1.THE EFFECT OF AUTOLOGOUS PLATELET-RICH PLASMA (PRP) ON BONE FORMATION AROUND DENTAL IMPLANT IN THE RABBIT : A HISTOMORPHOMETRIC AND REMOVAL TORQUE STUDY.

Platelet-rich plasma (PRP) has been known to increase the rate and degree of bone formation by virtue of growth factors in concentrated platelets. Although its great healing effect on bone defect or pre-implantation site preparation in conjunction with bone substitute has been reported, the effect associated with implant is unknown. The purpose of this study was to investigate the effect of PRP on rapid osseointegration of endosseous dental implants in the rabbit tibiae. Twenty two adult female New Zealand white rabbits, weighing approximately 2.7-3.3kg, were used for this study. Twelve of the 22 animals were used for histomorphometric analysis and ten of the 22 were for removal torque test. Each animal received two implants in each tibia (two treated with PRP and two as control) and was given fluorochrome intramuscularly. For histomorphometric analysis, rabbits were divided into four groups according to the healing period. At 1 week, 2 weeks, 4 weeks and 8 weeks postoperatively, each three animals were sacrificed serially and the amount and rate of bone formation around dental implant were examined on the undecalcified sections under fluorescent microscope, polarized microscope and light microscope connected to a personal computer equipped with image analysis system. For removal torque test, rabbits were divided into two groups and removal torque tests were performed at 4 weeks, 10 weeks after implant placement. In total, 88 screw shaped, commercially pure titanium implants (Neoplant, Neobiotech, Seoul, Korea) were used in this study. Labeling pattern reflected differences of two groups in bone formation rate at each period. Histomorphometrically, PRP group showed significantly higher bone volume within threads compared to control group at 2 weeks (70.30+/-4.96% vs. 50.68+/-6.33%; P<.01) and 4 weeks (82.59+/-5.94% vs. 72.94+/-4.57%; P<.05). PRP group at 1, 2 and 4 weeks revealed similar degree of bone volume formation comparable to control group at 2, 4 and 8 weeks, respectively. On the other hand, while PRP group showed higher bone-implant contact (47.37+/-8.09%) than control group (33.16+/-13.47%) at 2 weeks, there were no significant differences between PRP group and control group for any experimental period. Removal torque values also showed no significant differences between PRP group and control group at any experimental period (P>.05). These findings imply that PRP could induce rapid, more bone formation around implant during early healing period and get faster secondary stability for reducing healing period, though it has not induced bone maturation enough to resist functional loading.

2.A Case of Cystic Degeneration of Uterine Adenomyosis.

Adenomyosis is a common disease of middle-aged women and adenomyoma is a variety of adenomyosis that formed localized tumor. Cystic degeneration of an adenomyoma is a rare clinical manifestation. A 30-year-old parous woman suffered from severe dysmenorrhea and menorrhagia for about 5 months, was operated under the impression of endometriosis of the pelvis. Following the operation, cystic degeneration of an adenomyoma was found incidentally. The authors experienced a case of adenomyosis that formed cystic tumor of uterus and presented with a pertinent literatures.

A 62-year-old male was admitted for evaluation of a mass shadow on chest film. Chest PA showed 7×5cm lobulated homogenous mass in right upper medial area of lung. On chest computed tomography, there was a Barge irregularly lobulated mass with central necrotic low density area in apical segment of right upper lobe. Right upper lobectomy of the lung was performed. Partial adhesion to parietal pleura of posterior mediastinum and severe adhesion to right upper apicoposterior segment was found during the operation Microscopic and ultrastructural studies(including immunocytochemical stains) of the mass revealed malignant fibrous histiocytoma.

4.Effect of Probucol and Verapamil on Injury to Myocardium and Nerve Fibers in Rat Heart Induced by Doxorubicin.

BACKGROUND: Doxorubicin is considered to be one of the most effective drugs to treat a variety of human cancers. However, the dose-dependent cardiotoxicity of doxorubicin limits its clinical usefulness. This study aimed to evaluate the effect of probucol and verapamil on the cardiac neurotoxicity and cardiomyopathy induced by the long-term use of doxorubicin. METHODS: Sprague-Dawley male rats were grouped as the control group, the doxorubicin treated group, the doxorubicin treated with probucol group, and the doxorubicin treated with verapamil group. The rats were treated for 4, 6, 8 and 10 weeks. H&E staining and immunohistochemical staining for protein gene product 9.5, caspase-3, heat shock protein 70, and hsp 25 were performed. RESULTS: The degree of interstitial inflammatory cell infiltration was mildest in the probucol treated group. The reduction in the number of nerve fibers in the probucol treated group was less than the other treatment groups. There was a negative correlation between the treatment duration and stained nerve fibers in all the treatment groups. The number of caspase-3 positive cells was more increased in the doxorubicin group and the verapamil treated group than in the control and probucol treated group. CONCLUSION: It is suggested that probucol partly contributed to the inhibition of doxorubicin-induced cardiac neurotoxicity and cardiomyopathy, whereas the verapamil had no effect.

7.Mid Ureteral Stones: 5 Cases Treated with ESWL.

Use of ESWL has made many changes in the treatment of urinary trat stones. However, the treatment of the stone below the iliac crest was not possible owing to the arrangement of the apparatus and the location of the stone within the bony pelvis. For solving the problem, we selected 5 patients having mid ureteral stone. They were placed in prone position and administered shockwave from 500 times to 2600 times. The result was 3 patients had no residual stone and 2 patients showed decreased size of stones in follow-up examinations. All the patients showed no specific complication. The ESWL is able to disintegrate mid ureteral stone by using the change of patient`s position and avoiding the shield of bone around the stone.

9.Isolated Dorsal Ulnar Cutaneous Nerve Injury Occurring During Activities of Daily Living.

The dorsal ulnar cutaneous nerve (DUCN) is the sensory branch of ulnar nerve supplying sensation to the dorsal ulnar aspect of the hand. Isolated DUCN injury is known rare. We report 3 cases of isolated DUCN injury caused by stretch injury and mild blunt injury during activities of daily living. Isolated DUCN injury may more frequently occur than we thought. Nerve conduction study and ultrasonography are useful method to localize the lesion and to evaluate the structural etiology.
