Pancreatic ductal rupture can be an elusive diagnosis. The early signs and symptoms are often vague, and when it goes unnoticed, ductal rupture results in inceased morbidity and mortality. Unfortunately, no diagnoatic test has proven reliable in its timely detection. Endoscopic retrograde cholangiopancreatography (ERCP) is routinely utilized to visualize the anatomy of the pancreatic duct in elective situation. But the utilization of emergent ERCP will allow detection of pancreatic rupture in clincal settings where surgery might not otherwise have been prompted by complications. In addition, present, timely visualization of the pancreatic duct will permit intelligent planning for pancreatic sugery. Recently, we experienced a case of 36-year-old female who had upper abdominal blunt trauma with heavy iron. Her serum amylase was high but the physical examination was vague. Emergent ERCP was performed and it revealed dye leak from rurptured pancreatic duct at tail portion of pancreas. Emergent distal pancreatectomy and splenectomy was successfully performed. So we report this case with a review of literatures.

OBJECTIVES: The authors investigated the genotypes of ALDH II and the ways of coping with stress affecting the alcohol use of university freshmen to see the differences associated with the alcohol use according to genotype. METHODS: Five hundred fifty one Chungbuk National University freshmen were investigated on the amount of alcohol consumption per episode, the frequency of alcohol use, and the ways of coping with stress by using a self-questionnaires. Their blood samples were also analysed for their ALDH II genotypes by PCR-RFLP method. The degree of alcohol use was calculated by integrating the amount and the frequency of alcohol use, was assessed according to ALDH II genotype, sex, the ways of coping with stress associated. Collected data was analysed by stepwise multiple linear regression analysis according to genotype and sex. RESULTS: There was a significant difference of the amount of alcohol use per episode according to ALDH II genotype. But there was no difference of the frequency of alcohol use according to ALDH II genotype. Subjects with ND and DD genotype showed significantly lower degree of alcohol use than NN genotype. Women showed significantly lower amount, frequency and degree of alcohol use than men. Active coping, emotional expression, religious seeking, and support seeking for problem solving were associated with the degree of alcohol use in the group with D allele. Religious coping and fatalism were associated with the degree of alcohol use in women. CONCLUSION: There are significant differences of the degree of alcohol use and the ways of coping to stress according to sex and ALDH II genotype.

PURPOSE: Neonatal hyperbilirubinemia has benign courses in most cases, but the possibility of toxicity of hyperbilirubinemia required courses examination of every newborn infant to identify the severity of hyperbilirubinemia progress. This study aims to see how the body surface area of newborns influences the decline rate of serum bilirubin level in conventional phototherapy. METHODS: Based on the charts of the Pediatrics Department, Dae-Dong Hospital from January 2003 to December 2006, we analyzed 168 neonates diagnosed as neonatal hyperbilirubinemia (serum bilirubin > or =15 mg/dL) in retrospective way. We excluded newborn infants under 37 weeks of gestation and under 2,500 g birth weight and classified neonates into four groups by the calculation results of body surface area:males above 75 percentile (group A), males below 25 (group B), females above 75 (group C), and females below 25 (group D). RESULTS: Out of 168 samples, the number of group A, B, C, D was 30, 20, 20, 15 respectively. In conventional phototherapy, the mean decline rates of serum bilirubin of group B and D recording 2.09 mg/dL/day and 1.77 mg/dL/day, were significantly faster than those of group A and C recording 1.63 mg/dL/day and 1.41 mg/dL/day (P<0.01). No significant differences were found in different duration of phototherapy between groups below 25 percentile and those above 75 in both genders. CONCLUSION: In conclusion, body surface area influences of infants the decline rate of serum bilirubin level in conventional phototherapy.

A 65 year-old male patient presented with a large palpable mass beneath the areola of the right breast for 7 years. The resected breast tissue was almost totally replaced by a round large solid mass (9 6 cm) with a pink-gray to yellow firm, partly nodular cut surface. Microscopically, the tumor revealed the diagnostic biphasic cellular pattern of adenoid cystic carcinoma, which consisted of both cribriform pattern of myoepithelial cells and tubular pattern of epithelial cells. On immunohistochemistry, the tumor revealed immunoreactivities for alpha-smooth muscle actin and S-100 protein in the myoepithelial cells and for AE1/AE3 in the epithelial cells. Mitoses were scarce. Multifocal lymphatic permeation and foci of perineural invasion were also found. Underlying resection margins and overlying skin were invaded by the tumor. We diagnosed this tumor as grade II adenoid cystic carcinoma according to the system utilized for the salivary gland tumors.

