3.A Case of Solitary Cystic Kidney.

In our hospital, we experienced a case of left solitary cystic kidney. She was sixty years old woman. Her chief complaints were dull pain on left upper abdomen, terminal dysuria. tenesmus. and frequency. Cyst was situated to lower pole of kidney. Cyst was dissected from renal parenchyma. It contained about 250cc serous and clear cystic fluid, Post-operative general condition was good and her chief complaints were disappeared almostly.

4.Surgical Treatment of Cubitus Varus

The cubitus varus is one of the most common complications of supracondylar fracture of humerus. The authors treated surgically 45 cases of cubitus varus in the Seoul National University Hospital, from 1977 to 1981, and the results were as follows: 1. The mild ulnar n. palsy signs were found in 11 cases(24.5%). 2. The supracondylar, closed wedge osteotomy was done through lateral approach and fixed by cross pinning with 2 K-wires. 3. The postoperative immobilization must be more complete, and the average duration of immobilization was 7 weeks. The osteotomy site united in the all cases, and no limitation of range of motion developed. 4. The incidence of postop. ulnar nerve palsy was the same in the osteotomy only group as in the osteotomy and ulnar n. transposition group. The osteotomy and ulnar n transpositin group recovered naturally, but 2 cases of the osteotomy only group did not recover so ulnar n. transposition was performed later. 5. There was no recurrence of deformity after the average follow-up of 2.2 years, so it was recommended that the correction of deformity should be done in the early childhood.

6.A Clinical STudy on Nonunion of the Humeral Shaft Fractures in Adults

Usually, fractures of the humerus can be expected to unite. Circumstances often predisposing to nonunion are open reduction and internal fixation, infection, soft tissue interposition between fracture fragments and inadequate external immobilization. The effectiveness of various methods of treatment and the complication was discussed. The best results are achieved in cases treated by compression plates and iliac bone graft. Total 21 cases of nonunion of shaft of humeral fractures from Jan., 1980 to Dec., 1986 were treated at the Department of Orthopedic Surgery of Soon Chun Hyang University Hospital. The results were as follows ; 1. Incidence of nonunion of humerus shaft among the 270 cases has 7.8%; rates of non-union was 5.4% with nonoperative treatment of fractures of humeral shaft and 10.5% with operative methods. 2. Of 21 cases, male between 20–40th age group was predominant and mean age was 36 years. The cases of fracture was mainly due to the car and machinary accident. 3. Prevalent type and site was lower 2/3 transverse fractures. 4. There were associated injuries in ipsilateral upper extremity in 12(52%) patients. 5. The probable causes of nonunion were complex; 9 of inadequate internal fixation and fixation metal failure, 7 cases of poor external fixation, 3 cases of soft tissue interpostion between fracture fragments and 2 cases of infection. 6. The treatment performed were somewhat variable to include open reduction and plate (DCP) fixation for 18 cases, open reduction and circlage wiring for 2 cases and open reduction and screw fixation for 1 cases. All were supplemented with autogenous iliac bone graft. 7. Postoperative immobilization was done with shoulder spica cast, abduction splint and long arm cast for average 6.5 weeks. 8. All were united at average 4.7 months. Transient radial nerve palsy in 2 cases, and limitation of motion of shoulder and elbow iri 2 cases, which required considerable time of physical therapy, but postoperative infection was none.

8.A Study on Scoliosis Induced by Resection of Thoracic Spinal Nerves

Msny clinical and experimental studies trying to elucidate the cause of idiopathic scoliosis have been carried out. There, however, is no established theory to explain it. This paper is aimed at approaching one of various causes of idiopathic scoliosis by producing thoracic scoliosis in growing rabbits with unilateral resection of thoracic spinal nerves. All 43 rabbits were divided into four different groups. The first group(11 rabbits) consisted of rsbbits whose primary anterior and posterior divisions of the unilateral thoracic spinal nerves were resected. The second(11 rabbits) and the third(11 rabbits) were groups of rabbits whose primary anterior and posterior devisions were resected respectively. The fourth(10 rabbits) was the control group. The first group of rsbbits showed the most prominent curvature of the spine. The group of resection of primary posterior division showed less severe scoliosis than the first group but more marked scoliosis than the group of resection of primary anterior division. The biopsy of the denervated muscles included in the scoliosis revealed atrophy. It is concluded from the animal experiments with 43 growing rabbits that unilateral paralysis of the thoracic spinal nerves could be one csuse of iniopathic scoliosis and the primary posterior division of the thoracic spinal nerve contributed to the production of scoliosis more than the primary anterior division.

9.Study on Basic Culture Method of Tumor Chondrocytes

In vivo and in vitro culture of rat tumor chondrocytes is a good culture model for physiologic, pathologic and biochemical studies of tumor chondrocytes. Human tumor chondrocyte culture is another method, but it is difficult to maintain a cell line and in vivo culture. So called, Swarm Rat Chondrosarcoma, which wss developed spontaneously in a Spra-gue-Dawley rat and maintained by Dr. R. Swarm, can be cultured in vivo and in vitro at the same time. It is easy to maintain Swarm Rat Chondrosarcoma and is said that there are nearly no notable changes in cellular characteristics during consecutive cultures. In this study, in order to establish the basic culture method of rat tumor chondrocytes, the inoculated tumor mass were studied with swarm rat chondrosarcoma cell line which had been preserved at Orthopedic Research Laboratory of the Msssachusetts General Hospital. In vivo culture, injection of digested 1×10(6) cells, 1×10(7) cells and direct inoculation of tumor of mass were done at each 10 Sprague-Dawley rats group and examination was done at postinoculation 2, 3, 4, 5, 6, 7 and 8 weeks. In vitro culture, 1×10(6)/ml, 2×10(6)/ml and 4×10(6)/ml concentration cell suspensions were plated at 96-well-plate and observed at 2, 3, 4 and S days with inverted microscope. The results of this study are as follows. 1. In vivo culture, the best result was observed at direct inoculation of tumor fragments among 1×10(6) cells, 1×10(7) cells and direct tumor fragments inoculation. 2. The ideal time of obtaining tumor mass growing in rsts is between 4 to 6 weeks after inoculation. 3. In vitro culture, the proper cell density in 96-well-plate was 1 x 10(6)/ml among 1×10(6)/ml, 2×10(6)/ml snd 4 × 10(6)/ml.
