1.Estimating the Validity and Reliability of the Geriatrics Global Support Scale(GGSS) and the Geriatrics Physical Support Scale(GPSS).

BACKGROUND: There are many rating scales for assessment of geriatrics. But each of these scales were not sufficient to evaluate comprehensive geriatric assessment, physical and psychologic efforts for care. METHODS: We developed new scales to comprehensive geriatric assessment for care of geriatric patients. The Geriatrics Physical Support Scale(GPSS) evaluates physical effort consists of 10 areas and the Geriatrics Global Support Scale(GGSS) evaluates general condition of patients consist of 6 areas. Eighty probable and possible AD patients received the Clinical Dementia Rating Rating Scale(CDR), Korearn version of Mini-Mental State Examination(K-MMSE), Barthel Activity of Daily Living Index(B-ADL) GPSS, GGSS. We tested internal consistency, correlation among dementia rating scales. RESULTS: The GPSS correlated to CDR -0.63(p<0.01), B-ADL -0.90(p<0.01) and the GGSS correlated to CDR -0.60(p<0.01), B-ADL -0.75(p<0.01). The internal consistency were 0.69(GGSS), 0.92(GPSS). CONCLUSION: The Geriatrics Global Support Scale evaluate general condition of patients and the Geriatrics Physical Support Scale evaluate physical efforts needed for care of geriatric patients. These scales an also brief and easy rating scales to grade degree of caregiver`s burden.

3.Prostate Metastasis of Renal Cell Carcinoma.

Renal cell carcinoma may metastasize to various portions of the body but dissemination to the prostate is very rare. The presenting symptom of a 79 year old man was acute urinary retention which was clinically thought to be benign prostatic hyperplasia. Radical nephrectomy was performed 22 months ago due to renal cell carcinoma. The histologic examination of the transurethral resection specimen demonstrated metastatic renal cell carcinoma and there was no distant metastasis except the lymph node.

4.FDG-PET/CT Complements Bone Scan with Respect to the Detection of Skip Metastasis of Osteosarcoma: A Case Report.

Skip lesion is not uncommon feature in osteosarcoma and considered to be importantly associated with poor prognosis factor, and thus, should be excised with the main mass. The accurate pre-operative evaluation of the intramedullary extent of osteosarcoma is essential, because it determines the level of bone resection. Among the reliable detection methods, bone scan has a drawback of high rate of false negative results and regional MRI has a difficulty to cover the whole involved lesions without clinical suspicion. The authors report a case of osteosarcoma of the distal femur with a proximal skip lesion that was not detected by either regional MR imaging or by bone scan, but which was visualized by FDG-PET/CT.

5.Analysis of Prognostic Factors in Surgical Treatment for Lumbar Disc Herniation.

It is important to follow strict indications for surgery and recognize prognostic factors in order to get good results in the surgical treatment of herniated nucleus pulposus. 55 patients with herniated nucleus pulposus were studied who had had laminectomy and discectomy and were followed up for more than 1 year in order to analyze the correlation between various factors including history, physical examination and radiologic finding and surgical results. Age, sex, symptom duration, location of the herniation, smoking, occupation, physical examination, disc height change, MRI findings, operation time and bleeding volume were evaluated as prognostic factors that seemed to influence surgical result. The results are as follow 1. Female had significantly better surgical results than male (P<0.05). 2. Non-smokers had significantly better surgical result than smokers (P<0.01). 3. In MRI finding, protruded disc had significantly better surgical result when it had involved spinal canal posterolaterally than centrally (P<0.01). 4. The less bleeding volume, the better surgical result (P<0.05).

7.A Case of Neobladder Rupture Following Blunt Trauma.

Bladder rupture following blunt trauma is rare, and no neobladder rupture following blunt trauma has yet been reported. We present a case of neobladder rupture following blunt trauma. The patient was a 65-year-old male patient who had been treated for bladder cancer via a radical cystectomy with an orthotopic ileal neobladder four years prior to this admission, and who was admitted to our emergency department due to multiple trauma after a 1.5 m fall. Primary repair was performed for the neobladder rupture.

8.Blunt Splenic Injury by Gunshot.

