1.Echocardiographic Diastolic Left Atrial Emptying Index in Hypertensive Patients.

M-mode echocardiography has been demonstrated to be a sensitive indicator of the sequential changes occurring in the heart in systemic arterial hypertension. The changes in function as well as anatomic features have been recorded. Previous analysis has focused on the anatomy and systolic function of the left ventricle, analysed in hypertensive heart disease. 13 normal subjects(Group I), 14 hypertensive patients without evidence of cardiac involvement(Group II) and 13 hypertensive patients with echocardiographic evidence of left ventricular hypertrophy(Group III) were compared using M-mode echocardiographic data and clinical. findings. The results are as follows : 1) Mean age are; Group I : 41 years(19-62), Group II : 49 years(25-67) and Group III : 53 years(30-83). There are no significant age difference between Group I and II. Sex ratio are 4:9, 8:6 and 7:6 respectively. 2) Mean arterial pressure are; Group I : 98+/-5mmHg, Group II : 126+/-7 mmHg and Group III : 131+/-101mmHg. 3) Left ventricular posterior wall thickness are; Group I : 0.87+/-0.12cm, Group II : 0.89+/-0.12cm and Group III : 1.32+/-0.18cm(p>0.05 in I vs II). 4) Left atrial dimension are; Group I : 2.77+/-0.68cm, Group II : 2.96+/-0.44cm and Group III: 3.12+/-0.60cm. 5) Left ventricular mass arel Group I : 194+/-51gm, Group II : 192+/-51gm and Group III : 318+/-77gm(p>0.05 in I vs II, p<0.001 in I vs II and II vs III). 6) Ejection fraction are; Group I : 0.57+/-0.09, Group II : 0.60+/-0.01 and Group III : 0.52+/-0.01)p>0.05). 7) Mitral valve EF solpe are; Group I : 113.70+/-23.26mm/sec, Group II : 81.14+/-17.22mm/sec and Group III : 69.92+/-22.70mm/sec(p<0.05 in I vs II, p<0.001 in I vs III and II vs III). 8) AEI are; Group I : 0.70+/-0.04, Group II : 0.56+/-0.07 and Group III: 0.48+/-0.07)p<0.001 in I vs II, I vs III and II vs III). From the above results, the atrial emptying index seems to be an early indicator of abnormalities of left ventricular diastolic compliance in hypertensive patients.

4.BOHE HEALING PROCESS IN EARLY MOBILIZATION AFTER VERTICAL RAMUS OSTEOTOMY OF THE MANDIBLE IN ADULT DOGS.

This investigation is an experimental study of the bone healing and remodeling process after Vertical Ramus Osteotomy(abbreviated as VRO) with early mobilization in adult dogs. Nineteen dogs were divided into three groups. Normal Control Group(N=2) was free of surgery. Experimental Group I(n=5) received only VRO without any intentional movements of distal fragments, and Experimental Group II(n=12) got VRO and backward driving of distal fragments. Animals were killed serially and the operated sites were reviewed grossly and histopathologically after the plain radiographs and Magnetic Resonance Images(abbreviated as MRI) were taken. The obtained results are as follows : 1. The early bone healing after VRO occurred mainly by the fusion of endosteal and subperiosteal calluses, and osteoinduced calluses from the cortices of fragments in intramembranous bone formation process. And the cartilaginous bone formations, which were considered to be associated with early functional movements of the mandible, were evident at the junction of these calluses. 2. The overlapping cortices of each fragment were fused by the osteoinduced calluses after decreased mobility of fragments mainly achieved by the subperiosteal calluses at the ateral and posterior part of pterygomasseteric sling. 3. Osteocytes in the cortices of fragments were dead by ischeinia and the marrow tissues were predominantly replaced by fibrous tissues in early stages. Active revascularization in the cortex and formation of new bone in the marrow, seen as the cancellous bones, were observed almost simultaneously and they were maintained until the 8th week when bone unions were confirmed. 4. Morphology of the mandible was changed byremodeling of the bone with reattachments of adjacent muscles after the 8th week. Callus in the gap was changed more and more into the compact bone after the 8th week, which united the proximal and distal fragments securely. 5. Low or iso-signal intensity at the cortex of fragments in early stages of MRI was being increased from the 4th week till the 12th week after VRO. The marrow spaces had high signal intensities on the T2-weighted images in early stages, but they returned to normal T1 and T2 high signals after the 12th week. 6. Signal intensities of the interfragmentary gap were high at the early stages and were gradually decreased to low signals at the 24th week. Atrophy of the masseter muscles with the high signal spots suggesting the increased vascularity was seen from the 2nd week, but the recovery of muscular volume with disappearance of high signal spots were also observed after the 8th week. In summary, the overlapped fragments made by VRO were completely healed by true bony union in adult dogs, even though they had cortex to cortex overlapping and early mobilizations had been allowed. Development of caelluses through the intramembranous bone formation, and the cartilaginous bone formations at the union of them tore mainly involved in the early bone healing. Direct communication of the marrows of the both segments was not found in this investigation, and more advanced study will be needed to explain the details of the fate of bony union after VRO.

