, pain, incidental roentgenography and swelling were the first clues of diagnosis in our series. 5. In twelve cases curettage and fresh autogenous iliac bone graft was performed and good results were obtained in all of them. Of five cases only curettage had been done, no case recurred, another case showed residual cystic area, and the rest healed without further event. In one case with huge cyst curettage, iliac and fibular bone graft and bone cementing was performed with satisfactory result. 6. Bone shortening was noticed in two cases of active bone cysts in proximal humerus 2cm and 3cm, respectively.

Although osteltis fibrosa cystica was the clinical manifestation originally recognized as a feature of the primary hyperparathyroidism, its frequency in diagnosed cases currently has decreased. Affected patients are now being detected in earlier stages of the disease. We have experienced a case of primary hyperparathyroldism with the typical bony changes and the pathologic, subtrochanteric fracture of left femur. We have treated the fracture with Zickel nalling and parathyroidectomy was performed. Four months after the removal of parathyoid adenoma the fractured femur healed well.

Although the incidence of tuberculosis of the bone and joints is being decreased with good nutrition, hygine and the development of preventive medicine, it is still a common disease in our clinic. One hundred and ninty-eight cases of tuberculosis of the bone and joints were studied in the Department of Orthopedic Surgery, Seoul National University Hospital for 10 years from January, 1966 to December. 1975. The results were as follows: 1. Total cases of tuberculosis of the bone and joints were 588 Among them, peripheral bone and joints excluding the spine occupied 198 cases (34%). 2. Sex ratio was 1.7 : 1 (male : female). Age distribution showed the group below 10 years was 78 cases (39%). On the other hand, 7 cases were over 51 years old. 3. More than 50% of patients visited hospital after one year from the onset. 4. One-third of all cases had been given some management before admission and the most common treatment was anti-tuberculous chemotherapy (25%). 5. Erythrocyte sedimentation rate was definitly increased in 41%. 6. Associated tuberculous lesionsoe of extraskeletal system were confirmed in 66 cases (33%). Among them, active pulmonary tuberculosis were 51 cases (77%) 7. Various treatments (conservative, curettage, synovectomy, arthrodesis, etc.) had been carried out. However, surgical managements for saving the range of motion must be considered always before arthrodesis.

More than 100 surgical methods were described as the treatment of recurrent patella dislocation. These can be divided into two categories, the proximal, dynamic reconstruction and the distal, static reconstruction. We treated a case of recurrent patella dislocation with a new method, using gracilis. The tendon of gracilis was dissected and cut at the point about 7cm proximal to the insertion. A 'H' shaped slot was made subperiosteally in the anterior surface of patella. The both ends of cut gracilis tendon were imbedded into the 'H' shaped slot of patella and sutured. So the gracilis tendon could act as a static and dynamic stabilizer. The result was excellent at 1 year follow up.

Although oxcarbazepine (OXC)-induced hyponatremia is usually asymptomatic, it can lead to serious complications. We reports two cases of symptomatic hyponatremia induced by OXC. First case was 39-year-old female with partial epilepsy who developed the exceeding tiredness, headache, and seizures under OXC with a serum sodium level of 121 mEq/L. After the discontinuation of OXC, serum sodium was normalized and the clinical symptoms disappeared. Second case was 48-year-old female with partial epilepsy who complained of the fluctuating tiredness and fatigability, and then eventually developed seizures after OXC was increased in dosage. At that time, serum sodium was 121 mEq/L. She remained asymptomatic with normal level of serum sodium even if OXC was maintained with topiramate. Our cases may suggest that symptomatic hyponatremia is not rare unlike the results from previous studies. Therefore, clinician should be alert to mild symptoms such as headache, lethargy, and tiredness in patients treated with OXC.

By light and scanning electron microscopy, the morphologies of Korean pubic louse(Phthirus pubis) were described and measured. Especially, electron microscopic structures of palpus, tuft organ, sensilla basiconicum and palpal lobe were reviewed in detail. The variations were obserbed in the number of dorsal setae and the setae of membranous processes of abdomen. And also, the importance of public health and epidemiological aspects was discussed.
