2.MR Imaging with Gd-DTPA Enhancement in the Testicular Ischemia in Rat Model: Evaluation of Testicular Viability .

PURPOSE: To find the magnetic resonance (MR) imaging patterns and to determine the viability in normal, infracted and reversible ischemic testis of the rat. MATERIALS AND METHODS: Fifteen Sprague-Dawley rats were examined and they were divided into four groups. Group 1 was the control group, group 2 had a complete testicular artery ligation, group 3 had a complete ligation with reperfusion after 1 hour and group 4 had a complete ligation with reperfusion after 12 hours. All four groups were imaged every 5 minutes for 30 minutes. Delayed MR imaging was obtained every 30 minutes for 90 minutes. Two follow-up MR images were performed in all groups at a one-week interval. The signal intensity was measured in the normal testis, ischemic testis, and in muscle, water and fat in every rat at each time, with the phantom attached near the scrotum during the scanning. The signal intensities were analyzed statistically. RESULTS: On initial and 2-week follow-up examinations, the pattern of change differed among four groups (p<0.001). Group 1 and Group 3 did not show any marked change over time at each examination. Group 3 showed strong enhancement at the first week follow-up. Group 2 showed steadily delayed enhancement at each examination. Group 4 had same pattern with the Group 2 with higher enhancement intensity in parallel. CONCLUSION: MR images with Gd-DTPA could be useful for the diagnosis of damaged testicular tissue and for the determination of testicular viability.

6.Clinical study on twins.

Clinical survey was performed on 413 cases of twin pregnancies and their 816 twin babies who were born at IlSin Christian Hospital during 5years from January 1987 to December 1991. The result of study was as follows: 1) there was 413 twin pregnancies among 40,711 deliveries, so the incidence of twins was 1:98.6. Average male-female sex ratio of twins was 1.3:1, same sex pairs rate was 86% and different sex ratio was 14%. 2) The incidence of relation with maternal age was highest between 25 years old and 29 years old but the ratio of twin delivery to total delivery was 0.9% so, was not higher than other age group. 3) The incidence of relation with maternal parity was highest at first pregnancy but the ratio of twin delivery to total delivery was highest at second pregnancies. 4) Normal birth weight infants were 45.7%, low birth weight infants were 54.3% and very low birth weight infants was 9.3%. Fullt-erm infants were 63.7% and prematurity were 35.4%. 5) The incidence of feto-fetal transfusion was 7.3%. 6) Perinatal death rate was 76.3 and was more higher in second babies. The most common cause of perinatal death was prematurity, followed by respiratory complication, congenital anomaly, infection and asphyxia. Still-births were 8 cases.

7.Successful treatment with methotrexate injection on ectopic pregnancy embedded in the myometrium of a previous cesarean section scar.

Myometrial pregnancy developing in an previous cesarean section scar is the rarest of all ectopic pregnancy and provavly one of the most dangerous because of the risk of rupture and hemorrhage. For a young patient wanting to maintain her fertility, an earlier diagnosis and more conservative treatment are highly desirable. We present a case of an ectopic pregnancy embedded in the myometrium of a previous cesarean section scar in which the patient preserved her fertility through more noninvasive method, transvaginal ultrasound-guided Methotrexate injection.

8.Immuno-diffusion and complement-fixation test in paragonimiasis of cat.

In order to observe the complement fixation test and immuno-diffusion test of paragonimiasis, the sera taken at 10 days intervals up to 150 days from cats infected with Paragonimus westermani were examined by the above two immunological methods. The resultant findings were as follows: The complement fixation test showed positive reaction 20 days after the infection with 20 metacercariae, and 40-50 days after the infection with 10 metacercariae. The highest titer was observed 110 days later following the acceleration at 80 days later. In immuno-diffusion test, one are appeared 30 days after the infection with 20 metacercariae, but 60 days after the infection with 10 metacercariae. However, more than two arcs were observed since 70 days after infection. A relatively wide band appeared by the antigens of Fresh worm material and Somatic material. But relatively clear precipitin lines were observed in the diffusion test with V.B.S. antigen, increasing to 3-4 arcs after 110days. In general, complement fixation test showed earlier and higher sensitive reaction than immuno-diffusion test, and was considered to be more valuable method forr immunological diagnosis.

9.Direct Immunofluorescent Studies in the Scabies.

There are many reports about the deposition of immunoglobulins and complement by direct immunofluorescence(DIF) in the lesion of scabies. The authors investigated the immunoglobulins and C, for the purpose of studying the immunologic reactivity of the host to scabies mites by DIF. The results are summerized as follows: Total number of the cases investigated were 20 (18 males, 2 females), of whom 11 were in their third decade of age. The clinical forms of the tested lesions were papular type (7 cases), nodular type (12 cases), and Norwegian type (1 case). The durations of the patients were mostly within 2 months, but was 9 months in 1 case of nodular type, Deposition of immunoglobulin and C, was demonstrated by DIF in 10 of 12 cases (83. 3%) of nodular type, and 1 of 7 cases (14. 3%) of papular type. The Norwegian type showed negative results. There seemed to be no relation between the duration of scabies and positive rates. Of 20 cases tested, immunoglobulin and C, were deposited in the vascular wall in 11 lesions (55%), and at the dermoepidermal junction in 3 lesions (15%). While in 1 of 7 cases of papular type both IgM and C, were observed, IgM and C, were noted in 7 and 9, respectively, in 12 cases of nodular type.

10.Clinical Observation on Non-visualizing Kidney.

A clinical observation was made on the non-visualizing kidney of the in-patient in the Department of Urology, Capital Armed Forces General Hospital during the period from September, 1976 to December, 1981 and the following results were observed. 1. Of 1287 cases hospitalized, IVP was performed on 292 cases. Among these, 36 cases revealed non- visualizing kidney giving a rate of 12.3%. 2. The sex ratio between male and female was 8:1 and age distribution ranged from 1 to 66 years. 3. The clinical symptoms and signs were hematuria, flank pain, frequency, lower abdominal pain, and abdominal palpable mass in order. 4. The causes of non-visualizing kidney was classified as renal parenchymal and obstructive disease. Renal tuberculosis (9 cases) was the most frequent cause among the disease of parenchymal origin, and the calculus disease of the urinary track (5 cases) and hydronephrosis not caused by calculi (5 cases) were the most frequent cause among the disease of obstructive origin. 5. Surgery was performed in 29 cases (80.1%) and remaining 7 cases were managed conservatively.
