1.The Application of Endoscopic Mucosectomy in Various Mucosal Lesioss of Upper Gastrointestinal Tract.

In case of submucosal tumor, adenoma and atypism, we can't frequently use the snare polypectomy for treatment of them. Adenoma and atypism are premalignant, so other therapeutic modalities are necessary for resection of them. We have perfortned endoscopic mueosectomy such as strip biopsy, and "O"ring ligation on various mucosal lesions of upper GI tract in 23 patients(11 men, l2 women) from June 1993 to December l994. Fourteen patients wbo had adenoma were followed up endoscopically for 6 months to 24 months(mean: 14 months). The results were as follows: 23 patients(27 lesions) were enrolled in this study. Mean age was 50.3(range 32-74 years). The method of mucosectomy was strip biopsy in 19 cases(21 lesions), and "O"ring ligation in 4 cases(6 lesions). The rate of complete resection was 88.9%. If adenomas were resected incompletely, we added a laser therapy. There was no recurrence except a case in which carcinoma was detected on the other site during follow-up. No serious compication occurred such as massive bleeding or perforation. Based on the above results, the endoscopic mucosectomy is considered to be a safe and effective modality in the treatment of the flat polyps of upper GI tract and could be used for prevention of gastric cancer.

2.A Case of Gastric Anisakiasis Causing Severe Gastric Ulcer Bleeding.

Anisakiasis is a parasitic infectious disease caused by eating raw, infected fish. It causes severe abdominal pain-like acute abdomen, but not fatal. Anisakiasis was firstly reported by van Thiel et al. in 1960 in Netherlands and it has subsequently been reported in the Netherlands, Japan, the United states, Korea as well as other nations. Because Koreans like eating raw fish, the anisakiasis of the stomach was occasionally reported, but the case of severe gastric bleeding was very rare. So we report one case of gastric ulcer bleeding due to gastric anisakis larva proven by endoscopy and gastric resection.

3.Morphometrical Study of Uncinate Processes and Vertebral Body of Cervical Spine.

OBJECTIVE: The anatomical knowledge is the most important and has a direct link with success of operation in cervical spine surgery. The authors measured various cervical parameters in cadaveric dry bones and compared with previous reported results. METHODS: We made 255 dry bones age from 19 to 72 years (mean, 42.3 years) that were obtained from 51 subjects in 100 subjects who donated their bodies. All measurements from C3-C7 levels were made using digital vernier calipers, standard goniometer, and self-made fix tool for two different cervical axes (canal and disc setting). We classified into 4 groups (uncinate process, vertebral body, lamina, and pedicle) and measured independently by two neurosurgeons for 28 parameters. RESULTS: We analyzed 23970 measurements by mean value and standard deviations. In comparing with previous literatures, there are some different results. The mean values for uncinate process (UP) width ranged from 5.5 mm at C4 and 5 to 6.3 mm at C3 and C7 in men. Also, in women, the mean values for UP width ranged from 5.5 mm at C5 to 6.3 mm at C7. C7 was widest and C5 was most narrow than other levels. The antero-posterior length of UP tended to increase gradually from C3 to C6. The tip way, tip distance, and base distance of UP also showed increasing pattern from C3 to C7. CONCLUSION: These measurements can provide the spinal surgeons with a starting point to address bony architectures surrounding targeted soft tissues for safeguard against unintended damages during cervical operation.

4.Medical Education and Terminology : Focusing on Anatomical Terms.

The medical terminology we have used so far in our country has its origin in the terminology originally developed by Japanese scholors based on Chinese characters. Review has been done, focusing on anatomical terms, about the drawbacks of theterminology in educational aspect. We discussed the justification of changing the Japanese-Chinese terms in relation to the educational effects, the national identity and the role of physicians in a society. Also we discussed on how the direction of our future medical terminology should be changed and modified. New terms proposed in the Korean Association of Anatomists were classified in various points of view and some part of those discussions were presented in this paper.

