1.A New Embedding Method for Optimal Tissue Preparation for Mohs Mierographic Surgery.

BACKGROUND: The principle of Mohs micrographic surgery is to examine microscopically the entire peripheral margin of an excised tissue specimen to precisely localize the site of any residual tumor and enable its subsequent removal. In processing in the tissue for frozen horizontal sections, a technical difficulty arises in manipulating the deep and lateral margins into the same flat plane. OBJECTIVE: Our aim was to develop a new method allowing the optimal tissue preperation for Mohs micrographic smgery and to evaluate the usefulness of this methad. METHODS: The technique involves conversion of a non-planar surgical margin to a planar surface by applying and freezing the specimen against a flat glass surface. We compared this modified method used in our laboratory with standard methods for tissue preparation of Mohs micrographic surgery.

6.A Case of Inflammatory Metastatic Carcinoma of the Breast.

A 51-year-old Korean woman presented with a non-tender, well-demarcated, reddish, edematous patch on the right anterior chest where a previous mastectomy and radiation therapy had been performed. She had been diagnosed as having infiltrating ductal carcinoma of the right breast about 1 year ago. Histopathological findings of the skin lesions were consistent with inflammatory metastatic carcinoma of the breast. Inflammatory carcinoma or carcinoma erysipeloides is a well-established entity most frequently associated with carcinoma of the breast. It is characterized by dermal lymphatic invasion by malignancy and clinically should be distinguished from erysipelas or cellulitis. We describe a case of inflammatory metastatic carcinoma derived from an infiltrating ductal carcinoma of the breast which can be clinically confused with radiation dermatitis.

7.A Case of Favre-Racouchot Syndrome Treated with Retinoic Acid and Surgical Excision.

Favre-Racouchot syndrome is a disease of senile or actinic processes and another name is nodular elastosis with cysts and comedones. Clinically, facial skin shows, especially lateral to the eyes, multiple open and cystically dilatated comedones and can be defined as cutaneous disturbances characterized by a degenerative process of the dermis associated with cysts and comedones. Sunlight is a strong causative factor and dermal degenerative processes result in this condition. We report the case of a 78-year-old man who showed multiple yellowish papules on both sides of cheeks, the periorbital and, malar areas. He had had this condition for 25-years. His job used to involve physical labor and much exposure to sunlight. An excisional biopsy specimen from the facial malar lesion showed cystically dilated comedones with basophilically degenerated dermal tissue. We performed a therapeutic excision on the two large nodular lesions on both malar areas and applied topical tretinoin. The Patients clinical appearance was much improved 2 months later.

9.Purification of the major allergens from Korean Dermatophagoides pteronyssinus and production of the recombinant antigens.

BACKGROUND: Purified major allergens of house dust mite are essential for evaluation of the allergic mechanism in molecular basis and development of new modalities of immunemodulation. OBJECTIVE: In this study, we aimed to purif group 1 and group 2 allergens from Dermatophagoides pteronyssinus (Dp). In addition, cDNAs corresponding to Der pI and II in Korean Dp were isolated and recombinant Der p1 and Der pII were synthesized. MATERIALS AND and METHOD: Der pI allergen was purified by ammonium sulfate precipitation, anion -exchange column chromatography, and gel filtrat,ion chromatography. Der pII allergen was purified by anion exchange chromatography, gel filtration chromatography, and a preparative isoelectric focusing method. RESULTS: Eight hundred ug of Der pI and 50 ug of Der pII were obtained from 100 g of culture medium and 1 g of mite bodies, respectively. The purities of these allergens were confirmed by SDS PAGE and the strong reactivity to the patient sera was identified. In order to produce a recombinant allergens, poly(A) RNA from house dust mites were isolated and used for cDNA synthesis by RT PCR. The cDNA was inserted into prokaryotic expression vector and the vectors were transformed into E. coli. A little amount of recombinant Der pI protein was produced due to the low solubility, and 1.2 mg of recombinant Der pII was produced from 1 L of E. coli culture medium. The antigenicity of Der pI was relatively weak, however, Der pII showed a strong antigenicity. Amino acid sequence of the amplified cDNA deduced from DNA sequences of Der pII showed 6 different variants. The variation of amino acid sequences suggests the possibility of high incidence of mutation of Der pII protein. CONCLUSION: A simplified method for the purification of Der pI and Der pII was developed. Recombinant allergens will be useful for the diagnosis and treatment of allergy with lower costs.

Authors were reviewed clinically for the patients who revealed ocular manifestations related to systemic diseases in year of 1968 (Table 1). The incidence of ocular changes related to systemic diseases (Table 2, 3, and 4), and ocular mansfestations of various systemic diseases such as infectious diseases, allergic diseases, endocrine diseases, nutritional and metabolic, central nerve system, cardiovascular diseases, hematopoietic diseases and connective tissue disorders (Table 5 to 22) were statistically and clinically observed in many respects. We observed some interesting cases such as macular edema (OU, aged 22, male) related to epidemic homorrhagic fever, monocular pseudoptosis in ocular conversion reaction and optic nerve atrophy (OU, aged 34, female) in toxemia.
