1.Two Cases of Keratosis Palmaris et Plantaris.

We described here-in a 36-year-old male patient (case 1) with a severe form of keratosis palmaris et plantaris(Thost-Unna type) which was diffuse hyperkeratosis with hyperhidrosis on the entire surface of the palms and soles. His son, 8-year- old boy, showed symmetrical distribution of hyperkeratotic patches on the inner sides of both great toes(case 2). Therefore, it was suggested this genodermatosis occured in two generations of a family. The biopsy taken from the right palm of case 1 showed features of epidermal hyperplasia with hyperkeratosis, hypergranulosis and acanthosis. We tried the oral Vit. A therapy(50,000 unit, daily) in case 2. The hyperkeratotic lesions became slightly improved in 4 months after therapy.

2.Syphilis Prevalence in Teagu Hope Village.

Serologic tests for syphilis, including VDRL and TPHA tests, and Physical examination were carried out on 1, 605 subjects who lived in Taegu Hope Village, a social welfare center, from April, 198R to May, 1985. We prepared this study comparing with previous reports for healthy individuals and prostitutes. The results were as follows, The reactive rate of VDRL. qualitative test was 11. 6% in total, 10. 3%, in men and l3. 5%. in women. The reactive rates of VDRL. qualitative tests were highest in 7 th decade as 28, 7% and followed by 6 th decade as 16. 7% the decacile as 16. 0% 8 th decade as 14,3% 4th decade as 8. 8% and 3rd decade as 5. 9%, 3 In ]Hg sero-positive subjects, VDRL quantitative tests were done, and 35. 5% were reacted with 1: 8 or high and 29. 0% had one or more syphilitic lesions. 4 In 54 subjects who had one or more syphilitic lesions, 19 had chancres, 17 had alopecia syphilitica, 7 had rnaculopapular syphilid, 7 had condylomata lata. 3 had papular syphilid, and 1 had maculopapular syphilid and alopecia syphilitica. The biological false positive rate was 10. 1,4 in 138 VDRL reactive subjects, using 1;he TPHA test standard.

3.A Case of Benign Hemangioendothelioma.

We report here-in a case of benign hemangioendothelioma occurring in a 2 day-old male neonate on central portion of lumbar area. The lesion was movable, and elevated firm nodule, measuring 1.5x2.0 cm in size. For the purpose of diagnosis and therapy, excisional biopsy was performed, which revealed typical histopathologic pictures in the deep dermis and subcutaneous area. The lesion showed no recurrence at follow-up check after 6 months.

4.A Case of Bronchial Obstruction with Mucus Plug following Endotracheal Intubation .

To keep the patent airway during a general anesthetic procedure or when handling an unconscious patient is the most important concern as well as responsibility and/or duty of the anesthesiologist. Many delicate operations could and can be performed because of the development of anesthetic techniques aad methods, especially through the introduction of endotracheal intubations. However, at times, an endotracheal tube can unfortunately be a hazardous instrument-lethal instead of lifesaving. The authors experienced a case of left main bronchial obstruction with a thick mucus plug immediately following the completion of oro-endotracheal intubation. This was successfully remedied by direct bronchoscopic aspiration under transtracheal spray. In such a circumstance as this, the newly designed instrument, namely; a bronchofiberscope, is a real necessity.

5.Surgical Treatment of Intracranial Arachnoid Cyst by Excision.

Arachnoid cysts are benign developmental cysts that occur between aachnoid layers, which represent approximately 1 percent of all intracranial space-occupying lesions, their most common location being the middle cranial fossa. Current surgical treatment options for intracranial arachnoid cyst are either craniotomy and excision of cyst or shunting of cyst contents extracranially. We treated 10 cases of intracranial arachnoid cyst by excision during last two years. For the best surgical curability, we have, not only removed the cyst wall and adhesive gliotic tissue as possible, but also made the opening to basal cistern for communication. During average 8.4 months follow up, we observed, not specific morbidity and mortality, but good clinical improvement and brain expansion. As above result, if haven't specific problem, the direct surgical removal may be the primary choice for intracranial arachnoid cyst, especially in children with noncommunicating type in sylvian.

6.Surgical Experience of Multiple Recurrent Astrocytoma: Case Report.

