1.Experiences of Abdominoplasty without Undermining.

No one technique provides an optimal outcome for all body contouring patients. There are many surgical options for abdominoplasty. Among these, this abdominoplasty without undermining consists of liposuction around abdominal subcutaneous fatty tissue, excision of lower abdominal flap. The procedure allows aggressive thinning and sculpting of abdominal flap. This operation minimizes the dead space, which often leads to postoperative complications, and preserves neurovascular supply to the abdominal skin. From 1999 to 2004, 18 patients underwent the abdominoplasty without undermining, resulting in high satisfaction rates with no significant complications, such as, pulmonary embolism and deep vein thrombosis. Patients could return to normal activity within a week. This abdominoplasty without undermining is an effective and safe alternative with low complication rate and enhances aesthetic results compared to traditional abdominal surgery.

2.Histopathologic Features of Gastric Mucosa Colonized by Helicobacter pylori.

TO evaluate the histopathologic features of gastric mucosa by Helicobacter pylori (HP), we reviewed 70 endoscopically biopsied chronic gastritis and peptic ulcer. The results are as fonows. l. HP was detected in 923% (48/52) of active gastritis, 8l8% (9/l l) of peptic ulcer and l43% (1/7) of chronic gastritis. The prevalence of HP infection was significantly higher in active gastritis and peptic ulcer than chronic gastritis (P<0005). 2. The infiltration of intraepithelial neutrophil of the 58 HP-positive cases was significantly more than of the l2 HP-negative cases (P<0005). 3. The infiltration of neutrophil and chronic inflammatory cells in the lamina propria of the 58 HP positive cases were significantly more than of 12 HP-negative cases (P<0.005, P<0.01) 4. The gastric epithelium of 58 HP positive case showed characteristic degenerative change, such as epithelial pits (93.l%), irregular surface (84.4%), individua1 cell drop-out (46.5%) and microerosion (27.6%). The similiar changes were not seen in l2 HP-negative cases. In summary, HP is significantly correlated with inflammatory reaction of the gastric mucosa. It is also significantly correlated with the epithelial degenerative changes that is considered to the precursor of peptic ulcer.

4.Survey for Hygiene Behavior on Healthcare Personnel by Hygiene Inventory 23.

BACKGROUND: The purpose of this study was to survey hygiene behavior of healthcare personnel according to the Hygiene Inventory 23 (HI23) and to use the results in education and research for promoting hygiene behaviors. METHODS: We sampled a total of 400 people with 50 from each job category. The sample was obtained through convenient sampling among 8,200 employees working at a tertiary-care hospital with 2,600 beds in Seoul. The HI23 consisted of 5 subscales and 8 questions on general hygiene, 3 on household hygiene, 3 on food-related hygiene, 5 on hand hygiene methods, and 4 on personal hygiene, resulting in a total of 23 questions. The researcher also added 14 questions on the general characteristics of the subjects. The self-administered questionnaires were distributed on October 2009 and analyzed blindly. RESULTS: The average total score for hygiene behavior was 3.04+/-0.44 (maximum of 4). In the subcategory of hygiene behavior, the mean score was 3.20+/-4.24 for general hygiene, 3.07+/-0.68 for household hygiene, 3.07+/-0.68 for food-related hygiene, 2.91+/-0.69 for hand hygiene methods, and 2.97+/-0.48 for personal hygiene. When factors affecting the level of hygiene behavior were identified through multivariate regression analysis, independent factors were found to be gender (female), job category (food-related job), experience in hand hygiene education, and experience in teaching hand hygiene or general hygiene. CONCLUSION: Programs and education adapted to the characteristics of each job category in hospitals are needed to promote hygiene behaviors such as general hygiene, hand hygiene methods, household hygiene, food-related hygiene, and personal hygiene.

