2.A Case of Retinal Racemose Hemangioma.

PURPOSE: Racemose hemangiomas of the retina are rare developmental vascular anomalies with abnormal arteriovenous anastomoses. We report our observations of a retinal racemose hemangioma with marked dilation and tortuosity of the retinal vessels in the entire left retina. METHODS: In a 9-year-old boy with loss of vision and mild esotropia who presented with hugely dilated and tortuous retinal vessels in left retina, we examined fundus, fluorescein angiography, cerebral angiography, brain CT and MRI. RESULTS: Fundus and fluorescein angiography of the left eye showed marked dilation and tortuosity of the retinal vessels. Angiographic finding showed rapid filling of all vessels and no dye leakage. MRI finding revealed small tortuous vessels around left optic nerve, optic chiasm and tract. Brain CT and cerebral angiography showed nothing abnormal.

3.Conventional and Transesophageal Echocardiographic Demonstration of a Ventricular Septal Perforation with Bilateral Shunt and Chordae Rupture of Tricuspid Septal Leaflet Caused by Nonpenetrating Chest Trauma: A Case Report.

Blunt thorax trauma may produce a variety of cardiac lesions, which may occur alone or in combination. Nonpenetrating perforation of interventricular septum with chordae rupture of tricuspid valve is a infrequent pathologic event. A 40-years-old worker was transfered to hospital with increasing symptoms of right heart failure following a blunt compressing chest trauma with a huge overolling cement pipe(Wt.680kg) 6 weeks ago. The immediately diagnosed serial rib fracture of the left thorax cage and dislocation of the left acromoclavicular joint were treat conservatively. A conventional transthoracic color Doppler and two dimensional echocardiogram detected traumatic ventricular septal defect with bilatral shunt and tricuspid regurgitation. An additional transesophageal color echocardiopraphic demonstrated the ruptured chordae tendineae of the tricuspid septal leafet, which prolapsed deeply into the right atrium. The conventional color Doppler echocardiopraphy enhances the ability to detect the presence of a ventricular septal perforation and valve dysfunction in a patient with cardiac contusion. The transesophageal echocardiopraphy is a useful semiinvasive tool for the detailed morphological evaluation of atrioventricular valves and their substructure.

7.Testicular Biopsy: Comparison of Open Surgical and Percutaneous Needle Biopsy Techniques.

Testicular biopsy has been known as very useful diagnostic procedure for the male infertility. For the past 9 months percutaneous testicular biopsy procedures were attempted on 64 patients with azoospermia or atrophic testis using a new Tru-Cut biopsy needle. In addition surgical testicular open biopsy procedure on one side and percutaneous testicular needle biopsy procedure on the other side were performed in comparison with two techniques. Biopsy specimens were fixed in Bouin solution. They were stained and histologically examined. Results were summarized as follows: Numbers of seminiferous tubules obtained by needle biopsy technique were found to be fewer than that of the tubules obtained by open biopsy technique. But diagnostic accuracy was not compromised by the needle biopsy technique. There was no bleeding either from the testis or scrotum, no extusion of testicular tissue from the puncuate site. In conclusion percutaneous testicular biopsy using Tru-Cut biopsy needle has been proved to be simple, time saving, less expensive and accurate method for testicular biopsy.

8.Sperm Granuloma and Success Rate of Vasovasostomy.

A discrepancy between patency and pregnancy rates persists even if a surgical success rate of vasovasostomy, improved. A sperm granuloma has been considered that it has relation to improvement of quality of sperm by decompression of intravasal pressure. During the period from 1981 to 1983, a total of 160 vasovasostomies were performed. An age of the patient ranged from 24 to 65 with the mean of 36.5. Duration of obstruction ranged from 5 months to 20 years with the mean of 4.2 years. Viable sperms more than 10 x 1,000,000/ml appeared in 145 out of the 160 patients (90.6%) , and pregnancy occurred in 98 out of the 160 patients (61.2%) following the vasovasostomy The sperm granuloma was found in 50% of 160 vasectomized patients. Forty-eight percent of patients whose patency were restored had sperm granuloma. Fifty percent of patients who impregnated their wives had sperm granuloma. There was statistically no significant difference in both patency and pregnancy rates between patients with and without sperm granuloma. But sperm months at 6 months post operatively was found to be higher in patients with sperm granuloma than without sperm granuloma (p<0.05).

9.Clinicopathologic prognostic factors in renal cell carcinoma.

From 1980 to 1989, 144 patients underwent radical nephrectomy for renal cell carcinoma. Survival was analyzed in terms of pathologic stage and histologic grade. Data revealed that M category was the most important prognostic factor followed by, in order of importance, T category, N category, and grade. But grade as prognostic factor in renal cell carcinoma needs additional investigation. Stage pT1 and pT2 showed similar survival rate. The separation of pT1 and pT2 according to size of the primary tumor may not be a meaningful prognostic variable. Because vascular invasion may occur with tumors of various size, separating components into a description of the tumor (T), nodes (N), metastases (M) and renal vein involvements (V) makes good sense. The prognostic value of lymphadenectomy is limited by the fact that the tumor metastasizes through the blood stream and the lymphatic system with equal frequency.

10.Wegener' s Granulomatosis.

Wegener's granulomatosis is a systemic necrotizing vasculitis of unknown cause. The disease is characterized by the involvement of the upper airway, the lung, and the kidney. Skin lesions are frequent and the most common lesion is purpura distributed on the limbs and trunk. A 34-year-old female showed recurrent purpuric macules on the both lower extremities and buttocks. The patient. showed nasal septal perforation with saddle nose deformity and C-ANCA positivity. Histopathologic findings of purpuric lesion revealed the features of necrotizing vasculitis. The histologic specimen from the nasal cavity showed chronic inflammation with granuloma formation and kidney showed focal necrotizing glomerulonephritis. Therefore, we treated the patient with prednisolone and cyclophosphamide having diagnosed Wegener's granulomatosis.
