2.Comparative Study of Panoramic Mandibular Parameters in Postmenopausal Osteoporotic women.

Osteoporosis has recently been recognized as a major health problem in the elderly population. The disorder is manifested as a loss of bone mass accompanied by structural alteration of bone and increased incidence of fracture. Mandible also may be affected. So, I evaluated panoramic views of 66 postmenopausal women for finding the possibility of useful diagnostic mandibular parmeters of osteoporosis. To know the correlationship between skeleton and mandible, the average of the bone mineral density of lumbar from 2nd to 4th by the dual energy X-ray absorptiometry(DEXA, LUNAR DPZ. USA), and age and mandibular parameters, that is, the number of residual teeth, alveolar ridge resorption ratio, panoramic mandibular index (PMI), mandibular cortical width (MCW), angular cortical thickness (ACT), ramus cortical thickness (RCT), morphology of mandibular inferior cortical (MIC) were compared. And I divided the all tested women to the osteoporotic group and non-osteoporotic group by the use of T-score-2.0, which was derived from skeletal bone mineral density (BMD). To find the correlationship of the each group with mandibular parameters, t-test and discriminant analysis were done. The results of the t-test were that all parameters were highly related with 2 groups (p<0.05). Especially ACT, MIC, age have had even higher correlationship than others (p<0.001). The results of the discriminant analysis by the use of these ACT, MIC and age were that the discriminant function was Z =-2.973+(-1.447)x(ACT)+1.131x(MIC score)+(0.052)x(age), the cutting score was 0.257 and the classification accuracy was 84.8%. Therefore I suggest that the consideration of the angular cortical thickness (ACT), the age of patient and the morphology of mandibular inferior cortical(MIC) may help find the osteoporosis.

3.Time-sequential Development of Multiple Squamous Cell Carcinomas on Palms and Soles.

Squamous cell carcinoma of the skin arises mostly in the head and neck regions, less frequently in the rest of the body, and rarely in the palms and soles. We report a case of a 6S-year-old man who had had time-sequential development of multiple squamous cell carcinomas on his palms and soles for the past 12 years. These lesions were, in order of time, ulcerative nodules, ulcerative hyperkeratotic papules, hyperkeratotic plaques, maceratied plaques, dark discolored patchs, and hyperkeratotic papules. They were treated by total excision and cryotherpy apart from the last squatnous cell carcinoma in 1994.

4.A STUDY ON THE ANTIMICROBIAL SUSCEPTIBILITY OF ORAL MICROFLORA IN CHILDREN SUSCEPTIBLE TO INFECTIVE ENDOCARDITIS.

The present study has been performed to evaluate 20 cardiopathy children and 20 healthy children's oral micorbes at the point of antimicrobial susceptibilities for antimicrobial prophylaxis to prevent bacterial endocarditis. The results were as follows: 1. Both groups had similar oral microbes. 2. The antimicrobial susceptibility of S. viridans were: Penicillin< Oxacillin< Ampicillin< Cephalothin< Erythromycin< Clindamycin< Gentamicin< Ciprofloxacin< Vancomycin=Imipenem. The cardiopathy group was slightly lower antimicrobial susceptibility rates than healthy group. 3. The antimicrobial susceptibility of Neisseriaceae were: Clindamycin< Erythromycin< Vancomycin< Penicillin< Gentamicin< Cephalothin< Ciprofloxacin< Imipenem. The antibiotics of bacterial endocarditis antibiotic prophylaxis regimens for dental procedures according to the American Heart Association were generally lower antimicrobial susceptibilities, so they were considered inadequate for the first selective antibiotics and Imipemem was best suitable antimicrobials. Conclusively, when choose antimicrobials for treatment or antimicrobial prophylaxsis for bacterial endocarditis, surveillant culture must be performed to evaluated personal antimicrobial susceptibilities of intraoral microbes for proper antimicrobial choice for dental procedures.

5.Lateral Cephalometric Analysis of Asymptomatic Volunteers and Symptomatic Patients with Temporomandibular Internal Derangement.

