2.A Case of Congenital Hyperextension of the Knee.

Congenital dislocation of the knee is a very rare condition and was first described by Chatelaine in 1822. The etiology of this condition is unknown. It is generally subclassified as simple hyperextension, subluxation, and dislocation, depending on the degree of the joint displacement and the severity of disease. There are a large of associated conditions that have been described, the most common of which are congenital dislocation of the hip, club foot, arthrogryposis, and Larsen's syndrome. The mainstay of treatment is early serial rnanipulation and splinting. Operative treatment was indicated whenever conservative treatment did not lead to satisfactory reduction. The authors experienced a case of congenital hyperextension of left knee associated with the calcaneovalgus deformity of both foot. Early closed treatment obtanied a successful reduction and satisfactory knee motion.

3.Isolated Dorsal Ulnar Cutaneous Nerve Injury Occurring During Activities of Daily Living.

The dorsal ulnar cutaneous nerve (DUCN) is the sensory branch of ulnar nerve supplying sensation to the dorsal ulnar aspect of the hand. Isolated DUCN injury is known rare. We report 3 cases of isolated DUCN injury caused by stretch injury and mild blunt injury during activities of daily living. Isolated DUCN injury may more frequently occur than we thought. Nerve conduction study and ultrasonography are useful method to localize the lesion and to evaluate the structural etiology.

5.Molecular Cloning of hSC2 Encoding 5α-reductase-like Protein.

BACKGROUND: The human homologue of the SC2 gene from a human dermal papilla cell cDNA library has been isolated and designated hSC2. HSC2 protein also shares similarity with 5 -reductase, a protein important in testosterone metabolism. OBJECTIVE: Prior to knowing the functions of hSC2 in dermal papilla, we cloned it and analyzed its relative expression levels in adult tissues and cancer cell lines. METHODS: hSC2 was isolated from low abundant clones in dermal papilla cDNA library using cDNA array hibridization method. Full-length clone was sequenced and we studied its expression in different tissues by Northern blot hybridization. RESULTS: Sequence data reveals a single open reading frame, encoding a putative hydrophobic protein with a calculated molecular weight of 36 kDa. Its deduced amino acid sequences are almost 97.4% identical to t4ose of rat protein. Northern blot hybridization shows that hSC2 cDNA recognizes a 1.35 kb transcript that was expressed in various epithelial and mesenchymal tissues including testis and liver. CONCLUSION: We have cloned and analysed tissue distributions of hSC2. It was interesting that it had homology with 5α-reductase isozymes. Further studies will be needed to understand the involvement of hSC2 in androgen hormone signaling.

7.A Case of Primary Conjunctival Tuberculosis.

Conjunctival tuberculosis is a very rare disease, but importance of tuberculosis in Korea is high. We experienced a 49 year-old female patient with a small mass like lesion at inferior fornix with conjunctivitis. We performed excisional biopsy. Biopsy result was conjunctival tuberculosis. So we searched other body tuberculosis focus, but, not found. we emphasize differential diagnosis chronic conjunctivitis and conjunctival tuberculosis. Brief clinical history and clinical findings will be presented with pictures of the pathology findings and some literatures will be reviewd.

8.The Results of Pigtail Probe Canalicullar Reconstruction Surgery with Silicone Tube and Methylcellulose.

We performed simple and successful canalicular reconstruction with smooth tip pigtail probe in 56 cases of inferior canalicular laceration. After general anesthesia or local anesthesia in inferior and superior canthal area, upper lacrimal sac area and lacerated inferior eyelid, methylcellulose mixed fluorescein was injected through the superior canaliculus under the surgical microscope with cobalt blue filter and simply found cutting end of the inferior canalicullus. Smooth tip pigtail probe was inserted and silicone stent was connected the superior and inferior canaliculi and the peri canalicular sheath was sutured with and to end anastomosis. Silicone tube was removed after postoperative 8 weeks and 86% of the patients(48/56 cases) was successful without epiphora. Smooth tip pigtail probe, methylcellulose mixed fluorescein, silicone stent, and end to end anastomosis of pericanalicular sheath under the surgical microscope would increase the success rate of pigtail probe canalicular reconstruction.

10.Clinical Observation on the Effect of Parenteral Reserpine.

Parenteral reserpine was given intramuscularly to 32 hospitalized hypertensive patients: 10 hypertensive patients without renal insufficiency, 3 hypertensive patients with heart failure, 10 hypertensive patients of malignant phase or with uremia, and 9 hypertensive patients with cerebrovascular accident. Follwoings were the result. 1. In the majority of patients, the effective dose of reserpine was 2 to 3 mg. 2. Reserpine given intramuscularly lowered blood pressure in 2 to 4 hours, had its maximum effect in 3 to 6 hours and had a duration of 3 to more than 24 hours (average 9 hours). 3. When effective dose of reserpine was given, blood pressure was lowered significantly (more than 30mmHg in mean blood pressure) in 18 patients (81.7%) of 22 hypertensive patients without renal insufficiency, and in 4 patients (40%) of 10 hypertensive patients with renal insufficiency. 4. Major side effect was drowsiness which was more evident in the patients with renal insufficiency. 5. Reserpine administered parenterally is an effective and safe agent for the treatment of hypertensive emergencies on a short term basis especially in the patient without renal insufficiency.
