1.Ganglion around Cruciate Ligament of the Knee: Three Cases Report

Only a few cases of cyst related to cruciate ligament were reported in the literatrure. It is not only because of rarity but also difficulty of diagnosis by conventional method. Recentely, the precise intraarticular structure of the knee joint became to be seen by introduction of MRI and arthroscope and cycts would bave been found incidentally during the workup of the internal derangement of knee. We have experienced three cases of ganglion around the cruciate ligament in the patients whose symptom was mimic of intemal derangement of knee. We excised the ganglion with good clinical results.

2.A case of anosacral cutaneous amyloidosis.

We report a case of anosacral cutaneous amyloidosis in a 74 year-old-man. The skin finding is a patch of the size of a childs palm on the sacral area, consisting of brownish macular lines that radiate frorn the anus. Histopathology revealed defiosits of homogenous eosinophilic mateial s in the papillary dermis. Congo red staining and dylon staining was positive. Immuohistochemical staining using anti SAP antibodies and anti-keratin antibodies showed positive reactions.

3.Arthroscopic Total Meniscectomy of the Lateral Discoid Meniscus

The technique of the arthroscopic total meniscectomy of discoid meniscus is very difficult. Ikeuchi(1982) reported a technique of arthroscopic total meniscectomy of the lateral discoid meniscus by removal of 2/3 of the anterior part of the meniscus first and then by morselization. The purpose of this paper is to report a technique of arthroscopic total meniscectomy of the lateral discoid meniscus as a whole. During the period from October 1982 to October 1984, we had 12 experiences of arthroscopic total meniscectomy of the discoid meniscus and found good results, that is, little postoperative pain, short period of hospitalization and early restoration of joint motion without physical therapy. Complications are 3 cases of symptomless slight lateral instability and 4 cases of hemarthrosis.

4.Case of rat mite dermatitis.

We report three cases of rat mite dermatitis caused by Ornithonysus bacoti(Hirst, 1913), commonly referred to it as the tropicalrat mite. The first case, a 26-year old female had multiple pinhead sized vesicles with peripheral erythematous macule on the upper arms. The second case, a 34-year-old femal had multiple variable sized vesicles with erythematous base on the extremeties. The third case, a 43-year old male had multiple erythematous macules and papules with central punctum on the trunl: and extremities. Histopathologic sections from lesions of the first and second patient showed moderate perivascular lymphahistiocytic infiltration intermingled with some eosinophils. Causative mites, Ornithonyssus bacoti, female were collected around the skin lesion in rase 1, from the house dust in case 2 and furniture in case 3.

6.A Study on Scoliosis Induced by Resection of Thoracic Spinal Nerves

Msny clinical and experimental studies trying to elucidate the cause of idiopathic scoliosis have been carried out. There, however, is no established theory to explain it. This paper is aimed at approaching one of various causes of idiopathic scoliosis by producing thoracic scoliosis in growing rabbits with unilateral resection of thoracic spinal nerves. All 43 rabbits were divided into four different groups. The first group(11 rabbits) consisted of rsbbits whose primary anterior and posterior divisions of the unilateral thoracic spinal nerves were resected. The second(11 rabbits) and the third(11 rabbits) were groups of rabbits whose primary anterior and posterior devisions were resected respectively. The fourth(10 rabbits) was the control group. The first group of rsbbits showed the most prominent curvature of the spine. The group of resection of primary posterior division showed less severe scoliosis than the first group but more marked scoliosis than the group of resection of primary anterior division. The biopsy of the denervated muscles included in the scoliosis revealed atrophy. It is concluded from the animal experiments with 43 growing rabbits that unilateral paralysis of the thoracic spinal nerves could be one csuse of iniopathic scoliosis and the primary posterior division of the thoracic spinal nerve contributed to the production of scoliosis more than the primary anterior division.