6.Tibial Plateau Fracture An Analysis of the Results of Treatment in 37 Patients

Tibial plateau fractures are a common injury which often produce major disability. The extent of actural damage is always greater than that suggested by X-ray, a situation which, if not appreciated, can easily lead to a complacent surgical attitude. Although recent studies suggest that early knee motion and perhaps better surgical technique have improved clinical end-results, still recent series show unaccepatble results in 20-40 percent of cases. The authors analyzed the 37 cases of the tibial plateau fractures treated at the Department of Orthopaedics, Catholic University Medical College, Kang-Nam St. Mary's Hospital and following results were obtained. 1. The highest incidence was in the 5th decade and the next was 4th, and most frequent cases of trauma was pedestrian car accident(83%). 2. The most common fracture type was type VI by Schatzker classification and next was type II. 3. Among 37 cases, 27 were treated by open reduction and internal fixation and 14 out of those 27 cases meeded bone graft for the elevation of the tibial articular surface. 4. According to Blokker's criteria(1984), 32(86.6%) out of 37 cases had satisfactory result. Among 5 cases of unsatisfactory results 3 cases developed varus tilting of the tibial plateau more than 10 during the early stage (6–12 weeks post operation) of follow-up and those 3 were Schatzker's type VI fracture. 5. It is thought that open anatomical reduction of articular surface bone graft, internal fixation and early joint motion are the best way for the treatment of depressed or displaced fracture. 6. It is found tha Schatzker's type VI fracture has a tendency to develop varus tilting through the proximal tibial fracture before it unites completely.

7.Breast Augmentation using Expandable Implants.

Ideal results of augmentation mammaplasty consist of symmetry, natural shape, soft feeling and inconspicuous scar. In addition, patient's preferences about size and shape should be included. Static implants could not perfectly satisfy patients' desires for size and shape, but expandable implants enable to change the volume after the operation. From September 2001 to September 2004, 76 patients(150 breasts) underwent breast augmentation using permanent expandable implant. The procedure was unilateral in 2 women and bilateral in 74 women. Age ranged from 19 to 50 years(mean, 29 years). Fifty nine patients underwent simple augmentation mammaplasty, 7 patients were corrected of their severe asymmetry, 2 patients with the congenital breast deformity underwent mammaplasty using this, and 2 patients who had undergone unilateral mastectomy were reconstructed of their breasts using expandable implant. There were no definite complications such as capsular contracture, implant rupture, asymmetry. And there reported little dissatisfaction about the size. The permanent expandable implants might be good alternatives in cases of ordinary breast augmentation as well as tissue deficient patients, asymmetry, congenital anomaly, and breast reconstruction.

PURPOSE: Free nipple graft reduction mammaplasty is a simple and effective way to reduce huge breasts. However, this technique is frequently criticized for producing poor projection and hypopigmentation of the nipple areola complex(NAC). METHODS: Sixty three patients(126 breasts) underwent the procedure from 1998 to 2005. Authors' method is similar with the modified Gradinger's technique except the keyhole pattern. After skin flap closing, the position of NAC is determined considering symmetry. The NAC is initially harvested and then resection of the breast followed, leaving a deepithelized inferior parenchymal pedicle(5 x 5cm). The upper point of inferior pedicle is sutured to the fascia of the pectoralis to produce the upper bulge. The nipple is replaced as a free and composite graft. RESULTS: An average of 823grams of breast tissue per breast was removed. There was no major complications. All grafted nipples showed long lasting projection. And also, all NAC eventually regained their normal color except for 3 patients who needed medical tattoos. The overall results were good and patient satisfactory score was high. CONCLUSION: This useful technique greatly enhances long lasting projection and recovers nipple color.

10.Perception of Physicians and Nurses on Presence of Family during Invasive Procedures.

PURPOSE: The purpose of this descriptive study was to determine the perception of physicians and nurses on family presence during invasive procedures. METHODS: The study population consisted of 100 physicians and 100 nurses from five hospitals which have more than 500 beds in B city. Data collection was done from August 20 to September 19, 2007. The Family Presence During Resuscitation (FPDR) Inventory developed by Fulbrook, Albarran and Latour (2005) was used as the study instrument. The collected data was analyzed by t-test, ANOVA and Duncan's multiple range test using SPSS/WIN 14.0 version. RESULTS: Nurses had more positive attitudes to allowing family members to be present during invasive procedures than physicians did. However, compared to physicians, nurses had more concern about problems of confidentiality, arguing with medical team, and increased rate of legal action if family members were present during invasive procedure. CONCLUSION: The results of this study indicate that educational programs and policy for family presence be required within the hospital to enhance the perception of physicians and nurses to the family presence.