Study of dentofacial structure relationships relative to TMJ internal derangement is required to increase the predictability of TMJ internal derangement. But few studies have been reported. The purpose of this study is to reveal any correlation of dentofacial characteristics with TMJ internal derangement by lateral cephalometric analysis. Patients were devided into two groups. (1) Symptomatic patients with TMJ internal derangement (2) Asymptomatic volunteers with no TMJ internal derangement. Twenty symptomatic patients with TMJ internal derangement(7male, 13female) were selected from our clinic and had undergone a standarized clinical examination, panorama, transcranical view, TMJ tomography. Twenty asymptomatic volunteers(9male, 11female) were selected from our clinic with no pain, no limitation of motion. All subjects had undergone lateral cephalometric analysis. The results were obtained as follows. 1. No significant difference between ID and normal group is detected in cranial base. 2. Maxilla position of ID group is located more posterioly than normal group. 3. Mandible position of ID group is located more posteriorly than normal group and facial profile is hyperdivergent. 4. Posterior facial height of ID group is less than normal group thus facial profile is hyperdivergent. The patients, as mentioned, have a high prevalance of ID thus it should be careful in TMJ ID diagnosis and treatment.

6.A Case of Chronic Gonorrheal Orbital Cellulitis.

7.Augmented bony change following platelet rich plasma(PRP) application on maxillary defect.

PURPOSE: Platelet Rich Plasma(PRP) application is increasing with sinus inlay bone graft, but there is few research with radiographic ananlysis on effect of PRP in maxillary sinus. The author investigated the amount of bony changes of maxillary sinus for dental implantation among the patients with maxillary inlay graft. MATERIALS AND METHODS: With 10 patients who were treated with sinus inlay autologous bone grafting combined with PRP technique, and with 5 patients who were treated with sinus inlay grafting only without PRP, the panoramic radiographys which were taken at preoperation, immediate postoperation, 3months postoperation, and 4 months postoperation(a month after dental implantation)periods were analysed. The films had been scanned, and then proceeded throughout image analysis system. The bone density of maxillary grafted sites was compared with adjacent tooth enamel density and remeasured according to density luminosity of each film. The density changes on PRP group and bone graft only group were an lysed with non-parameteric statistics method. RESULTS: In PRP combined patients group, bone density on postoperation periods was increased totally. The remarkable enhanced change of bone density was observed on 3 months postoperation period, thereafter the increasing rate was slightly reduced . In only bone graft patients group, bone density on postoperation periods was also increased compared with preoperation period, but the bone density of 4 months postoperation period was decreased compared with 3 months postoperation period. The amount of bone density on PRP group was significantly changed according to periods in contrast to bone graft only group. CONCLUSION: The bone density on PRP group was remarkably increased at 3 months postoperation compared to bone graft only group and it was seemed to be associated with more new bone formation, less grafted bone resorption at bone grafted sites with PRP.

8.A clinical study on ludwig's angina.

This is a retrospective study on the Ludwig's angina patients treated in Dept of Oral and Maxillofacial Surgery of Pusan National University Hospital for the period of Jan. 1985 to March 1996. The used materials of this study were 30 in total, including 18 male patients, 12 female patients. The author analyzed the distribution and incidence of sex, age, etiologic factor, pus culture, involved systemic disease, treatment methods, mortality rate. The results were as follows; 1. The incidences was higher in sixth decades(26.7%) and prevalent in male. 2. The most cause was odontogenic(83.3%) and in 15 cases(50%), the symptom aggravated after dental treatment. 3. The causative organism isolated from pus culture were Streptococci (40.7%), Staphylococci(22.2%), Bacteroid(14.8%), Candida(11.1%), Pseudomonas(3.7%), Pneumococci(3.7%), Salmonella(3.7%). 4. The patients with systemic disease were 66.7%, involved disease were DM(25%), malnutrition(22.7%), rheumatoid arthritis(11.4%), pulmonary disease(11.4%), hypertension(11.4%). 5. In all patients, surgical I&D was done, and 5 cases(16.7%) required tracheostomy. 6. In all patients, we used combination antibiotics, and the most commonly used antibiotics in the treatment were combination of Penicillin, Cephalosporin and Aminiglycoside(46.7%), and combination of Cephalosporin, Aminiglycoside and Metronidazole(30%). 7. The mortality rate was 6.7%.

10.Plasma Cell Leukemia Having Pseudopods: A Case Report.

Plasma col1 leukemia with motility-related morphological behavior is rarely studied. The plasma cells have variable degrees of cytoplasmic morphologies as dairy Projections, long extensions and pseudopods. These morphological evidences show the papa bility of wide spread and dissemination of disease itself. We present a case of a 38 year old woman who had back pain for 4 months and was diagnosed as a solitary plasmacytoma of the third lumbar vertebra. In spite of resection of the tumor and chemotherapy, the plasmacytoma was disseminated into both breasts and ovaries within less than a year. On her blood examination, we counted 34% of plasma cells in peripheral blood and 91.6% of plasma cells in bone marrow aspiration. Most of them resealed hairy projections and pseudopods of the cytoplasm.