7.In-vitro investigation of the mechanical friction properties of a computer-aided design and computer-aided manufacturing lingual bracket system under diverse tooth displacement condition

OBJECTIVE:
The purpose of this study was to compare the static (SFF) and kinetic frictional forces (KFF) of a computer-aided design and computer-aided manufacturing lingual bracket (CAD/CAM-LB) with those of conventional LB (Con-LB) and Con-LB with narrow bracket width (Con-LB-NBW) under 3 tooth displacement conditions.
METHODS:
The samples were divided into 9 groups according to combinations of 3 LB types (CAD/CAM-LB [Incognito], Con-LB [7th Generation, 7G], and Con-LB-NBW [STb]) with 3 displacement conditions (no displacement [control], maxillary right lateral incisor with 1-mm palatal displacement [MXLI-PD], and maxillary right canine with 1-mm gingival displacement [MXC-GD]; n = 6/group). While drawing a 0.016-inch copper or super-elastic nickel-titanium archwire with 0.5 mm/min for 5 minutes in a chamber maintained at 36.5℃, SFF and KFF were measured. The Kruskal-Wallis method with Bonferroni correction was performed.
RESULTS:
The Incognito group demonstrated the highest SFF, followed by the 7G and STb groups ([STb-control, STb-MXLI-PD, Stb-MXC-GD] < [7G-MXC-GD, 7G-MXLI-PD, 7G-control] < [Incognito-MXLI-PD, Incognito-control, Incognito-MXC-GD]; p < 0.001). However, there were no significant differences in SFF among the 3 displacement conditions within each bracket group. Within each displacement condition, the Incognito group demonstrated the highest KFF, followed by the 7G and STb groups ([STb-control, STb-MXLI-PD] < Stb-MXC-GD < 7G-MXLI-PD < [7G-control, 7G-MXC-GD] < [7G-MXC-GD, Incognito-MXLI-PD, Incognito-control] < [Incognito-control, Incognito-MXC-GD]; p < 0.001). MXC-GD exhibited higher KFFs than MXLI-PD in the same bracket group.
CONCLUSIONS
The slot design and ligation method of the CAD/CAM-LB system should be modified to reduce SFF and KFF during the leveling/alignment stage.
8.A Cses of Total Occlusion of the Left Main Coronary Artery.

A patient had total occlusion of the left main coronary artery that was proved by coronary arteriography. Patients with total occlusion of the left main coronary artery have a varying clinical presentation and may have prolonged survival. In patients with good collaterals, left ventricular function may be preserved. This report reveiws the clinical and angiographic findings of a patient with occlusion of the left main coronary artery with symptoms of unstable angina pectoris but without congestive heart failure or EKG evidence of myocardial infarction.

9.Clinical Study of Lichen Planus : An Analysis of 35 Cases.

BACKGROUND: Lichen planus is a papulosquarnous disease characterized by pruritic violaceous papules most commonly an the extremities of middle-aged adults. It may or may not be accompanied by oral and genital mucous membrane involvement. Its course is generally self-limited for a period of several months to years, but it may last indefinitely. OBJECTIVE: This study was performed to find the clinical characteristics of lichen planus. METHODS: Thirty-five cases of lichen planus comfirrned clinically and histopathologically were analyzed. RESULTS: The incidenc of lichen planus was 0.073% of the total number of dermatologic patients. The age distribution was in the range of 14-63 years old being most prevalent in the 6th decade and there was ariequal sex ratio. The most common site of involvement was the mucous membrane, and the extremities were more common than the trunk. Pruitus or pain and discomfort developed in 28 pat,ients(80% ). In most cases, the treatment was topical corticosteroid and antihistamine. Sixteen cses improved within 1 month to 2 years. Five cases recurred in the improved cases. CONCLUSION: The clinical analysis of 35 patients with lichen planus was performed. The incidence of lichen planus was 0.073% of the total number of dermatologic patients. Males and females were equally affected Mucous membrane involvement was detected in 21 patients(60%).

10.The Effect of Magnesium Depletion on the Electrocardiogram and Plasma Electrolytes in Liver Cirrhosis.

Plsama and urinary Mg were measured in relation to EKG change in 24 patients with liver cirrhosis, and summarized as below. 1. Plasma albumin and Ca were below normal, however Na, K, and Mg level were in the lower limit of normal. 2. Abnormal EKG findings were QT prolongation, ST elevation, peaked T, low T, ST depression and U wave in the order of frequency. 3. Coefficient of correlation between plasma Mg and QT prolongation was -0.5 compared with that of Ca-0.28. 4. Mg depletion was moderately correlated with plasma albumin (r=0.59). 5. Marked urinary Mg excretion in the presence of normal plasma Mg suggest that plasma Mg level poorly reflect total Mg depletion.
