1.A Study of Types and Distribution of Orthodontic Patients in the Department of Dentistry.

174 patients who visited in the Department of Dentistry, College of Medicine, Yeungnam University from 1983. 7. 15 to 1986. 11. 30 were surveyed on the tendency of patient distribution and the state of Angle's classification. The results were as follows 1. There was increased visiting rate of patient per year. Female outnumbered male by a ratio of 1.3:1. 2. 8-15 age group was 74% in total visiting in the most frequency. The average visiting numbers of each month were higher among the vacation and was about 74% in that period. 3. As showed the living distribution, Nam Go and Su Sung Gu's patients were 53% of the total. 4. As the motive of visiting in Dental Department, patient of 30% in total patients visited for themself to receive orthodontic treatment and other was introduced patients. 5. By classification, Class I malocclusion was 63% in total visiting patient, Class II malocclusion 17%, Class III malocclusion 20%. Crowding among Class I malocclusion was 60%.

2.Three dimensional finite element stress analysis of five different taper design implant systems.

STATEMENT OF PROBLEM: Dental implant which has been developed gradually through many experiments and clinical applications is presently used to various dental prosthetic treatments. To conduct a successful function of implant prosthesis in oral cavity for a long time, it is important that not only structure materials must have the biocompatibility, but also the prosthesis must be designed for the stress, which is occurred in occlusion, to scatter adequately of load support. Therefore, it is essential to give the consideration about the stress analysis of supporting tissues for higher successful rates. PURPOSE: Recently, many implant manufacturing company produce various taper design of root form implant, the fixture is often selected. However, the stress analysis of taper form fixture still requires more studies. MATERIAL AND METHOD: This study we make the element models that five implant fixture; Branemark system(Nobel Biocare, Gothenberg, Sweden), Camlog system(Altatec, Germany), Astra system(Astra Tech, Sweden), 3i system(Implant Innovations Inc, USA), Avana system(Osstem, Korea)were placed in the area of mandibular first premolar and prosthesis fabricated, which we compared with stress distribution using the three-dimension finite element analysis under two loading condition. RESULTS: This study compares the aspect of stress distribution of each system with the standard of Von mises stress, among many resulted from finite element analysis so that this research gets the following results. 1. In all implant system, oblique loading of maximum Von mises stress of implant, alveolar bone and crown is higher than vertical loading of those. 2. Regardless of loading conditions and type of system, cortical bone which contacts with implant fixture top area has high stress, and cancellous bone has a little stress. under the vertical loading, maximum Von mises stress is more higher in order of Branemark, Camlog, Astra, 3i, Avana. under the horizontal loading, maximum Von mises is more higher in order of Camlog, Branemark, Astra, 3i, Avana.

3.Angle's Class II Division 2 Malocclusion Treated by Bioprogressive Mechanism: Report of a Case.

A 25 year and 7 month old man patient who had Angle's class II division 2 malocclusion combined with anterior crowding of upper & lower part was treated by bioprogressive mechanism. After setting out objectives through the use of V. T. O., we programmed a sequence of mechanic. The possible objectives of treatment in the class II division 2 malocclusion can be listed as follows. 1) Relief of crowding & irregularities. 2) Relief of anterior gingival trauma & correction of interincisal inclination. 3) Correction of buccal segment relationship We'd applied the class II intermaxillary elastics, Quad helix, utility arch wire and sectional arch wire in order to achieve anticipated objectives. As compared with pre & post treatment cephalogram, the result accomplished by this mechanics showed to us that interincisal angle was improved and favorable molar relationship was achieved.

5.Surgical Treatment of Phenytoin Induced Gingival Hyperplasia: A Report of Case.

Enlargement of the gingival caused by phenytoin. An anticonvulsant used in the treatment of epilepsy, occurs in some of the patients receiving the drug. Its incidence varies from 3 to 62 percent, with the greater frequencies in younger patients. The hyperplasia is usually generalized throughout the mouth, but is more severe tendency in the maxillary and mandibular anterior regions. 18 year old male patient was admitted to our Department of Dentistry with the complaint of generalized painless gingival swelling. After the consult of the N.M. and laboratory study, the gingivectomy and gingivoplasty was performed. The periodontal pack and tin foil was applied on the attached gingival to protect a surgical site and bleeding control. We obtained a good result of improved esthetics and function.

6.Postoperative radiotherapy for endometrial cancer.

