3.The Standard Electroretinogram in Non-diabetic Branch Retinal Vein Occlusion.

We carried out five common electroretinographies according to the recommendation by the International Society of Clinical Electrophysiology of Vision in the eyes of 40 non-diabetic brach retinal vein occulusion. The clear media, no previous history of photocoagulation and no evidence of neovascularization of the retina and/or iris were criteria for inclusion in this study. The data were compared with those of fellow eyes, expecting that this procedure may provide a new feature that could have some clinical significance. The reduction of the summed amplitude of oscillatory potentials, and the timing delay of the cone response b-wave and 30Hz flicker responses were of most frequent occurrence. hence, the reduction of the summed amplitude of oscillatory potentials and the timing delay of the cone response b-wave and 30Hz flicker responses may be valuable indicators. These abnormalities represent overall inner retinal dysfunction. The cone response b-wave timing was more frequently affected than that of the maximal combined response b-wave. This fact may reflect that an ischemic lesion is located in the vicinity of the posterior pole of the fundus.

4.New quantitative measuring technique for microleakage of the restored tooth through 3D reconstruction.

Established microleakage tests have their own disadvantages. In this study, 3D reconstruction method was tried to overcome these disadvantages. Four types of microleakage tests were used and relationships among them were estimated: penetrated dye volume; marginal adaptability; degree of dye penetration and relative penetrated length to cavity wall. Twenty-four Class V cavities were bulk filled with composite (Esthet X) following surface treatments: N group (no treatment); E group (etching only); T group (etching + Prime & Bond NT). 50% silver nitrate was used as a dye solution after thermocycling (5degrees C & 55degrees C, 1,000 times). Teeth were serially ground with a thickness of 0.2 mm. Volume of dye penetration was estimated from a three-dimensionally reconstructed image with a software (3D-DOCTOR). Percentage of margin without gap was estimated from SEM and degree of dye penetration and the relative length of dye penetration to overall cavity wall were also estimated. ANOVA and Scheffe test for dye volume, Kruskal-Wallis and Mann-Whitney test for marginal quality, Spearman's rho test for checking of relationships among methods were used. The results were as follows: 1. Dye penetration could be seen from several directions, furthermore, its volumetric estimation was possible. 2. Reverse relationship was found between dye volume and marginal quality (r = -0.881 / p = 0.004). 3. Very low relationship was seen between dye volume and two-dimensional tests (degree of dye penetration and relative length). However, 2D evaluation methods showed high relationship (p = 0.002-0.054) each other. 4. Three times vertical section could be recommended as a 2D test.

6.A Case of Dern atomyositis Associated with Primary Hepatoma.

Dematomyositis is an inflammatory cutaneo-muscular disease of unknown etiology, characterized by symmetrical proximal muscle weakness and typical dermatologic manifestation. Association with systemic maligmancy has been reported in many literatures, but the relationship of dermatomyositis to internal malignancy remains controversial. In Korea, several cases of dermatomyositis or polymyositis associated with malignant neoplasm were reported. Dermatomyositis or polymyositis associated with primary hepatoma has not been reported, Recently authors experienced a patient with dermatomyositis associated with primary hepatoma, and present the case with review of literatures. This may be the first report in Korea.

7.A study on the radiation effect on microvasculature of N-methyl-N-Nitrosourea-induced mammary carcinoma in rats

Mammary carcinoma was induced in rats by itravenous injection of N-methy-N-Nitrosourea. Microangiography was perfromed to evaluate the microvascular alterations in mammary carcinoma after irradiation. The tumors were givena single dose of 1.400 rads using Co-60 telethrapy unit with field size of 4 x 4 cm at 40 cm SSD. The dose ratewas 147.5 rads per minute. Microangiography was performed prior to irradiation and at one, two, and four weeksfollowing irradiation. The results are as follows: 1. Before irradiation, mammary carcinoma in rats tended to formlobules and the basic vasculature consisted of peripheral vascular pattern with central penetrating vessels. Theperipheral vascular pattern was always richer than that of the center. Irregular and tortuous vessles stretchedfrom the periphery into the center of lobule. 2. One week following irradiation, an increase in the number ofsmaller, tortuous vessels and decreased intervasular distance were obseved in the central portion of each lobule.This finding seems to be due to an improved filling of some previously existing but unfilled vessels. This maylead to improved metabolic changes and reoxygenation. 3. Later's changes of microvasculature after irradiation aretortuosity, irregularity, narrowing, abrupt tapering, fragmentation, and extravasation. These findings progressedafter a lapse of time. 4. The results can be considered as the microangiographic demonstration of the fact that reoxygenation after irradiation is mainly due to dilatation of the collapsed tumor vessels.

