2.Treatment of Giant Cell Tumor of Bone

22 cases of giant cell tumor of bone treated between 1973 and 1979 have been reviewed after a miaimum follow-up of two years and following results were obtaind. 1. Fourteen cases (63%) were in the third and fourth decade. 2. Fourteen cases (63%) were located around the knee joint. 3. Five (42%) of the twelve tumors treated by curettage and bone graft recurred. Five tumors treated by En bloc resection and two tumors treated by amputation had no recurrences. 4. Five of six recurrences occured within two years after mitial treatment.

3.A Case of Congenital Hyperextension of the Knee.

Congenital dislocation of the knee is a very rare condition and was first described by Chatelaine in 1822. The etiology of this condition is unknown. It is generally subclassified as simple hyperextension, subluxation, and dislocation, depending on the degree of the joint displacement and the severity of disease. There are a large of associated conditions that have been described, the most common of which are congenital dislocation of the hip, club foot, arthrogryposis, and Larsen's syndrome. The mainstay of treatment is early serial rnanipulation and splinting. Operative treatment was indicated whenever conservative treatment did not lead to satisfactory reduction. The authors experienced a case of congenital hyperextension of left knee associated with the calcaneovalgus deformity of both foot. Early closed treatment obtanied a successful reduction and satisfactory knee motion.

5.Control Mechanisms of Ovarian Follicle Development by Follicle Stimulating Hormone and Pituitary Adenylate Cyclase-activating Polypeptide.

OBJECTIVE: Pituitary adenylate cyclase-activating polypeptide (PACAP), a novel hypothalamic neuropeptide, has been suggested to play a role in ovarian folliculogenesis. The present study evaluated the effect of PACAP on the growth of preantral follicles. METHODS: Preantral follicles were mechanically isolated from ovaries of 21-day-old rats and cultured in groups for 3 days in serum-free medium in the absence or presence of PACAP-38 (10-6 M). RESULTS: Treatment with PACAP-38 resulted in an increase in follicle diameter by 75% whereas treatment with follicle stimulating hormone (FSH) increased follicle diameter by 65%. PACAP-38 treatment enhanced the granulosa cell proliferation as measured by thymidine incorporation analysis. Furthermore, the production of progesterone by cultured granulosa cells and GFSHR-17 cell line was stimulated by PACAP-38. Interestingly, PACAP enhanced FSH action on stimulation of SF-1 and aromatase gene expression. CONCLUSION: The present results demonstrate that PACAP stimulated preantral follicle growth by potentiating proliferation and by stimulating steroidogenesis.

6.A Case of Diffuse Leukoencephalopathy Caused by 5-Fluorouracil Derivatives.

A stomach cancer patient is reported, who presented with asterixis and dysarthria, after the administration of 5-fluorouracil derivatives, and progressed to a state of arnnesia and disorientation. At first impression was metastatic brain tumor however7 the result of a cerebrospinal fluid cytology for malignancy was negative, and magnetic resonance imaging study(MRI) revealed diffuse hyperintense lesion of the white matter, indicative of leukoencephalopathy. Upon discontinuing the drugs, patient recovered significantly.lly predominant muscle weakness from distal myopathy with rimmed vacuole formation While we present a young female.

7.A Cses of Total Occlusion of the Left Main Coronary Artery.

A patient had total occlusion of the left main coronary artery that was proved by coronary arteriography. Patients with total occlusion of the left main coronary artery have a varying clinical presentation and may have prolonged survival. In patients with good collaterals, left ventricular function may be preserved. This report reveiws the clinical and angiographic findings of a patient with occlusion of the left main coronary artery with symptoms of unstable angina pectoris but without congestive heart failure or EKG evidence of myocardial infarction.

9.Expression of Proapoptotic Bcl-2 Family Member in the Mouse Ovary (i).

OBJECTIVES: Bok, Bcl-2-related ovarian killer, is a proapoptotic Bcl-2 family protein identified in the ovary based on its dimerization with the antiapoptotic protein Mcl-1. The present study examined the hormonal regulation and localization of Bok messenger RNA levels in the mouse ovary during the follicle development. METHODS: The animals were implanted subcutaneously with Silastic brand capsules containing the synthetic estrogen, DES at 21~23 days of age. Ovaries were collected 1~3 days after implantation for RNA analysis and in situ hybridization. Some mice were removed capsule for 1~2 days to induce ovarian follicle apoptosis. Ovaries were also collected from 26 day-old immature mice at various times after treatment with 10 iU PMSG. Some mice received a single intraperitoneal injection of 10 iU hCG to induce ovulation, and ovaries were obtained at different time intervals for Northern blot and in situ hybridization analysis, respectively. RESULTS: Treatment of immature mice with diethylstilbestrol (DES) for 24~48 h increased ovarian Bok mRNA levels. Bok mRNA was remained the same levels in mice removed DES for 24~48 h to induce apoptosis. High signals of Bok mRNA after DES treatment were detected in granulosa cells of early antral follicles. Treatment of immature mice with PMSG for 12 h increased markedly ovarian Bok mRNA expression which was detected mainly in preantral and atretic follicles. interestingly, low levels of Bok mRNA were also expressed in granulosa cells of preovulatory follicles. Treatment of PMSGprimed mice with hCG stimulated strongly ovarian Bok mRNA expression at 6~9 h. At that time, Bok mRNA was expressed in granulosa cells of atretic and small growing follicles. CONCLUSION: These results demonstrate that Bok is one of proapoptotic Bcl-2 members expressed in early growing and atretic follicles during the ovarian follicular development. Gonadotropins induce a transient increase of Bok gene expression in granulosa cells of preantral and preovulatory follicles indicating some role in the ovulatory process.

10.The basal electrical skin resistance of acupuncture points in normal subjects.

The inhibitory component of the skin against given electrical current, also called as the electrical skin resistance, is subject to change in response to many factors, especially pain. In order to find out more definite relationship between pain and skin resistance, one should make measurement in the state devoid of any external disturbing stimuli to get the "basal skin resistance (BSR)", which is known to be different from point to point on body surface. Also, the "active points" have more consistent BSR characters than other points and most of them share same locations with "acupuncture points" which is easy to localize accurately and repeatedly in normal subject. Therefore, the comparison of BSR of certain acupuncture points of normal subject and pain-suffering subjects is expected to be able to figure out any pain-induced BSR changes. Our measurement at 16 acupuncture points (asymmetrical 8 pairs) of 10 normal subjects showed inconsistently asymmetrical distribution of the BSR values with significant order among themselves including left SP (spleen)-6 at their lowest position, but neither the measuring system itself nor any of the 16 points was statistically reliable enough for diagnostic purposes. So the measuring device needs to be improved with after evolution, and more acupuncture points need to be explored to complete our pain-related BSR map. In addition, the meaning of the above BSR distribution pattern is waiting to be explained through such efforts.
