1.Radiologic analysis & diagnostic value of lateral tomography on ossification of posterior longitudinal ligamentof c-spine

The ossification of posterior longitudinal ligament of the spine(OPLL) is newly recongnized clinical entity,although compression of the spinal cord by an OPLL was reported by key as early as in 1839 in Guy's HospitalReport. OPLL was noticeable in lateral tomography as an abnormal dense radiopacity along the posterior margins ofthe vertebral body. Authors retrospectively analysed the diagnostic values and findings of lateral tomography ofthe cervical spine in 11 cases at Kang Nam General Hospital Public Corporation during 1 yr from July 1984 to June1985. The results were as follows; 1. Among suspected 11 cases of OPLL, 9 cases were confiremd as OPLL on lateraltomogram. 2. Age range was 25 years old to 55 years old and more prevalent age was over 5th decades & male wasmore involved than female. 3. Frequent involvement was C2-C5 level and number of vertebral bodies involved was 3.6in average. 4. This ossification developed 4 modes, a continuous type 11%, segmental 33%, mixed type 33%,circumscribed type in 22%. 5. OPLL thickness were from 2mm to 4.5mm and spnal canal narrowing ratio were form 25%to 44% and there were norational relationships between clinical symptom and thicknness of OPLL. 6. On diagnosis ofOPLL, lateral tomography is accurate and recommendable screening study due to easy, noninvasive, indisipensable and less harmful technique, compared to those of myelography or computed tomography.

3.Computed tomography of osteitis condensans ilii

The CT is a more accurate technique for detecting sclerotic bony change of osteritis condensans ilii thanplain radiograph. We analysed a comparison between CT and plain radiography of osteitis condensans ilii, acorrelation between osteitis condensans ilii and women of childbearing age. The result were as follow: 1. Theincidence of osteitis condensans ilii is 5.3% on KUB, 11.7% on CT when the width of iliac sclerosis is more than7.5mm as diagnsotic criteria. 2. We observed a osteitis condensans ilii between 19 years and 51 years of age, mostfrequently in fourth decade. 3. The width of iliac sclerosis is 10-13 mm in 3 cases of osteitis condensans ilii onboth CT &KUB, 7.5-9mm in 4 cases of osteitis condensans ilii on CT only. 4. The incidence of osteitis condensansilii is increased significantly when the width of iliac sclerosis is less than 7.5mm as diagnostic criteria. 5.Relatively high correlation between osteitis condensans ilii and delivary in our study (66.6%)

4.Comparative Study of Postoperative Analgesic Effect of Morphine According to Timing of Injection.

BACKGROUND: The initiation of treatment seems to be important in provoking preemptive analgesia and to provide excellent postoperative pain control. To verify the effectiveness of postoperative analgesia and preemptive effects, pre-incisional and post-incisional epidural infusion of morphine and bupivacaine mixture were compared. METHODS: Patients scheduled for elective upper abdominal surgery were divided into 2 groups. Each group received 2 mg of morphine in 10 ml of 0.25% bupivacaine as a bolus followed by 0.1 mg/ml/hr of morphine infusion in 0.125% bupivacaine epidurally. In post-incisional group, the epidural injection of mixture was started 15 minutes after skin incision and in pre-incisional group, the epidural infusion of mixture was started 15 minutes before skin incision. Each groups were evaluated in visual analogue scales (VAS) for pain, pain scores in movement, and total administered doses in postoperative 1, 2, 4, 8, 24, 48, and 72 hours and compared with each others. RESULTS: There were no differences between pre-incisional and post-incisional start of continuous epidural mixture infusion of morphine and bupivacaine in VAS for pain, pain scores at movement, and total amount of infused doses for 72 hours postoperatively. Also, there were no differences in the incidence of complications and satisfactions of patients between two groups. CONCLUSIONS: The continuous epidural mixture infusion of morphine 0.1 mg/ml/hr in 0.125% bupivacaine following a bolus dose of 2 mg morphine in 0.25% 10 ml bupivacaine has no difference in postoperative analgesic effect whether it starts after or before surgical incision.

6.Ultrasonographic measurement of the neonatal adrenal glands.

TO determine the normal neonatal adrenal gland size, ultrasonographic examinations were performed in 145 newborn infants. They were divided into 3 groups according to the days of age. The group I is 1~3 days, group II is 6~9 days and group III is 21~50 days of age. 1) The adrenal gland size was as follows. In group I, the length was 29.05mm and the width was 4.02mm. In group II, the length was 24.04mm and the width was 2.79mm. In group III, the length was 19.54mm and the width was 2.21mm. 2) With increasing age, the size of adrenal gland became smaller. 3) The size of adrenal gland correlated well with the birth weight and height. We conclude that the ultrasonographic examination is useful in infant adrenal gland disease.

8.Histopathologic changes of the craniomandibular joint according to the amount of distraction after 6 weeks of distraction osteogenesis in rabbits.

The purpose of this study is to observe histopathologic changes in the bilateral craniomandibular joints after allowing 6 weeks of consolidation by varying the amount of distraction in rabbit mandible. Eight rabbits weighing about 2 to 3kg were used. After corticotomy was performed on the left mandibular body between the first premolar and the second premolar region, a unilateral fixation device was placed. Then, a 7-day period was allowed without distraction of the device. The mandible was lengthened 0.5mm/day. Corticotomy and lengthening of mandible were not performed in control group. After the completion of the lengthening process, a 6-week-consolidation period was allowed. Then, the rabbits were sacrificed, and histologic examination of the craniomandibular joints was performed. Proliferative changes were observed in the craniomandibular joints in all groups. With the increasing amount of distraction, hypertrophy of the cartilage layer became more severe, bone formed was dense and enchondral ossification was clearly shown in subchondral bone. Hypertrophy of the cartilage layer was also seen in the non-distracted side as the distracted side in the experimental group. These results indicate that when physical force is applied constantly to joints, the proliferation of articular cartilage and bone formation are present. When more than 6 weeks of consolidation period is allowed at the time of performing distraction for more than 5mm, articular changes, especially, in the contralateral side should also be noted.

9.The Surgical Outcome of Clear Lens Extraction for Correction of Severe High Myopia.

The authors performed clear lens extraction on 22 eye to treat severe high myopia more than 12 diopters(D). The changes in spherical equivalent (S. E.), visual acuity improvement, and the complications were evaluated during 6 months postoperatively. A scleral pocket incision and continuous curvilinear capsulorhexis was made and lens extraction was performed via phacoemulsification and low power poserior chamber intraocular lens was implanted. The postoperative refractive error target were form -1.5D to-2.0D. More than 85% of eyes were within +/-1D of targeted refractive error. The posoperative uncorrected visual acuities were increased more than 20/200 in all eyes and 20% of eyes were increased more than 20/40 at postoperative two months. The postoperative corrected visual acuities were increased two or more limes in more than 80% of eyes. Intraoperatively, hyphema in one eye, wound leaking in two eyes and radial tear during contimuous curvilinear capsulorhexis in one eye were developed. Poltoperatively, retinal detachment in two eyes, subretinal neovascularization in one eye and posterior capsular opacification in three eyes were developed. In conclusion, the clear lens extraction was a effective method for correction of severe high myopia but longer follow-up of large number of cases is needed to fully assess the safety.
