1.Direct Immunofluorescent Studies in the Scabies.

There are many reports about the deposition of immunoglobulins and complement by direct immunofluorescence(DIF) in the lesion of scabies. The authors investigated the immunoglobulins and C, for the purpose of studying the immunologic reactivity of the host to scabies mites by DIF. The results are summerized as follows: Total number of the cases investigated were 20 (18 males, 2 females), of whom 11 were in their third decade of age. The clinical forms of the tested lesions were papular type (7 cases), nodular type (12 cases), and Norwegian type (1 case). The durations of the patients were mostly within 2 months, but was 9 months in 1 case of nodular type, Deposition of immunoglobulin and C, was demonstrated by DIF in 10 of 12 cases (83. 3%) of nodular type, and 1 of 7 cases (14. 3%) of papular type. The Norwegian type showed negative results. There seemed to be no relation between the duration of scabies and positive rates. Of 20 cases tested, immunoglobulin and C, were deposited in the vascular wall in 11 lesions (55%), and at the dermoepidermal junction in 3 lesions (15%). While in 1 of 7 cases of papular type both IgM and C, were observed, IgM and C, were noted in 7 and 9, respectively, in 12 cases of nodular type.

3.Radiological Assessment of Progressing of bony Union after Ender Nailing in Acute Tibia Shaft Fracture

Acute unstable tibial shaft fractures are common in these days of high energy trauma. There are many options for this common and complicated fracture. Flexible intramedullary nailing using closed technique is prevalent in that this has much technical, functional, and physiological advantages. We reviewed 51 cases of acute tibial shaft fractures treated with flexible intramedullary nailing. There were 3 nonunions (6%), and two refractures. Among 46 united cases, 8 cases (17 %) showed no bridging callus formation within 20 weeks. Followings are the results of average time reaching each of radiological bony progression stage for 46 united cases. l. Average time to earliest discernible callus formation (stage I) was 13.3 weeks (range:8-21 weeks). 2. Average time to definite and bridging callus formation (stage II) was 18.1 weeks (range:8-32 weeks). 3. Average time to some oblitetation of fracture line (stage III) was 32.8 weeks (range:17-50 weeks). 4. Average time to complete obliteration of fracture line (stage IV) was 14.0 months(range:12-18 months).

4.A Case of Sea Urchin Granuloma.

Sea urchin granuloma is a rare skin disorder, considered to be allergic or foreign body reaction caused by stick of spines of sea urchin. However, detection of the spine in the granuloma is quite difficult. A 27 year-old marine was seen with several asymptomatic hard nodules on the dorsum of hands, fingers, forearrns and knees at the injured site of the spines after a latent period of 6 rnonths. Histolagical examination revealed dermal granulomatous inflamation with hyperkeratotic and acanthotic epidermis. On examination with polarized microscopy, sea urchin spines in the granuloma were seen as bright white fragments in the dark field. Those nodules were regressed with intralesional injection of triamcinoIone.

6.Difference in Right Ventricular Function between Post-tuberculosis Emphysema and Primary Emphysema.

BACKGROUND: Tuberculosis itself causes not only lung parenchymal destruction but also pulmonary vascular damage. Secondary emphysema also causes pulmonary vascular damage, which can develop as a late sequela of pulmonary tuberculosis. Therefore, pulmonary circulatory impairment tends to be more severe in post-tuberculosis emphysema than in primary emphysema. In post-tuberculosis emphysema, the right ventricular function may play an important role. However, little information regarding the right ventricular function is available. The purpose of this study was to evaluate and compare the right ventricular function between post-tuberculosis emphysema and primary emphysema. METHOD: Post-tuberculosis emphysema(PTE) or primary emphysema(PE) was diagnosed by history, HRCT finding and pulmonary function. Twenty patients with post-tuberculosis emphysema were matched with 20 patients with primary emphysema according to both FEV1 and FVC. Arterial blood gas analysis and echocardiography were done at rest and immediately after symptom-limited exercise. The right ventricular function was evaluated with the right ventricular ejection fraction using a modification of Simpson's method. RESULTS: There was no significant difference inthe demographics and pulmonary function between the two groups. In post-tuberculosis emphysema, the PaCO2 was higher (42.9±4.7 vs 38.8±3.1 mmHg at rest ; 47.9±7.0 vs 41.1±5.9 mmHg after exercise ; p<0.01) and the right ventricular ejection fraction was lower (57.6±6.5 vs 61.4±4.7 % at rest ; 51.1±10.8 vs 59.8±6.6 % after exercise ; p<0.01) both at rest and after exercise. The PaO2 after exercise was also lower (65.7±12.6 vs 80.2%±14.4 mmHg, p<0.01), while the PaO2 at rest tended to be lower(82.9±12.0 vs 87.8±7.5, p>0.05). In both groups, right ventricular ejection fraction correlated with the PaO2 after exercise(PTE r=0.536, PE r=0.557), and the PaCO2 at rest(PTE r=-0.576, PE r=-0.588) and after exercise(PTE r=-0.764, PE r=-0.619). CONCLUSION: Impairment of the right heart function and gas exchange were more serious in post-tuberculosis emphysema than in primary emphysema, and gas exchange may be influenced by the right ventricular function in post-tuberculosis emphysema.

7.Distribution of Causative Fungi in Home Environment of Parients with Tinea Capitis Caused bu Microsporum Canis.

Isolation of Microsporum canis from home environment was carried out for 30 tinea capitis patients infected by M, canis. M. canis was isolated in 84.2-100% of patients caps, mattresses, clothes and pillows and families' clothes. The greatest mean number of M. canis colonies per petri dish was found in the patients pillows and followed by caps, mattresses, outerwear, and underwear. Mean number of M. canis colonies in the clothes of mothers, grandmothers, brothers, sisters and roommates of patients was smaller than that of the M. canis colonies in the clothes of patients but greater than that isolated from the clothes of other family members. Frequency of isolation and mean number of colonies decreased drarnatically after 2 weeks therapy and rarely isolated after 6 weeks therapy. M. canis was isolated in 100% of the house dust of patients. Isolation index was greater in the house dust of patients with concomittant infection of other family members and with lesions of scaly and inflammatory type. M. canis was isolated from cats and goat of patients by the hair brush technique.

8.A Case of Kaposi's Varicelliform Eruption Occuring in Pemphigus Foliaceus.

This report presents a case of a 17-year-old-girl who developed a, Kaposis varicelliforrn eruption(eczema herpeticum) during oral prednisolone therapy in order to control pemphigus foliaceus. Diagnosis was made by the history, clinical feature and histologic examination. Within 24 hours after the initiation of therapy with ribavirin(Viramid), new lesions had ceased to develop.