5.Identification of the Orchid Mycorrhizal Fungi Isolated from the Roots of Korean Native Orchid.

The orchid symbiotic fungi were isolated from the roots of Korean native orchid (Cymbidium goeringii) collected and Chinese orchid (C. sinense) obtained from greenhouses. They were identified as a species of Rhizoctonia, based on the sequences of 18r rDNA, the microscopic observations of mycelia, and the symbiotic relationships with commercial orchids. The isolate collected from Chinese orchids was revealed to be a species of Ceratobasidium endophytica, and to be different from the other isolates at the thickness of the mycelia stained in the root cells of Korean native orchids. The other isolates collected from the Korean native orchids were considered to be a species of Tulsanella repens (anamorphic: Epulorhiza repens) or its related one. The physiologic or microscopic variations were oftenly observed among them, but the tendency of grouping these in the 18s rDNA sequences were observed to be consistent with those of the localities collected. The further taxonomical segregating for Korean symbiotic fungi was not made because the information concerned were limited in this moment, but was recognized as based on the sequences of 18s DNA.

6.Intrapoperative Femur fractures during Cementless femoral stem insertion

Fracture of the femur during total hip arthroplasty presents the simultaneous problems of prosthetic stability and fracture treatment. The treatment of such fractures can be complex and is one of the major challenges and this complication is associated with a high morbidity and a significant delay in convalescence. Among the 237 cases, who were treated by cementless total hip arthroplasty from Jan. 1986 to Dec. 1992, 13 cases of femoral fracture were experienced. They usually occured during insertion of prosthesis and preparation of the medullary canal. There were 6 cases of type II fracture, 1 case of type III fracture, 5 cases of type IV fracture and 1 case of type V fracture according to AAOS classification. All of the fracture healed, but the prognosis of these cases must be determined by follow up. We recommend that femoral fractures can be prevented by accurate preoperative evaluation of the size of the femoral canal, identification and management of the predisposing factors for intraoperative fractures.

7.A Case of Sebaceous Epithelioma.

Sebaceous epithelioma is an uncommon and primary neoplasm of sebaceous gland. We report herein a case of sebaceous epithelioma in an 85-year-old female. Physical examination showed a solitary, 2.5 x 3.5cm sized, reddish. irregularly surfaced, and well defined tumor on the intercanthal area. Histopathological findings showed tumor parenchyma. romprised of several large nests of three kinds of epithelial cells, mature sebaceous cells, undifferentiated cells arranged in pa1isade fashion at the periphery of a cell nest, and transitional cells showing beginning fatty vacuolizatiorn of the cytoplasm. Electron micocopy revealed immature dark cells, transitional cells, and mature sebaceous cell which showed many scattered foci of sebaceous ditferentiation characterized by intracytoplasmic lipid droplets and glycogen particls.

8.Paroxysmal pruritus as the fi rst relapsing symptom of neuromyelitis optica

Itching is a rare symptom in neurological disease. Paroxysmal symptom is uncommon in neuromyelitis
optica (NMO) or multiple sclerosis. We describe a 45-year-old woman who presented with paroxysmal
pruritus in association with relapse of NMO on two separate occasions. Cervical MRI showed a
lesion at a level corresponding to the dermatomal site of the itching. Paroxysmal pruritus could be
a predictive symptom of relapse in NMO unless there are other comorbid causes such as systemic
diseases, allergy, drug reaction, or emotional stress responsible for the itching.
