3.A Case of Early Congenital Syphilis.

We report one case of 1 month old male with early congenital syphilis Who represented the characteristic clinical features and laboratory findings including darkfield examindtion and quantitative VDRL test. The histopathologic findings of the patients skin biopsy specimen showed extravasation of erythrocytes and perivascular infiltration of plasma cells were found. in addition to the previous described findings by Fraser. He was managed by daily injection of 200,000 units of aqueous procaine penicillin for 8 days and then three injections of 150,000 units of PA.M at two days intervals with excellent effect

4.Two Different Penicillin Preparations in the Treatment of Gonorrhhea.

Penicillin is still the first choice of drug in the treatment of gonorrhea. The ever lowering sensitivity of Neisseria gonorrhoeae to penicillin and the emergence of PPNG (Penicillinase Producing Neisseria Gonorrhoeae) in the Far East and other parts of the world neceessitate reapprisal of treatrnent schedules with penicillin for gonococcal infections. In Korea, no authentical study in this regard has been done recently. The presence of intracellular gram r.egative diplococci in urethral smear as weIl as the presence of oxidase positive typical, round, convex, smooth, glistening, tratmslucent, greyish colonies of 0.5 to 2mm in diameter a 24 hours composed of grarn negative diplococci are used for presump'ive evidence of gonococccal infection 224 male patients with acnte uncomplicated gonococal infections were treated one of following regimens. Regimen A: 4.8mega unit combicillin (3.6mega unit procaine penicillin+l.2 mega unit sodium penicillin) i.m.. preceded by 1gm probenecid p.o., Regirnen B: 4.8 mega unit procaine penicillin i.m. preceded by 1gm probenecid P.O. Regimen A showed 14 failures(11.3% arnong 124. Regimen B showed 19 failures (19%) among 100 It is suggested that in Korea sesnsitivity to penicillin of Yeisseria gonorrhoeae is quite low.

5.Analysis of charges per case by hospital characteristics: In regard to acute appendicitis and NSVD.

To identify the factors influencing the charges per case of acute appendicitis and normal spontaneous vaginal delivery (NSVD), the personal data-base files and hospital-characteristics-reporting data files of Korea Medical Insurance Corporation were analyzed. One hundred and twenty-nine institutions were selected. The results of this study were as follows: 1. The differences of charges per case with respect to hospital ownership, location, and equipment levels were statistically significant. 2. The results of multiple regression analysis revealed that bed capacity was the most significant variable in both diseases. 3. Ownership was significant variable in acute appendicitis. In NSVD, ownership and hospital equipment level were statistically significant. In conclusion, bed capacity was statistically the most significant variable in the analysis of charges per case. And we thought that the results of this study would influence the policy of the hospital bed supply.

6.A Case Report of Amelanotic melanoma.

A case of amelanotic melanoma developed on a 19 years old boy was reported. Since the identification of amelanotic melanoma, there were few reports about the tumor but none in Korea. In this case, the histologic findings showed ulceration of the epidermis and irregular junctional activity, and in the dermis, there showed alveolar formation and nevus cell nest like cell masses. And in the deep dermis, the loss of dermal stroma with numerous bizzare giant cells and variable sized, hyperchromatic, plemorphic, mitotic figures were also seen, Strikingly, there were no melanin pigments in H-E stain as well as in Masson-Fontana stain, which was thought to be a amelanotic melanoma. The patient was treated with B.C.G caccination by intralesional and hypodermic injection, on the lesion and on both, the lesion was markedly improved with only scarformation on previously ulcerated lesion. The patient shall be followed up continuously.

8.A Study of PTH in Urolithiasis.

We reviewed 63 urolithiasis patients who were admitted to the Department of Urology, College of Medicine, Yonsei University during the period of 6 months from December, 1983 to May, 1984 to discover hyperparathyroidism in urolithiasis and to evaluate the relationship between PTH and urolithiasis. The following results were obtained. 1. Among 63 urolithiasis patients, there were 47 men and 16 women with a ratio of 2.9: 1 And peak age incidence of urolithiasis occurred in the third and fourth decades (68.3 %). 2. Urinary calculi occurred most commonly in the upper ureter (33.3%) and in decreasing order of frequency kidney and ureter (17.5 %), kidney (16.8 %). 3. In urolithiasis patients, elevated PTH level was found in 5 cases (7.9%) and hypercalciuria was found in 10 cases (15.9 %).And elevated PTH level with hypercalciuria was found in 2 cases(3.2%). 4. In urolithiasis patients, the range of serum calcium, phosphate, protein, albumin and urinaryphosphate, creatinine were within normal limit. And the range of TRP, TRCa were within normal limit, too. 5. Urolithiasis patients with elevated PTH had serum calcium level higher than urolithiasis patients with normal PTH and the difference of the values was statistically significant (P<0.01). But in serum calcium, phosphate, urinary phosphate, TRP and TRCa, the differences of the values between the elevated PTH group and normal PTH group were not statistically significant(p>0.01).

10.Arm Wrestler's Injury (Report of fourteen Cases)

According to literatures, the different types of fractures can occur as a result of arm wrestling ; Spiral fracture of the humeral shaft associating often with a bntterfly fragment, fracture of medial epicondyle of the humerus, and the marginal radial head fracture with anterior dislocation. The last type is very rarely reported. In addition to the fracture occurrence, other injuries such as muscular strain of the arm, and sprain or luxation of the shoulder, elbow and wrist joints can be taken place. However, it is thought that the most of those injuries by arm wrestling have been treated simply by general practioners unreported. The arouse the attention of the jury of the game, wrestlers and the surgenons, the cases of bony injuries sustained during the arm-wrestling are reported. Thirteen patients were treated by us since 1977. Among them, five cases had fractures of the humeral shaft, and eight cases had fractures of medial epicondyle of the humerus. Among the five shaft fractures, three were treated by conservatively, and two by operatively. Among 8 avulsion fractures of the medial epicondyle only two were treated conservatively, and 6 operatively. One patient sustained the medial epicondylar avulsion fracture twice for which K-wire fixation treatment was done. The results of treatment were all satisfactory. However, because of the possibility of wrestlers sustaining such injuries. Arm wrestling should not be considered a totally benign sport.
