The author performed an experiment to measure the bonding strength of acrylic bone cement when the bonding surfaces of bones were under various conditions; namely different axial compression forces and different means of preservation of bones, in the air and in Hartman's solution. Mechanical tests for compression forces revealed that the bonding strength was greatest when the bonding surfaces were dry, being weaker when soaked with alcohol and whole blood, in that order. When different strengths of axial compression forces were applied, the bonding strength was greatest at 5 kg of compression force, being weaker at 3 kg and 1 kg, in that order. The bonding strength was greater when the bones were preserved in the air than in Hartman's solution.

Penicillin is still the first choice of drug in the treatment of gonorrhea. The ever lowering sensitivity of Neisseria gonorrhoeae to penicillin and the emergence of PPNG (Penicillinase Producing Neisseria Gonorrhoeae) in the Far East and other parts of the world neceessitate reapprisal of treatrnent schedules with penicillin for gonococcal infections. In Korea, no authentical study in this regard has been done recently. The presence of intracellular gram r.egative diplococci in urethral smear as weIl as the presence of oxidase positive typical, round, convex, smooth, glistening, tratmslucent, greyish colonies of 0.5 to 2mm in diameter a 24 hours composed of grarn negative diplococci are used for presump'ive evidence of gonococccal infection 224 male patients with acnte uncomplicated gonococal infections were treated one of following regimens. Regimen A: 4.8mega unit combicillin (3.6mega unit procaine penicillin+l.2 mega unit sodium penicillin) i.m.. preceded by 1gm probenecid p.o., Regirnen B: 4.8 mega unit procaine penicillin i.m. preceded by 1gm probenecid P.O. Regimen A showed 14 failures(11.3% arnong 124. Regimen B showed 19 failures (19%) among 100 It is suggested that in Korea sesnsitivity to penicillin of Yeisseria gonorrhoeae is quite low.

To investigate the effect of tumor removal and administration of OK432 on the splenic natural killer (NK) cell activity in the subcutaneous tumor bearing rats, NK cell activity assay using a 4-hour 51Cr release assay and flow cytometric analysis for NK cell population were performed. The results were as follows: 1. Splenic NK cell activity and population in the subcutaneous tumor bearing rats decreased along with the growth of the tumor. 2. The rats with subcutaneous tumor removal showed decrease of splenic NK cell activity, but splenic NK cell population was not decreased. 3. In the rats with subcutaneous tumor removal and OK432 administration, splenic NK cell activity was significantly increased 1 week after administration of OK432 and then gradually returned to normal, whereas increase of NK cell population was not significant. In the present study, splenic NK cell activity was significantly decreased despite removal of subcutaneous tumor. But with the administration of OK432, splenic NK cell activity returned to normal. Considering the role of NK cells on the first line of defense against the metastatic implantation of circulating tumor emboli, we suggest that perioperative administration of immunopotentiator such as OK432 may improve the patient's outcome after surgery of human neoplasm.

PURPOSE: To investigate the prognosis, survival rate and curve on the eyes for corneal dystrophy and corneal degeneration, we performed statistical analysis. METHODS: Forty-one recipients (62 eyes) have received penetrating keratoplasty for corneal dystrophy and corneal degeneration for about 14 years from April 1984 to October 1997. We analyzed survival rate and curve on sex, age and type of corneal dystrophy and corneal degeneration. RESULTS: Mean age was 35 years and sex distribution of recipient was predominant in female patients (M: 16 recipients, 19 eyes, F: 25 recipients, 43 eyes). In the causative disease of first keratoplasty, corneal dystrophy and corneal degeneration were 46 eyes and 16 eyes. Granular type was the most common type of keratoplasty. In 62 grafts, graft rejection was developed in 19 eyes (30.6%). Graft rejection periods ranged from 53 days to 10 years and 3 months. Mean graft rejection time was 2 years and 8 months. In this study, 5 years survival rate developed in 75% of corneal dystrophy and developed in 31% of corneal degeneration. The statistical analysis showed significant relationship between survival rate and disease type (p=0.0032, p<0.05). But age and sex were not maintained with association with survival rate (p>0.05). CONCLUSIONS: This result suggests that corneal dystrophy was higher than corneal degeneration in survival rate with statistically significant difference. But sex and age did not have influence on survival rate.

OBJECTIVE: To investigate the optimal condition for activation of motor cortex by electrical stimulation of forelimb in rat. METHOD: Eleven adult rats (Sprague-Dawley rat) were studied, each of which was anesthetized and craniotomized. While the electrical stinulation was given on the contralateral forelimb of the fixed rat at stereotaxic frame, the evoked potential (EP) was obtained at the motor cortex of rat brain. The conditions of electrical stimulation were changed with 5 kinds of frequencies (1, 3, 5, 10, 15 Hz), 4 kinds of stimulus intensities (1, 3, 5, 7 mA) and 3 kinds of pulse widths (100, 200, 300 microsecond). RESULTS: The peak latencies of EPs in the motor cortex were significantly decreased and the amplitudes were significantly increased along with the decrement of stimulus frequency and the increment of stimulus intensity. The peak latencies and amplitudes of EPs were not significantly changed by stimulus pulse widths. CONCLUSION: The motor cortex of rat was more activated with lower frequency and higher intensity regardless of pulse width in the given condition of electrical stimulation in this study.

A 62-year-old male was admitted for evaluation of a mass shadow on chest film. Chest PA showed 7×5cm lobulated homogenous mass in right upper medial area of lung. On chest computed tomography, there was a Barge irregularly lobulated mass with central necrotic low density area in apical segment of right upper lobe. Right upper lobectomy of the lung was performed. Partial adhesion to parietal pleura of posterior mediastinum and severe adhesion to right upper apicoposterior segment was found during the operation Microscopic and ultrastructural studies(including immunocytochemical stains) of the mass revealed malignant fibrous histiocytoma.

We had experienced anesthetic management of two patients with bilateral pheochromocytoma. They had been treated with phenoxybenzamine for 4 weeks preoperatively. Anesthesia was managed with thiopental sodium for induction,enflurane-N2O-O2 for maintenance, vecuronium for muscle relaxation, and sodium nitroprusside for controlling severe hypertension. After tumor resection, severe hypotension was controlled by rapid transfusion, fluid and dopamine infusion. A tolerable blood pressure and pulse rate were maintained throughout the procedure. Preoperative preparation, sufficient sedation, smooth anesthetic induction, complete analgesia, good muscle relaxation, adequate ventilation and proper cardiovascular control are required in resection of pheochromocytoma.

OBJECTIVE: The authors describe four patients in whom we performed a single stage, pedicle subtraction and extension wedge osteotomy for the correction of sagittal imbalance by creating lumbar lordosis. METHODS: One patient presented with congenital hemivertebra, one with post-laminectomy kyphosis, and two with lumbar degenerative kyphosis. All patients suffered from back pain and were unable to walk due to forward stooping of the trunk. We performed a single stage, pedicle subtraction and extension wedge osteotomy for the correction of sagittal imbalance. RESULTS: The angles of correction were 37, 44, 45, and 49 degrees. No case had any neurologic complication. All patients reported complete relief of back pain and were able to upright standing. CONCLUSION: Single-level wedge osteotomy and reduction provide excellent sagittal correction of rigid kyphotic deformities in the lumbar lesion without neurologic compromise.
