2.A Case of Retinal Racemose Hemangioma.

PURPOSE: Racemose hemangiomas of the retina are rare developmental vascular anomalies with abnormal arteriovenous anastomoses. We report our observations of a retinal racemose hemangioma with marked dilation and tortuosity of the retinal vessels in the entire left retina. METHODS: In a 9-year-old boy with loss of vision and mild esotropia who presented with hugely dilated and tortuous retinal vessels in left retina, we examined fundus, fluorescein angiography, cerebral angiography, brain CT and MRI. RESULTS: Fundus and fluorescein angiography of the left eye showed marked dilation and tortuosity of the retinal vessels. Angiographic finding showed rapid filling of all vessels and no dye leakage. MRI finding revealed small tortuous vessels around left optic nerve, optic chiasm and tract. Brain CT and cerebral angiography showed nothing abnormal.

3.Conventional and Transesophageal Echocardiographic Demonstration of a Ventricular Septal Perforation with Bilateral Shunt and Chordae Rupture of Tricuspid Septal Leaflet Caused by Nonpenetrating Chest Trauma: A Case Report.

Blunt thorax trauma may produce a variety of cardiac lesions, which may occur alone or in combination. Nonpenetrating perforation of interventricular septum with chordae rupture of tricuspid valve is a infrequent pathologic event. A 40-years-old worker was transfered to hospital with increasing symptoms of right heart failure following a blunt compressing chest trauma with a huge overolling cement pipe(Wt.680kg) 6 weeks ago. The immediately diagnosed serial rib fracture of the left thorax cage and dislocation of the left acromoclavicular joint were treat conservatively. A conventional transthoracic color Doppler and two dimensional echocardiogram detected traumatic ventricular septal defect with bilatral shunt and tricuspid regurgitation. An additional transesophageal color echocardiopraphic demonstrated the ruptured chordae tendineae of the tricuspid septal leafet, which prolapsed deeply into the right atrium. The conventional color Doppler echocardiopraphy enhances the ability to detect the presence of a ventricular septal perforation and valve dysfunction in a patient with cardiac contusion. The transesophageal echocardiopraphy is a useful semiinvasive tool for the detailed morphological evaluation of atrioventricular valves and their substructure.

7.Wegener' s Granulomatosis.

Wegener's granulomatosis is a systemic necrotizing vasculitis of unknown cause. The disease is characterized by the involvement of the upper airway, the lung, and the kidney. Skin lesions are frequent and the most common lesion is purpura distributed on the limbs and trunk. A 34-year-old female showed recurrent purpuric macules on the both lower extremities and buttocks. The patient. showed nasal septal perforation with saddle nose deformity and C-ANCA positivity. Histopathologic findings of purpuric lesion revealed the features of necrotizing vasculitis. The histologic specimen from the nasal cavity showed chronic inflammation with granuloma formation and kidney showed focal necrotizing glomerulonephritis. Therefore, we treated the patient with prednisolone and cyclophosphamide having diagnosed Wegener's granulomatosis.

8.Tuberculosis in Sex Accessory Organs.

PURPOSE: The incidence of genitourinary tuberculosis(tbc) has consistently declined as results of effective chemotherapy and health care. But it still has significant associated morbidity. The pathogenesis of prostatic and epididymal tbc remains obscure. To assess the progress of tuberculous infection in sex accessory organs including prostate, epididymis and testis, prospective clinical study was performed. MATERIALS AND METHODS: Of the 58 male patients with genitourinary tbc during a 8-year period(1989-1996), 23 patients (39.7%) had tuberculosis in sex accessory organs. Thirty patients who demonstrated tuberculosis in at least one organ amomg prostate, epididymis, testis and bladder were evaluated. The conclusive diagnosis was made on the basis of isolation of Mycobacterium tuberculosis, histopathology. A patients with organ confined tbc was considered to be caused by metastatic spread of infection. RESULTS: In 12(40%) of 30 patients tubercle bacilli were detected in urine. 14(87.5%) of the 16 patients with tuberculous epididymitis and 10(76.9%) of the 13 patients with tuberculous prostatitis were considered to have an infection extended through the metastatic spread. Three(25%) patients had tuberculous cystitis directly extended from the tuberculous prostatitis. All three tuberculous orchitis were secondary to the tuberculous epididymitis. CONCLUSIONS: The results suggest that the route of infection of tuberculosis in the prostate and the epididymis are mainly through the hematogenous spread and direct extension of infection is likely deveploped by descending spread.

9.Quantitative Analysis of the Seminiferous Epithelium in Human Testis.

Number of germ cells in the seminiferous epithelium was analyzed quantitatively in testicular biopsy specimens of 23 patients without ductal obstruction and of 4 patients with ductal obstruction. Roth number of mature spermatids within each cross-section of seminiferous tubule and number of atrophic tubule were counted in biopsy specimens. Results were expressed as cell number of mature spermatids per seminiferous tubule and percentage of atrophic tubules. A significant correlation was demonstrated between sperm density and mature spermatid counts. Patients with sperm counts of less than 40 x l0(6)/ml had mature spermatids counts of less than 25 per seminiferous tubule. Coefficients of correlation between mature spermatid count and percentage of atrophic tubules were higher than those of correlation between sperm counts and percentage of atrophic tubules. In asoospermrc patients with epididymal obstruction, sperm count after corrective surgery could be predicted correctly by this quantitative analysis technique of testicular biopsy specimens and partial obstruction of anastomotic site of seminal tract could be proved in oligozoospermic patients after corrective surgery.

10.A Case of Primary Carcinoid Tumor of the Rete Testis.

Testicular carcinoid tumor is uncommon and most of these tumors occur in fourth decade' of life. A 33-year-old man presented with intrascrotal painless mass, which was initially misdiagnosed as epididymal tumor. Epididymectomy including the mass was performed and pathologic examination revealed a carcinoid tumor confined to the rete testis. There was no evidence of carcinoid syndrome. The 24 hour urinary 5-HIAA level was normal. Abdominal CT scan and GI contrast study showed no evidence of carcinoid in other organ including the gut. Radical orchiectomy and hemiscrotectomy were undergone as a curative therapy. He has been well for 40 months without recurrence.