5.The Effect of Sciatic Nerve Transection on the Somatostatin and Substance P Cells in Dorsal Root Ganglia of Rats.

The authors investigated the morphometric analysis of substance P(SP)- and somatostatin(SOM)- containing nerve cells in dorsal root ganglia. For this purpose, immunohistochemical method was used to determine the number, size and the morphological characteristics of SP- and SOM-reactive cells in L5 dorsal root ganglia of rats. In addition, changes in type A, type B, SP- and SOM-containing nerve cells in ganglia after sciatic nerve transection were also determined. The results were as follows : 1) SP- and SPOM-reactive nerve cells belong to the population of type B cell, but N/C ratios of immunoreactive cells were higher than others ; 2) in normal group, SP- and SOM-reactive nerve cells were 12.5 and 3.2% of total nerve cells in ganglia, respectively ; 3) the case of coexistence of SP and SOM in one cell was not found ; 4) and there was a marked reduction in the number of SP- and SOM-reactive cells at 2 weeks after nerve injuries. And SP-reactive nerve cells were increased in number at 6 weeks after operation, but SOM-reactive cells were not. According to these results, SP- and SOM-reactive nerve cells belong to type B cells, but do not coexist in one cell. These nerve cells were decreased in number after nerve transection. SP-reactive nerve cells were recovered at 6 weeks after operation but recovery of SOM-reactive cell was not found.

6.Morphometric Study of the Pedicles of Thoracic and Lumbar Vertebrae in Korean

Total 7,446 pedicle measurements were made from T1 to L6. Direct dimensional measurements were obtained from dried adult human spinal columns. Parsmeters considered were the pedicle isthmus diameter in the coronal and sagittal planes, pedicle angles in the transverse and sagittal planes, and the depth to the anterior cortex in a line parallel to the midline of the vertetral body and along the pedicle axis. The results were as follows. 1. The widest pedicle transverse diameters were seen at L6, the narrowest at T4. 2. The widest pedicle superoinferior diameters were seen at T12, the narrowes at T1. 3. In the transverse plane, the posterolateral to anteromedial pedicle axis orientations were seen at all levels except at T12 and T12. 4. In the sagittal plane, the pedicles angled caudally at L5 and cephaladly from L3 to T1. 5. In the thoracolumbar junction, the percentage less than 6mm in the transverse diameters of pedicles is most high at L1(27%) and followed by at the level L2, T11 and T12. 6. The use of 6mm transpedicular screws can be expected to violate the cortex of the pedicles in a significant number in the upper lumbar spine. 7. The 5mm transpedicular screws can be used safely below the T11 level. 8. It is advisable to use roentgen image intensifier during insertion of the transpedicular screws.

7.Clinical and Histological Analysis of 126 Cases of Gastric Polyps.

The gastrointestinal polyp is a premalignant lesion in varying degree. According to the size, the pathology and the location of polyps, it should be removed. Polyps in the gastrointestinal tract can be removed by surgical excision, electrosurgery, cryotherapy, regional radiotherapy and laser therapy with some limitations. Considerable advance in therapeutic endoscopy as well as in diagnostic endoscopy along with newly developed instruments has now made it possible to excise gastrointestinal polyps by means of high frequency generator. We have underwent endoscopic polypectomy, strip biopsy and "O" ring ligation on gastric polyps in 112 patients who visited in this hospital from July l988 to January 1994, and the clinical characteristics including histopathology has been evaluated. Removed polyps were variable in size from less than 0.5 cm up to 4cm. Finally, we concluded that endoscopic polypectomy is a safe and relativly simple procedure and postpolypectomy complication can be considerably reduced by conservative antiulcer therapy.

8.Efficacy of Endoscopic Variceal Ligation on Bleeding Esophageal Varices.