To investigate the mechanism of monocrotaline-induced pulmonary arterial hypertension, authors performed immunohistochemical study using antibody to von Willebrand factor(vWF), cell kinetic study using 5-bromodeoxyuridine and ultrastructural study after single subcutaneous injection of monocrotaline(MCT) to Wistar rats. The results of this study demonstrated that the expression of vWF by pulmonary endothelial cells was markedly increased from day 3 until 2 months after MCT injection. The labeling index of pulmonary microvessel endothelium began to increase after six days and was maximal on the third weeks, and thereafter it remained slightly increased above basal level. Electron microscopic study revealed attachment of inflammatory cells an platelets to endothelium from 6 hours and degranulation of attached platelets 24 hours after MCT injection. Evidences of endothelial injury began to appear from 12 hours after MCT injection. Evidences of endothelial injury began to appear from 12 hours and was maximal after 48 hours. From the third day, ultrastructural change of cell regeneration and hypertrophy began to appear and was continuosly observed until 2 months. In addition, we evaluated the changes in the number of pulmonary neuroendocrine cells using antibody to gastrin releasing peptide but it demonstrated no change until 2 months suggesting no role of neuroendocrine cells in the development of pulmonary hypertension of Wistar rats at early stage. In conclusion, the results indicate that pulmonary hypertension by MCT injection is due to increased vascular resistance caused by vasoconstriction and hyperplasia of endothelium with musculariz ation of the pulmonary arterioles induced by endothelial dysfunction and some biologic substances released form endothelium and platelets.

OBJECT: Pre-ALL is a very rare preteukemic state, which percedes acute lymphocytic leukemia, while MDS(pre-ANLL), usually the well-known type of preleukemic state, precedes acute non-lymphocytic leukemia. Initially it shows transient pancytopenia without any evidence of leukemia in bone marrow findings, followed by acute lymphocytic leukemia after recovery from pancytopenia of a short period within weeks or months. We report a case with pre-ALL in childhood. CASE: A 15-month-old male baby was admitted with the complaints of fever and cough for 5 days and pallor for 2 weeks prior to admission. On admission, CBC showed pancytopenia without any evidence of leukemia, which was recovered spontaneously in a short period, and then was followed by acute lymphocytic leukemia of CALLA negative, early pre-B cell type. During antileukemic chemotherapy, he had suffered from severe bacterial infections and was finally died of sepsis 8 months after first admission. CONCLUSION: We report a case of pre-ALL in childhood, which was preceded by CALLA negative, early pre-B cell ALL, with a review of the literatures, briefly.

OBJECTIVES: Along with the fact that people change their attitude on aesthetic surgery, many people become to have positive viewpoint on aesthetic surgery. However, according to the recent study result, it showed that aesthetic surgery patients tend to have psychopathology. To investigate effects of aesthetic surgery and tendency of aesthetic surgery patients which is presumed to be changed, various psychological evaluations were performed in the sample who had undergone aesthetic surgery. Also psychiatric and psychological attributes of aesthetic plastic surgery patients were compared with those of reconstructive surgery group and those of normal controls. METHOD: 50 of reconstructive surgery group and 50 of normal control group were selected from Dept. of Plastic Surgery of Chungbuk National University Hospital and two private plastic surgery clinics in Cheong-ju. To find out each group's psychological attributes, pre- and post-operative psychological conditions were evaluated by administering basic survey, body image scale, ego identity scale, and SCL-90-R. RESULT: 1) Patients in the group of reconstructive surgery received more stress than those in the group of aesthetic surgery and normal controls. 2) There were no significant differences in the scores of body image scale, ego identity scale and SCL-90-R between reconstructive surgery group and normal control group. 3) After aesthetic surgery, aesthetic surgery group showed improvement in almost every item in SCL-90-R, such as somatization, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, hostility, paranoid ideation, GSI, PSDI and PST sub-scales. 4) The factors which affected on the satisfaction after surgery among group of aesthetic surgery were self-acceptance, uniqueness in self-identity scale and obsessive-compulsive, somatization in SCL-90-R. When self-acceptance was low and self-uniqueness was high, satisfaction was high. And the more patients complained about physical symptom, satisfaction was low. 5) The factors which influenced on the opinion of re-operation among group of aesthetic surgery were stability in self-identity scale, obsessive-compulsive and somatization in SCL-90-R, and physical health in body image scale. When stability was low, complaints about physical symptoms were high, viewpoint on one's health was negative and the chance of re-operation was high. 6) In group of aesthetic surgery, the higher the expectation was before surgery, the lower the satisfaction was after surgery. CONCLUSION: As described in above study result, patients who planned to have aesthetic surgery were not different from common people in terms of psychopathology. And it was also found that aesthetic surgery could improve patients' mental health. If simply administrable evaluation method to measure complaints about physical symptoms, viewpoint on one's health, stability and obsessive-compulsive is invented in the near future, it will be possible to screen patients who are likely to have low satisfaction after surgery and high tendency to have re-operation.

Subperiosteal Cortical Defect is a symptomless rarefaction of cortical bone which begins most often on the external surface of the medial and posterior cortical walls of the lower end of the femur in children between the ages of 3 and 6 years. The name of Subperiosteal Cortical Defect was first used by Aegerter who differentiated it from non-osteogenic fibroma. The authors found out 5 positive cases of incidental findings among 62 cases of both knee joint X-ray of which 3 was male and 2 was female. Also we experienced 3 cases which necessiat d a correct diagnosis and differential diagnosis. Because this is a common lesion, it frequently accompanies more serious bone conditions, and because this is self-limited disease, treatment is not necessary. It was emphasized that its nature should be understood by all those dealing with bone disease lest it be overdiagnosed and overtreated.
