1.A New Embedding Method for Optimal Tissue Preparation for Mohs Mierographic Surgery.

BACKGROUND: The principle of Mohs micrographic surgery is to examine microscopically the entire peripheral margin of an excised tissue specimen to precisely localize the site of any residual tumor and enable its subsequent removal. In processing in the tissue for frozen horizontal sections, a technical difficulty arises in manipulating the deep and lateral margins into the same flat plane. OBJECTIVE: Our aim was to develop a new method allowing the optimal tissue preperation for Mohs micrographic smgery and to evaluate the usefulness of this methad. METHODS: The technique involves conversion of a non-planar surgical margin to a planar surface by applying and freezing the specimen against a flat glass surface. We compared this modified method used in our laboratory with standard methods for tissue preparation of Mohs micrographic surgery.

6.A Case of Inflammatory Metastatic Carcinoma of the Breast.

A 51-year-old Korean woman presented with a non-tender, well-demarcated, reddish, edematous patch on the right anterior chest where a previous mastectomy and radiation therapy had been performed. She had been diagnosed as having infiltrating ductal carcinoma of the right breast about 1 year ago. Histopathological findings of the skin lesions were consistent with inflammatory metastatic carcinoma of the breast. Inflammatory carcinoma or carcinoma erysipeloides is a well-established entity most frequently associated with carcinoma of the breast. It is characterized by dermal lymphatic invasion by malignancy and clinically should be distinguished from erysipelas or cellulitis. We describe a case of inflammatory metastatic carcinoma derived from an infiltrating ductal carcinoma of the breast which can be clinically confused with radiation dermatitis.

Favre-Racouchot syndrome is a disease of senile or actinic processes and another name is nodular elastosis with cysts and comedones. Clinically, facial skin shows, especially lateral to the eyes, multiple open and cystically dilatated comedones and can be defined as cutaneous disturbances characterized by a degenerative process of the dermis associated with cysts and comedones. Sunlight is a strong causative factor and dermal degenerative processes result in this condition. We report the case of a 78-year-old man who showed multiple yellowish papules on both sides of cheeks, the periorbital and, malar areas. He had had this condition for 25-years. His job used to involve physical labor and much exposure to sunlight. An excisional biopsy specimen from the facial malar lesion showed cystically dilated comedones with basophilically degenerated dermal tissue. We performed a therapeutic excision on the two large nodular lesions on both malar areas and applied topical tretinoin. The Patients clinical appearance was much improved 2 months later.

9.Primary Ovarian Transitional Cell Carcinoma.

Primary ovarian transitional cell carcinoma is extremely rare tumor. The histologic subtype was divided from malignant Brenner tumor due to it's own histologic characteristics and chemosensitive nature. Most of recent studies revealed that transitional cell carcinoma has a good response to chemotherapy and long-term survival. Recent histopathologic reports show that transitional cell carcinoma of the bladder and of the ovary are immunophenotypically different. We experienced a case of primary ovarian transitional cell carcinoma, and report this case with a brief review of the concerned literatures.

Extramammary Paget's disease is a rare cutaneous malignancy and frequently associated with an underlying adnexal carcinoma and perhaps with underlying internal malignancy. In our case, genital Paget's disease was followed by the discovery of renal cell carcinoma. We report a rare case of extramammary paget´s disease of the penis and scrotum which associated with a renal cell carcinoma.