Endoscopic injection sclerotheraphy(EIS) was an effective method for treatment of bleeding esophageal varices. However, it might be associated with some undesirable complications. Endoscopic variceal ligation(EVL) is a recently developed method for control of active variceal bleeding and eradicating esophageal varices with similar efficacy and minimal risk of complications compare to EIS. We performed EVL in 40 patients who had recently bled from esophageal varices. Total 198 variceal ligations were performed during 64 separate EVL session. Control rate of acute bleeding was 90%(36 of 40patients) and 4 patients died after EVL because of failure of bleeding control. Rebleeding following initiation of EVL occured in 5 patients-three patients were successfully controlled by EVL, one patient was controlled but eradication was impossible and one patient died. Varices were eradicated or reduced to grade I in 31(86.1%) of 36 survivors by 1-9 ligation(mean 5.0) in l-4 EVL sessions(mean 1.6). After EVL, there were mild complications-mild substernal discomfort in 4 patients, mild dysphagia in 2 patients and fever in 2 patients. These results suggest that EVL is a safe and effective method for treatment of bleeding and eradication of esophageal varices with less complication.

9.Micromorphometric change of implant surface conditioned with tetracycline-HCl: HA and oxidized surface.

The present study was performed to evaluate the effect of tetracycline-HCL on the change of implant surface microstructure according to application time. Implant with pure titanium machined surface, HA-coated surface and TiUniteTM surface were utilized. Implant surface was rubbed with 50mg/ml tetracycline-HCL solution for 1/2min., 1min., 1 1/2min., 2min., and 2 1/2min. respectively in the test group. Then, specimens were processed for scanning electron microscopic observation. The results of this study were as follows. 1. Both test and control group showed a few shallow grooves and ridges in pure titanium machined surface implants. There were not significant differences between two groups. 2. In HA-coated surfaces, round particles were deposited irregularly. The roughness of surfaces conditioned with tetracycline-HCL was lessened and the cracks were increased relative to the application time. 3. The anodic oxidized surfaces showed the craterous structures. The surface conditioning with tetracycline-HCl didn't influence on its micro-morphology. In conclusion, the detoxification with 50mg/ml tetracycline-HCL must be applied respectively with different time according to various implant surfaces.

10.Comparison of Endoscopic Injection with Hypertonic Saline-Epinephrine(HSE) and Absolute Ethanol for Bleeding Peptic Ulcers.

BACKGROUND/AIMS: The endoscopic injection therapy with hypertonic saline-epinephrine(HSE) and absolute ethanol are the most widely and easily used procedure in the management of patients with bleeding peptic ulcers. We performed a prospective, randomized trial to compare the hemostatic efficacy between HSE and absolute ethanol injection therapy. METHODS: During the period of 1993 to 1996, 60 patients who were identified as active bleeding or visible vessel were enrolled for this trial. The 60 patients were randomly divided into two groups(HSE group, 30 cases: ethhnol group, 30 cases). We performed endoscopic injection therapy with 3% saline and 1:10,000 epinephrine solution in HSE group, and with 99.5% absolute ethanol in ethanol group. The two groups were matched for sex, age, site of bleeding, endoscopic findings, initial hemoglobin, and concomitant illness at randomization. RESULTS: Bleeding was initially controlled in 21(70%) of the HSE group and in 29(96.7%) of the ethanol group(p < 0.05). Rebleeding occurred in 4(19%) of the HSE group and in 2(6.9%) of the ethanol group(p < 0.1). The ethanl group achieved a better hemostatic effect for spurting hemorrhage(3/13 vs 9/10, p < 0.05). The emergent operations were undergone in 10(33.3%) and 2(6.7%) for HSE group and ethanol group respectively(p < 0.01). The stay in hospital were less than in ethanol group(mean 9.5 vs 16.3, p < 0.01). The death were noticed in 6 cases(20%) in HSE group and 1 case(3.3%) in ethanol group(p < 0.05). CONCLUSIONS: This study shows that absolute ethanol injection is more efficaceous and better satisfactory method in peptic ulcer bleeding.