The treatment of recurrent supratentorial glioma is one of the most challenging and at the same time one of the oldest problem in neurosurgery, especially in case of multiple recurrence. We have experienced 15-year-old male, multiple recurrent supratentorial glioma patient who was subjected to 6 times of operation, radiation therapy and chemotherapy druing 5 year follow-up period. We reveiewed the literature and discussed about recurrent rate, malignant change, method of management and indication of reoperation in low grade glioma.

STUDY DESIGN: A literature review on the radiologic findings of pelvic parameters for treatment of spinal deformity OBJECTIVES: This review examines sagittal spine alignment, pelvic parameters, and methods for assessing alignment, and examines the relationships among all of these parameters to understand spinal deformity. SUMMARY OF LITERATURE REVIEW: Understanding the main pelvic and sagittal spinal parameters and recognizing their correlation is imperative in the diagnosis and treatment of various spinal disorders. MATERIALS AND METHODS: Review of the literature. RESULTS: As spinal and pelvic parameters tend to have a strong correlation, it is essential to measure not only spinal parameters but also pelvic parameters in analyzing sagittal balance. Degenerative changes have the potential to greatly disrupt the normal curvature of the spine, leading to sagittal malalignment. Analysis of sagittal balance is crucial to optimizing the management of spinal diseases. Improvement in surgical outcomes may be achieved through better understanding of radiographic spino-pelvic parameters and their association with deformity. CONCLUSIONS: Understanding spinal and pelvic parameters raises awareness of the relationship among alignment and balance, the soft tissue envelope, and compensatory mechanisms, which will, in turn, provide a more comprehensive understanding of the nature of spinal deformity and the modalities with which it is treated.

8.Home Visits in an Office Practice in Seoul.

BACKGROUND: As the population with chronic degenerative disease or functional impairment has increased in terms of the advanced aging society, the inevitability of home health services for the homebound have been augmented as a token of the trend toward the nuclear family along with the family capacity of care declined. For the past several years, home nursing service facilities have been enlarged and partially have been fulfilling these requirements. However, there are a great number of demands for home health service by physicians. Thereupon, we designed the following study to observe the cases of home visits by a medical office practice and to provide some information about the need of the home visit and its clinical features. METHODS: The study was conducted reviewing currently remained 84 data of home visit records at a home-visit- specialized medical office practice for 10 months in 1999. Its information collected was as below: patient's sex, age, frequency and duration of visit, distance to visit location, reason being homebound, and reason for visit. RESULTS: Of the reviewed records of 84 patients, comprised of 356 home visits, the median age of the patients was 67.5 years. They were visited 2 times as a median and with a median duration of 4 days. Physician drove a median distance of 6.1km one-way. Most common diagnoses were cancer and cerebrovascular diseases, equally with 10.8%. Reasons for being homebound were neurologic problem (28.6%), frail elderly (21.4%), terminal illness (20.2%) in order. Sixty two patients (73.8%) were permanently homebound and 12 patients (14.3%) were not. Reasons for visits were routine follow-up (42.1%) and evaluation of a new problem (19.9%) in the chronic homebound and terminal illness care (17.1%). While 102 visits (28.6%) should have begun by doctor-based visit, 233 visits (65.4%) including routine follow-up could be considered to be replaced for home nursing services. Even out of 233 visits, not all could be replaced and some should remain as physician's regular follow-up. CONCLUSION: There were needs of home visit in both permanent and transient homebound patients, in cases of exacerbation, new problem and routine follow-up of chronic homebound patients, and also in acute illness of previously healthy persons. To meet the needs of homebound patients in seeing the physician, and to offer adequate health services, the physician's role should be acknowledged in home nursing service, and home visit by physician should be institutionalized and carried into effect.

9.A Case of Zosteriform Kaposi's Sarcoma.

Kaposi's sarcoma is a neoplastic, vascular tumor involving the skin or internal organs. We report a case of a 72-year-old male who presented with multiple, grouped, purpuric papules, hemorrhagic bullae and nodules showing zosteriform distribution on the sole of his right foot. In our case, as in some of the cases from the literature, the patient was initially misdiagnosed clinically as having herpes zoster. Histopathologic findings revealed a diffuse infiltrate of small blood vessels and slit-like spaces extending through most of the dermis. Many of the spindle-shaped cells expressed factor VIII-related antigen, and human herpes virus 8 (HHV-8) DNA was detected by polymerase chain reaction (PCR) in paraffin-embedded tissue.