Trauma is frequently not purely penetrating or purely blunt. Such mixed trauma can result from the mechanism of injury. Recently, we encountered a patient who accidentally shot himself with a shotgun. He had a 15 x 8-cm-sized penetrating injury on left flank that did not penetrate into the peritoneal cavity and a blunt splenic injury with hemoperitoneum. Surgical and interventional treatments were performed for each injury. We present this case with a review of the related literature.

9.SpO2 Changes During Spinal Anesthesia with or witbout Epinephrine in Arthroscopic Surgery.

BACKGROUND: The purpose of this study is to evaluate the effects of spinal anesthesia with or without epinephrine on pulse oximeter readings, recorded from sympathetically affected(foot) and unaffected (hand) areas. METHODS: 20 Adult male patients, scheduled for arthroscopic surgery, received 3 ml of 0.5% hyperbaric bupivacaine with(group 1) or without(group 2) 0.1 mg of epinephrine into the subarachnoid space. Two pulse oximeter probes were applied to the index finger and toe of the patients, and the SpO2 values at 10min, 20min and 30min were recorded. RESULTS: After the onset of spinal anesthesia, a progressive decrease of SpO2 value recorded from the hand was observed at 20 min and 30 min (group 1), 10 min, 20 min, 30 min(group 2) compared with values from the foot. There is no significant difference in SpO2 value between group 1 and group 2 except for the SpO2 values of foot at 10 min after block. CONCLUSIONS: Because of compensatory vasoconstriction, which can occur more slowly when epinephrine is added to local anesthetics, pulse oximetry during spinal anesthesia gives a falsely low readings when oximetric sensor is placed at the upper limb.

10.Psychosocial Factors Associated with Suicidal Idea among Korean Elderly.

OBJECTIVES: The aim of this study was to analyze psychosocial factors associated with suicidal idea among Korean elderly. METHODS: The sociodemographic data and psychopathology were evaluated by questionnaire and psychiatric interview in community-dwelling elderly with age over 60 (N=100) and adults with age 30-49 (N=60). Suicidal idea was assessed by 9th item of Beck Depression Inventory (BDI). To analyze factors associated with suicidal idea in Korean elderly, we assessed family function (Family Adaptation Partnership Growth Affection Resolve, APGAR), alcohol problem (Cut Annoyed Guilty Eye-Opener, CAGE), depressive symptoms (HAM-D, BDI), cognitive function (CDR, MMSE-K, GDS), and functional activities (ADL, I-ADL). RESULTS: 1) The frequency of suicidal idea of the elderly with age over 60 (20.0%) was significantly higher than that of adult with age 30-49 (6.7%)(p<0.05). 2) The factors associated with suicidal idea were major depressive disorder (OR=42.17, 95% C.I. : 2.62-679.03), physical illness (OR=27.01, 95% C.I. : 1.18-616.50), depressive symptoms (OR=12.18, 95% C.I. : 2.91-49.98), poor economic status (OR=7.18, 95% C.I. : 1.05-48.86). 3) The means of variables in the elderly with suicidal idea were ADL 6.4+/-1.1, HAM-D 17.8+/-7.7, BDI 30.0+/-9.5, APGAR 4.8+/-2.6, which were significantly higher than those of the elderly without suicidal idea (p<0.05, respectively). 4) The HAM-D score in the Korean elderly had statistically significant correlation with APGAR (gamma=0.376, p<0.01), BDI (gamma=0.808, p<0.01), ADL (gamma=0.316, p<0.01), GDS (gamma=0.282, p<0.01), CDR (gamma=0.258, p<0.01) and education level (gamma=-0.208, p<0.05). The BDI score had statistically significant correlation with APGAR (gamma=0.341, p<0.01), HAM-D (gamma=0.808, p<0.01), GDS (gamma=0.340, p<0.01), CDR (gamma=0.301, p<0.01), ADL (gamma=0.325, p<0.05) and I-ADL (gamma=0.206, p<0.05). CONCLUSION: These results suggested early detection, psychogeriatric intervention of the elderly with high risk factors and active treatment of medical illness could reduce suicidal idea among them.