PURPOSE: To investigate the prognostic factors and effectiveness of postoperative radiotherapy alone for endometrial carcinoma. MATERIALS AND METHODS: Sixty four patients with stage I-III endometrial cancer (EC) treated with postoperative radiotherapy alone between January 1989 and December 2008 at the Keimyung University Dongsan Medical Center were chosen for the present study. Typically, total hysterectomy, salpingo-oophorectomy and lymphadenectomy were performed on the patient's pelvis. Total dose from 50.4 Gy to 63 Gy was irradiated at pelvis or extended field. Thirteen patients were treated with Co-60 or Ir-192 intracavitary radiotherapy. Follow-up periods were from 7 to 270 months, with a median of 56 months. RESULTS: Five year overall survival (OS) rate was 58.7%, respectively. Five year disease-free survival (DFS) rate was 59.2%, respectively. In univariate analysis for OS and DFS, stage, menopausal age, type of operation, serosal invasion, and lymph node involvement were found to be statistically significant. Histologic type was marginally significant. In multivariate analysis for OS and DFS, stage, types of operation, histologic type were also found to be statistically significant. Treatment failure occurred in 14 patients. The main pattern of failure was found to be distant metastasis. Time to distant metastasis was from 3 to 86 months (median, 12 months). There were no grade 3 or 4 complications. CONCLUSION: Stage, types of operation, and histologic type could be the predictive prognostic factors in patients. We contemplated postoperative radiation as effective and safe treatment method for EC. Additional treatment would be needed to reduce distant metastasis.

7.Comparison of Catecholamine Levels in Hemiplegic Patients with and without Reflex Sympathetic Dystrophy.

OBJECTIVE: To determine whether the cause of sympathetic dysfunction is due to increased regional sympathetic outflow or receptor supersensitivity to circulating catecholamines in the pathogenesis of reflex sympathetic dystrophy in hemiplegia. METHOD: Ten hemiplegic patients with reflex sympathetic dystrophy were instructed to refrain from smoking or using caffeine and alcohol, and medications that influence catecholamine metabolism were witheld for 24 hours before blood sampling. Patients with cardiovascular disease, diabetes or abnormal liver and renal function tests were excluded from the study. Patients with a history of sympathectomy were also excluded. Ten hemiplegic patients without reflex sympathetic dystrophy served as the control group. Both groups of patients rested in supine position in a quiet room for 30 minutes. A needle with heparin cap was inserted into the dorsal venous arches of the affected hand and patients rested for another 20 minutes, after which blood was drawn through the heparin cap. The blood samples were assayed using high-performance liquid chromatography (HPLC) and norepinephrine and epinephrine were detected electrochemically. 24 hour urine was collected during rest and vanillylmandelic acid (VMA) and metanephrine were also detected using HPLC. RESULTS: The mean plasma norepinephrine levels were 1.05 0.24 ng/ml and 0.47 0.06 ng/ml in RSD affected and unaffected groups respectively, and the plasma norepinephrine level was significantly higher in the patient group with reflex sympathetic dystrophy (p<0.05). The plasma epinephrine and 24-hour urine VMA and metanephrine levels were not significantly different in two groups. CONCLUSION: These results may support a hypothesis of increased regional sympathetic outflow in the pathogenesis of reflex sympathetic dystrophy in hemiplegia.

8.MR imaging of internuclear ophthalmoplegia due to cerebrovascular diseases.

Internuclear ophthalmoplegia is a conjugated gaze disorder characterized by impaired adduction on the side of a lesion involving the medial longitudinal fasciculus with dissociated nystagmus of the other abducting eye. Six patients with INO (who had clinical cerebrovascular diseases) underwent MR imaging and the results were as follows: 1. The MLF lesions were identified by MR imaging in 5 cases 2. The ratio of unilateral INO to bilateral INO was 5:1 3. The nature of lesions was infarction in 4 cases and hemorrhage in 1 case 4. The sites of MLE lesion were in the midbrain in 4 cases and in the pons in 1 case 5. All 5 cases of INO identified by MR imaging had other lesion sites in addition to MLE lesion.

9.Concurrent chemoradiotherapy improves survival outcome in muscle-invasive bladder cancer.

PURPOSE: To evaluate survival rates and prognostic factors related to treatment outcomes after bladder preserving therapy including transurethral resection of bladder tumor, radiotherapy (RT) with or without concurrent chemotherapy in bladder cancer with a curative intent. MATERIALS AND METHODS: We retrospectively studied 50 bladder cancer patients treated with bladder-preserving therapy at Keimyung University Dongsan Medical Center from January 1999 to December 2010. Age ranged from 46 to 89 years (median, 71.5 years). Bladder cancer was the American Joint Committee on Cancer (AJCC) stage II, III, and IV in 9, 27, and 14 patients, respectively. Thirty patients were treated with concurrent chemoradiotherapy (CCRT) and 20 patients with RT alone. Nine patients received chemotherapy prior to CCRT or RT alone. Radiation was delivered with a four-field box technique (median, 63 Gy; range, 48.6 to 70.2 Gy). The follow-up periods ranged from 2 to 169 months (median, 34 months). RESULTS: Thirty patients (60%) showed complete response and 13 (26%) a partial response. All patients could have their own bladder preserved. Five-year overall survival (OS) rate was 37.2%, and the 5-year disease-free survival (DFS) rate was 30.2%. In multivariate analysis, tumor grade and CCRT were statistically significant in OS. CONCLUSION: Tumor grade was a significant prognostic factor related to OS. CCRT is also considered to improve survival outcomes. Further multi-institutional studies are needed to elucidate the impact of RT in bladder cancer.