8.Post-Laminectomy Swan Neck Deformity in Adults

Five adult cases of post-laminectomy swan-neck deformity are reported. The factors causing the deformity, prophylactic measures, and treatment of the deformity are discussed. It is strongly adviced to the neurosurgeons and orthopedic surgeons that they should be aware of this unwanted complication of the multi-level cervical laminectomies, and that they should provide every preventive measures before post-laminectomy swan neck deformity develops. Anterior interbody fusion spanning the entire unstable segments is preferably recommended together with application of pre-fusion traction.

9.Flowcytometric DNA Analysis of Osteosarcoma Cells

Flowcytometry is a very important technique for the analysis of cell properties, with the advantages of simultaneous multiparameter analysis of large cell population in a short time. Recent advances in computer science and techniques in cell preparation and staining make it more valuable for the study of cell biology and its clinical application. This study was performed to establish the techniques of flowcytometry analysis of osteosarcoma cells, to evaluate the results of the characteristics of the DNA and specific cell cycle phase of osteosarcoma cells obtained by preparation of paraffin-embedded tissue blocks, and to analyze any possible difference between cell populations lacated apart from each other in the tumor mass for making a base for further clinical application. Paraffin-embedded tissue blocks were obtained from 10 cases of primary osteosarcoma, which had undergone amputation without chemotherapy or radiotherapy. Tissue blocks obtained from the most superficial and the deepest portions of the tumor mass from the skin surface were selected respectively in each cases. To evaluate the technique and results obtained, analysis of the whole sample were performed twice in a separate setting. Satisfactory DNA histogram was obtained from 14 of 20 tissue blocks, with the values of distribution in the specific cell cycle phases. DNA aneuploidy was found in 2 cases with a DNA index of 1.6 and 1.3, and no difference in DNA ploidy by the location in tumor mass. The S-phase and G2+M phase fraction were 13.2±8.5 and 6.2±3.1 respectively, reflecting the increased cell proliferation compared with normal cell population. There was no statistically significant difference of these values between superficial and deep portions, but the difference was 9.0±9.7 with a maximum of 26.6, much greater than the difference 3.3±3.6, between the first and second set of analysis. Flowcytometry is a very useful technique in the analysis of the DNA and cell cycle phase properties, and the characteristics of DNA and cell proliferation status of osteosarcoma cells were successfully evaluated by this technique. Unsatisfactory DNA histograms were thought to be the result of inappropriate samples. To adequately evaluate the changes in the tumor mass, standardization in obtaining tumor tissue about the location in the tumor mass is suggested for future studies with flowcytometry about the properties of tumor cells.

10.A case of a 6-year-old boy diagnosed with lipid transfer protein syndrome using the ImmunoCAP and ImmunoCAP Immuno Solid-phase Allergen Chip

We experienced a rare case of a 6-year-old boy who was suspected of having lipid transfer protein (LTP) syndrome; the patient was diagnosed with LTP syndrome using the ImmunoCAP/ImmunoCAP Immuno Solid-phase Allergen Chip (ISAC) test. LTP syndrome affects patients who are already sensitized to pollens with signs and symptoms of allergic rhinitis with or without bronchial asthma.Its severity is closely associated with the geographical location as well as the level, nature, and duration of the allergen exposure in a susceptible individual with or without cofactors, such as exercise, alcohol, chemicals, or nonsteroidal anti-inflammatory drug. Patients with LTP syndrome may present with diverse manifestations ranging from local symptoms, such as mild contact urticaria, oral allergy syndrome, or gastrointestinal problems, to anaphylaxis and even anaphylactic shock. Our case highlights the usefulness of the ImmunoCAP/ImmunoCAP ISAC test in establishing a diagnosis of LTP syndrome.
