2.Seroconversion in Nonresponders to Hepatitis B vaccination after double dose vaccination only once.

BACKGROUND: Korea is, known to be an endemic area of viral hepatitis B, and 5-6% of population are carriers. Immunization can reduce the spread of hepatitis B infection. Hower, not all people respond to hepatitis B immunsation. 5-15% of primary vaccinees fail to be immunized. After revaccination, only 37.5 44% of the primary failures elicit antibody response. Recent studies have reported that it is effective to inject a double dose vaccination in immunocompromsed patients. In some country, they recommend that neonate whode mother is HBV carrier shoud receive double dose vaccination. This study documented the results of double dose vaccintion only once to nonrsponders toward the HBV immunzation. METHODS: Healthy nonresponders to Hepatitis B immunization were recuited from Sep. 1995 to Aug. 1996. Subjects with their AST/ALT level over 40, or over 65 years old were excluded. They were received 2ml of plasma-derived vaccine(Hepavax-B) intramuscularly in the deltoid muscle. Anti-HBs antibody testing were performed at 1 to 3 months after vaccintion. RESULTS: 17 healthy nanreponders to Hepatitis B immunization(male 10, female 7) were included. Their mean ages were 47(32 to64), AST/ALT level 20/19(16 to 28/11 to 35), smoking 4.3pys(1 to 7). Seroconversion occured in 13 of them(76.5%). CONCLUSIONS: Nonresponders to Hepatitis B vaccintion respond well to double dose vaccination once.

The absence of palmaris longus musle is common, about 10% in general population, but absence of another muscle(S) in forearm and/or hand is very rare. We experienced a case of conenital absence of flexor digitorum profundus in both hands. It was confirmed by surgical exploration. This anomaly seems to be extremely rare case in the English literature.

BACKGROUND: As people have been interested in healt,h and as the microcomputers with multimedia steeing have been supplied more and more, there is increasing need of multimedia information about health. The CD-ROM, which is a representative multimedia tool, is capable of interacting between information-provider and users, offering audiovisual interface, and saving great deal of knowledge. METHODS: We developed the health-information system to provide ordinary people with hea1th information using multimedia tools. It, has 7 parts ; self-diagnosis, exploring human body, drug information, hospital information, first-aid treatment, Q and A, and medical common sense. We got image data such as figures, photos, X-rays, electrocardiograms and pathologic specimens with scanner and film scanner, also moving image(video) with video-capturing program. RESULTS & CONCLUSIONS: We hope the multimedia health-information system to become a useful methodology of patient education and to be used as an information source in medical information network in the near future.

5.Phospholipase A2 Contributes to Hemorrhage-induced Acute Lung Injury Through Neutrophilic Respiratory Burst.

BACKGROUND: The present study was carried out in association with neutrophilic respiratory burst in the lung in order to clarify the pathogenesis of acute respiratory distress syndrome(ARDS) following acute severe hemorrhage. Because oxidative stress has been suggested as one of the principal factors causing tissue injury, the role of free radicals from neutrophils was assessed in acute hemorrhage-induced lung injury. METHOD: In Sprague-Dawley rats, hemorrhagic shock was induced by withdrawing blood(20 ml/kg of B.W) for 5 min and the hypotensive state was sustained for 60 min. To determine the mechanism and role of oxidative stress associated with phospholipase A2(PLA2) by neutrophils, the level of lung leakage, pulmonary myeloperoxidase(MPO), and the pulmonary PLA2 were measured. In addition, the production of free radicals was assessed in isolated neutrophils by cytochemical electron microscopy in the lung. RESULTS: In hypotensive shock-induced acute lung injury, the pulmonary MPO, the level of lung leakage and the production of free radicals were higher. The inhibition of PLA2 with mepacrine decreased the pulmonary MPO, level of lung leakage and the production of free radicals from neutrophils. CONCLUSION: A. neutrophilic respiratory burst is responsible for the oxidative stress causing acute lung injury followed by acute, severe hemorrhage. PLA2 activation is the principal cause of this oxidative stress.

7.Fiberoptic Laryngoscopic View of the Laryngeal Mask Airway Placed in the Hypopharynx.

