BACKGROUND: Seroincidence of hepatitis B virus (HBV) among Korean blood donors has not been reported. This study was conducted to calculate the seroincidence of HBV among blood donors and to estimate the risk of post-transfusion hepatitis B by donated blood in window period of infection. METHODS: HBV seroincidence was calculated among repeat-donors who had donated from Nov. 1994 through Dec. 1996. To calculate the person-years, the database of the Korean National Red Cross was used in which results for HBsAg enzyme immunoassay were filed up. The observed incidence was adjusted by interdonation interval of incident cases, who were defined as donors showing seroconversion. The risk of post-transfusion hepatitis B by donated bloods in window period of infection was estimated. RESULTS: HBV seroincidence was estimated to be 180.85/100,000 person-years. This was adjusted as 602.83/100,000 person-years by considering interdonation intervals. The risk of post-transfusion hepatitis B was estimated to be 974 units per one million of whole blood units due to be in window period of infection. CONCLUSION: The estimated seroincidence of HBV among Korean blood donors and thereby the risk of HBV transmission by donated bloods in window period of infection was about 50 to 60 times higher than those of Japan and United States of America.(Korean J Blood Transfusion 10(1): 1-4, 1999)

Myoclonic-astatic epilepsy is an epileptic syndrome characterized bv the presence of myoclonic and astatic seizures. It is a rare generalized epilepsy of childhood and represents 1 to 2% of epilepsis in children up to the age of 9 years. In addition to myoclonic and astatic seizures, generalized tonic-clonic seizure, tonic seizure and absence status (non-convulsive status) can be combined. Non-convulsive status can produce apathetic and stuporous appearance. We represent 4-year-old boy who showed nonconvulsive status of myoclonic-astatic epilepsy with typical clinical and eleckoencephalographic characteristics.

In order to search for anti-Herpes simplex virus (HSV) type-1 agents from Korean medicinal herbs and Korean traditional prescriptions (herb complexes), we selected 80 medicinal herbs and 45 prescriptions, based on a review of the Korean traditional medicine books. Both methanol extracts and boiling-water extracts were tested by means of the MTT assay (tetrazolium based colorimetric assay). Ten of the 125 methanol extracts: CM-11, CM-18, CM-19, CM-21, CM-22, CM-39, MM-3, MM-18, MM-29, MM-73 (see explanation of nomenclature below), showed efficacy against HSV-1. Twelve of the water extracts: CW-2, CW-3-I, CW-3-II, CW-18, MW-3, MW-5 MW-6, MW-12, MW-47, MW-69, MW-73 and MW-75 were active. #3 (individual herb) and #73 (individual herb) were interesting because both water and methanol extracts were active. Especially, #3 is a part of composition of Hong-il-laksamd ngbang and Hojanghaedokt'ang which have anti-HSV-1 activitives. The SI value of MW-69 and CW-18 was relative high as 10.2+/-0.7 and 11.8+/-2.2. The cytotoxic effect on Vero cells of Panch' n-ch' ngbang, Taraxacum platycarpum H. Dahlst. and acycloguanosine was determined by MTT assay. Water extracts of Panch' ngch' ngbang (prescription) and Taraxacum platycarpum H. Dahlst. showed very weak cytotoxic effects on Vero cells at > 100 mug/ml but acycloguanosine showed strong cytotoxic effects on Vero cells at > 100 mu g/ml. As a result, #3, #73, MW-69 and CW-18 are considered as potentially useful for anti-HSV-1 agent and will be the focus of further research. Molluscum Contagiosum is a self-limited or contagious caused by a virus of the Pox virus group. It has ocular importance to examine the eyelids for Molliscum in case of conjunctivitis unresponsive to routine therapy. We had an experience of a case of Molluscum Contagiosum on the left lower eyelid. On the corresponding site of this Molluscum at the bulbar conjunctiva, the limbus, and the cornea, no follicles, epithelial keratitis nor erosions were seen. This patient was managed by simple excision of the lesion under local infiltration anesthesia. Now, we report our experience with this patient along with the review of other papers in this report.

Lumbar hernia presents as a reducible protruding mass in the flank region between the 12th rib and the iliac crest. The superior lumbar hernia (Grynfeltt's) is covered by the thin latissimus dorsi muscle and bounded by the erector spinae and internal obliquus abdominis muscle. The lumbodorsal fascia forms the floor of the Grynfeltt's triangle. Spontaneous or acquired non-traumatic lumbar hernia occurs more frequently after middle age. The untreated lumbar hernia progresses in size gradually. As in most hernias, the larger the defect, the more complicated and difficult is the repair. Here we report 2 female patients with spontaneous lumbar hernia diagnosed at Presbyterian Medical Center with computerized tomogram. Their treatment consisted of tension-free surgical repair utilizing the overlap method for defect edge and the application of prolene mesh.

This is the clinical studies of 171cases of acute poisoning in pediatric age which were visited at the emergency room of Han-gang Sacred Heart Hospital during the past 2 years and 5 months (Apr., 1974~Aug., 1976). The results were summerized as followings; 1. The incidence of acute poisoning among the emergency pediatric patients were 2.3%(171/7,310). 2. The sex ratio of male to female was 1.5:1. 3. The peak age group was 2 to 5 years of age (41.5%). 4. The peak incidence in a year was shown in August (17.5%) with prominence in summer season (47.3%). 5. The accidental poisoning was occured the most frequently from 4: 00 P.M. to 8:00P.M. during the day. 6. The motive of poisoning was mainly accidental causes (85.9%). 7. The most frequent causative substances were insecticides (29.8%) and then rodenticides (14%), D.D.S. (8.8%), reserpine (4.1%) were the next causes. 8. RH-787 poisoning were occured in 4 cases with the complication of diabetes mellitus 2 cases. 9. In poisonous plants, there were azalea (Rhododendron) poisoning (4 cases) and Gingko biloba Linne fruit poisoning (1 case).

Angiomyofibroblastoma is a distinctive, hitherto uncharacterized, benign soft tissue tumor of the vulva with histology similar to an aggressive pelvic angiomyxoma. It can be distinguished from an aggressive angiomyxoma by its circumscribed borders, higher cellularity, more numerous blood vessels, the frequent presence of plump stromal cells, minimal stromal mucin, and rarity of erythrocyte extravasation. We experienced a case of angiomyofibroblastoma of the vulva occurring in a 45-year-old woman. The lesion was a well-defined but not encapsulated mass, 4.5x4.2 cm. Histologically the mass was characterized by alternating hypercellular and hypocellular edematous zones in which abundant blood vessels were irregularly distributed. Immunohistochemically, the spindled, plump spindled, and oval stromal cells were reactive for vimentin and desmin, but not for cytokeratin, or S-100 protein.
