The authors investigated the incidence of stratum corneum and upper epidermal cytoplasmic antibodies with 30 untreated and 20 treated psoriasis sera, and normal human sera using normal human skins of 5 different sites and psoriatic lesion by the method of indirect immunofluorescence in order to evaluate immunologic responses i n psoreasis. The results are summarized as follows . I) The positivity of stratum corneum antibodies in untreated psoriasis sera(78.7%) was si, nificantly higher than that in normal sera(64.0%). The incidence of stratum corneum antibodies in untreated psoriasis sera was found to be the highest in the arm, followed by the scalp, leg, abdomen, and face as substrate. 2) The positivity of stratum corneum antibodies in psoriasis and normal human sera was significantly higher when tested with the psoriatic lesion as substrate than norma! skin as substrate, 3) The titer of stratum corneum antibodies in 5 sera using human skin obtained from 5 different sites on the body as substrate are the highest in the arm, and leg, and(in decreasing order of frequency) the scalp, abdomen, and face. 4) The positivity of upper epidermal cytoplhsmic antibodies in normal human sera (40.7%) was significa.ntly higher than that in untreated psoriasis sera(21.3%). 5) Ir.. the majority of cases, upper epidermal cytoplasmic antibodies coexisted with straturn corneum antibodies in the sera of patients with psoriasis and in the sera of normal humans.

In general, arthroplasty in the hand is divided into three categories, namely fusion, soft tissue arthroplasty and interpositional arthroplasy, each of that having some problems and limited indications. Frequently orthopedic surgeons fall into difficulties in deciding the treatment for the ankylosed joints of the hand in young adults. We now report a case of living joint transplantation from the M-P joint of 2nd toe for post-traumatic ankylosis of that of index finger. The follow-up shows improved range of motion and good pain relief.

PURPOSE: We performed a retrospective study to determine the comparative characteristics of vesicoureteral reflux (VUR) between the infants less than 1 year and children more than 1 year old and whether early treatment in infantile VUR may be of benefit to preserve the renal function and to prevent the new renal scar. MATERIALS AND METHODS: All of 62 cases (infant group 19 and children group 43) with primary VUR were managed as the ureteral reimplantation from January 1991 to December 1996. RESULTS: The average age of patients was 7.1 months in infant group and 6.0 years in children group. The sex ratio of male to female showed no difference between infant and children group (1.7:1 Vs 1.5:1). The average follow up periods were 20.5 and 23.7 months in infant and children groups, respectively. The high grade reflux and renal scarring in infant group were significantly more frequent than those in children group (P<0.05), but the prevalence of urinary tract infection in children group was significantly higher than that in infant group (P<0.05). There was no significant difference in the rates of bilaterality and in the sex ratio between infant and children group. The success rate of operation was 100 percent in both groups. CONCLUSION: We suggest that the early surgical treatment in infants with bilateral and higher grade VUR may be of benefit to the preservation of renal function and the decrease of renal parenchymal change.

Dermstomyositis is a disease mainly of the skin, muscle and blood vessels in which erythematous and edematous changes in the skin are associated with muscle weakneas and inflammation. In adult the disease is commonly associated with underlying malignancy or reticulosis. A 23 year-old male had generalized weakness, intermittent fever, myalgia and purplish red patches on the face. Diagnosis of dermatomyositis was established by clinical manifestations, electromyogram, histologic findings of skin and muscle. He was treated with systemic steroid, with some improvement. Seven months after the onset of dermatomyositis, ecchymoses was noted on the eyelids and forearms. Bone marrow aspiration showed many immature lymphoblasts, characteristic of acute lymphocytic leukemia, He was treated with systemic steroid and vincristine, but no favorable effect was noted. He died of sepsis due to retal abscess on the 34th day in the hospital

The study was undertaken to assess the CT density of the antibiotics solution. CT scan of six antibioticssolution-streptomycin, chloramphenicol, Na-penicillin, ampicillin, kanamycin and cefamezine-in concentration ofabout 33% (approximatly single dose of intramuscular injection) was performed, using plastic syringes. Variousconcentrations of striptomycin, chloramphenicol and Na-panicillin were also examined for evaluation of relationbetween concentration and the CT density of the antibiotics. In addition, relationship between CT number andmathematically calculated effective atomic number and electron density of the antibiotics was evaluated. Theresults are as follows; 1. The CT densities of all antibiotics reveal high density (CT number 80–150) inconcentration of single intramuscular injection dose. 2. CT number of striptomycin, chloramphenicol andNa-penicillin gradually increased with increase of concentration of the antibiotics, producing linear proportionto concentration, effective atomic number and electron density of the antibiotics. 3. Therefore, density ofantibiotics should be included in differential diagnosis when high density on CT scan is observed.

Cavernous lymphangioma in mediastinum is a rare congenital malformation of lymphatic system and is caused by lack of adequate drainage from the sequestrated lymphatic vessels to the venous system due to insufficiency or atresia of the efferent lymphatic channels The authors experienced 10 month-old male patient with cavernous lymphangioma in mediastinum documented by chest CT and open biopsy The review of the literature was made briefly.

Color Doppler ultrasound imaging( DCI), non-invasive technique, provides the informations of vascular distributions and hemodynamic status, which is helpful to make the differential diagnosis between renal cell carcinoma and benign lesions. We evaluated tumor vascularity, peak systolic velocity, end diastolic velocity, and resistive index(RI) using DCI in 11 cases of renal cell carcinoma and 2 cases of benign lesions. Hypervascularity within the tumor was seen in all patients with renal cell carcinoma. Peak systolic velocity was higher than 3kHz(average 5. 5kHz) in 9 cases. End diastolic velocity was higher than 1kHz(average 2.2kHz) in 9 cases. RI was lower than 0.7(average 0.59) in 9 cases. Arteriovenous shunt was seen in 7 cases. In 4 cases with intermediate mass on CT and USG,' DCI was helpful to make the diagnosis preoperatively. In 2 cases of renal cell carcinoma, DCI was beneficial to evaluate the presence and the extent of tumor thrombi. Whether the hemodynamic parameters could be a prognostic factor needs more cases to be defined.

Two kinds or intraprostatic stent (Prostakath and Nissenkorn catheter) were inserted under local anesthesia in 17 patients for 20 times who had benign prostatic hyperplasia. Within 1 month after insertion of the stents. the results are good in 10 of 17 patients. fair in 4 and poor in 3 from the view points of improvements in urine flow and voiding symptoms. Of 9 patients in whom the stents have been inserted longer than 6 months. 4 are good. 2 are fair and 3 are poor in the results of long term insertion. It seems that this method is applicable as s definitive method for benign prostatic hyperplasia if there is a few progressions in the location where this stent is inserted. in the quality or stent material and in the easy way to change the stent.