BACKGROUND: The laryngeal mask airway (LMA) should be correctly placed into the hypopharynx for adequate ventilation. The purpose of this study was to evaluate a LMA position relation to the laryngeal skeleton and narrowing degree of a LMA lumen by the epiglottis. METHODS: The LMA (# 3 or # 4) was placed into the hypopharynx after induction of anesthesia and muscle paralysis. The fiberoptic laryngoscopic findings through the lumen of LMA were recorded at ten minutes after LMA placements. The position of the LMA was estimated in relation to its distal aperture to the laryngeal skeleton as central, posterior, right and left lateral position. The narrowing degree of the LMA by the epiglottis was estimated as 0%, 1~25%, 26~50%, 51~75%, or 76~100%. RESULTS: The fiberoptic laryngoscope showed central positions in 70.1%, lateral deviations to the left or right in 21.2% and posterior positions in 9%. The most frequent incidence (84/231, 36.4%) of narrowing by the epiglottis is 76~100% but ventilating problems were not developed. However, ventilation was impossible immediately after LMA placement in one patient, so the LMA was removed and the trachea was intubated. Esophageal enterance was visible in one patient without regurgitation of the stomach content. CONCLUSIONS: These findings show that LMA provides a reliable and safe airway management technique, although inadequate positioning and narrowing of LMA lumen by the epiglottis may frequently occur.

8.Comparative enamel bond strength between light- and dual-cured composites bonded by self-etching adhesives.

This study compared the microshear bond strength (microSBS) of light-cured and dual-cured composites to enamel bonded with three self-etching adhesives. Crown segments of extracted human molars were cut mesiodistally, and 1 mm thickness of specimen was made. They were assigned to three groups by used adhesives: Xeno group (Xeno III), Adper group (Adper Prompt L-Pop), and AQ group (AQ Bond). Each adhesive was applied to cut enamel surface as per manufacturer's instruction. Light-cured (Filtek Z 250) or dual-cured composite (Luxacore) was bonded to enamel of each specimen using Tygon tube. After storage in distilled water for 24 hours, the bonded specimens were subjected to microSBS testing with a crosshead speed of 1 mm/minute. The mean microSBS (n = 20 for each group) was statistically compared using two-way ANOVA, Tukey HSD, and t test at the 0.05 probability level. The results of this study were as follows; 1. The microSBS of light-cured composite was significantly higher than that of dual-cured composite when same adhesive was applied to enamel. 2. For Z 250, the microSBS of AQ group (9.95 +/- 2.51 MPa) to enamel was significantly higher than that of Adper goup (6.74 +/- 1.80 MPa), but not significantly different with Xeno group (7.73 +/- 2.01 MPa). 3. For Luxacore, the microSBS of Xeno group (5.19 +/- 1.32 MPa) to enamel was significantly higher than that of Adper goup (3.41 +/- 1.19 MPa), but not significantly different with AQ group (4.50 +/- 0.96 MPa).

10.Surgical Treatment of the Metastatic Tumors of the Femur.

Metastasis to bone with destruction of the skeletal system is a common problem in old patients with malignancy. Destruction of the musculoskeletal system poses major problems for the patients, including uncontrollable pain, forced immmobilization, pathologic fracture and hypercalcemia. The purpose of this study is to evaluate the effectiveness of surgical treatment and to suggest treatment guidelines for impending or established pathologic fractures due to metastasis of the femur. Between 1985 and 1995, thirty six patients who had pathologic or impending fractures in the femur underwent surgical treatment. The indications of surgery for pathologic fractures are 1) the possibility of survival time more than three months, 2) more benefits of sungical treatment than those of conservative treatment, 3) the possibility of rigid internal fixation, 4) patient s general condition allowing surgery, and those in impending fractures are 1) cortical destruction more than 50% or lytic lesion more than 2.5cm. The method of surgery was excision of the tumor and rigid internal fixation to allow early ambulation with or without bone cement augmentation. The results were analyzed by three criteria, the performance status, pain relief and survival time. Pain relief more than Lots was achieved in 80.6% of patients according to the Kaiko's criteria. The performance status improved more than one grade in all patients except one who died of acute respiratory failure and the mean improvement was 1.8 grade by the Functional Classification of the New York Heart Association. Postoperatively, most of the patients had only slight to moderate limitation of physical activities and felt comfortable at rest and ordinary activities. The survival time was 20.9 months in patients with single metastasis and 10.2 months in patients with multiple metastases. But there was no statistical significance due to large deviation of survival time in each case. In summary, surgical stabilization of the pathologic lesion involving the femur provided definite improvement of quality of life including pain relief and early ambulation. In selected patients with single metastasis, low grade malignancy, or good performance status, postoperative survival time may be expected to be prolonged. More aggressive treatment for the metastatic lesion of the femur is suggested.
