1.Echocardiographic Assessment of Left Ventricular Function in Ventricular Premature Beats.

Ventricular premature beats(VPB) are the most common cardiac arrhythmia and frequently found in apparently normal healthy persons as well as in various heart diseases. Postectopic potentiation of ventricular contraction has widely reported by electrocardiography. In this study, the authors performed comparative assessment of the left ventricular function at the preextrasystolic, extrasystolic and postextrasystolic beats by M-mode echocardiography and electrocardiography. The results obtained were as followings; 1) Among 22 patients of isolated, unifocal ventricular premature beats with fully compensatory pause, male were 6 and female 16, mean age being 47.2 years. 2) Hypertensive heart disease was the most common underlying disease of ventricular premature beats(5 patients) and the others were valvular heart diaeases(3), ischemic heart diseases(3), cardiomyopathy(2), intracerebral hemorrhagy(1), atrial septal defect of secundumtype(1), cor pulmonale(1), pericarditis(1), 1degree A-V block(1) and enteric fever(1). In 3 patients the causes were not clarified. 3) R wave amplitude was higher in the postextrasystolic beat(2.02+/-0.79cm) than preestrasystolic beat(1.86+/-0.66cm) 4) PEP/LVET ratio showed more decrease(18%) in postextrasystolic beat than that of preextrasystole. 5) Diastolic filling time of mitral valve was more reduced in the extrasystole than preextrasystole, but that of postextrasystole exceeded the preextrastystole. 6) Aortic cusps separation was more decreased in the extrasystole than preextrasystole and slightly more increased in the postextrasystole than preextrasystole. 7) LVID, LSa, ENa, DeltaEN/Deltat, LVEDV, SV and LV mass showed more decrease in extrasystole compared with those in pre-and postextrasystole, which was higher than preextrasystole. But LVESV was slightly lower in extrasystole than preextrasystole. 8) In extrasystolic beat, LVEDV, LVESV, stroke volume, and ejection fraction of ventri cular premature beat were 15.77%, 0.18%, 24.05% and 12.21% lower, respectively, than those of preextrasystole. And in the postextrasystole, LVEDV, LVESV, stroke volume and ejection fraction were 23.35%, 2.42%, 28.51% and 10.4% higher, respectively, than these of preextrasystole.

3.Evaluation of Left Ventricular Diastolic Function in Patients Receiving Doxorubicin.

Doxorubicin(Adriamycin(R)) is effective in the treatment of various solid tumors and hematologic malignancies. Because of dose-related cardiotoxicity, however, early inappropriate discontinuation of doxorubicin therapy may minimize its therapeutic efficacy in many patients. Consequently, clinically sensitive tests are needed to select patients in whom treatment must be stopped early. Various techniques have been used for early detection of subclinical doxorubicin-induced cardiotoxicity, including electrocardiography, systolic time intervals, echocardiography, radionuclide angiography and endomyocardial biopsies. Most studies of doxorubicin cardiotoxicity have dealt with systolic function of the left ventricle and effects on diastolic function have not been reported. In order to determine whether impaired diastolic function may be an early sign of doxorubicin cardiotoxicity, a retrospective study was performed in 12 patients who had undergone serial radioangiography and were found to have left ventricular ejection fractions(LVEF)> or =55% prior to doxorubicin treatment and during follow-up. Average rapid filling velocity(RFV) and slow filling velocity(SFV) were both significantly reduced after doxorubicin treatment. RFV fell from 5.17+/-1.52 units/second to 4.18+/-0.96(P<0.01) and SFV fell from 2.20+/-1.32 units/second to 1.42+/-0.62(P<0.05). There were no significant changes in filling volume ratio, total diastolic time and diastolic time ratio. Since a change in left ventricular diastolic function can occur before ejection fraction falls to subnormal levels, diastolic function as well as systolic function should be examined in the early detection of doxorubicin cardiotoxicity.

4.Plasmid Profile and B - Lactamase Type of Multidrug - Resistant Salmonella typhi Isolated from Korea, 1997.

Eight strains of multidrug-resistant (MDR) Salmonella typhi were isolated from Kyonggi area during January-February,1997. They were resistant to ampiciUin, amoxicillin, carbeniciillin, tetracycline, chloramphenicol, trimethoprim/sulfamethoxazole, trimethoprim. Eight strains had one plasmid respectively which size was approximately M.W 220 kb and showed same restriction pattern by endonuclease HindIII. The plasmid was similar to the plasmid in size that was related to multidrug resistant S. typhi isolated from southeast Asia. It were transferred by conjugation to recipient E, coli K-12 in frequency of 2.43 x10-4 - 1.73 x 10-2 and transconjugant showed same drug-resistant pattem with donor cells. All of 8 strains produced B-lactamase that was assummed to TEM-1 type by isoelectric focusing and PCR.

5.Temporary Opening of the Testis-blood Barrier by Triolein Fat Emulsion.

PURPOSE: Although the purpose of the blood-testis barrier (BTB) is to protect germ cells from harmful influences, it also impedes the delivery of chemotherapeutic agents to the testis. This study was undertaken to determine whether a triolein emulsion could transiently alter the permeability of the BTB in cats. MATERIALS AND METHODS: An emulsion of 0.05ml triolein in 20ml of saline or just 20ml of normal saline, as the control, were infused into the testicular arteries in 18 and 15 cats, respectively (embolic and control group). Pre- and post-contrast magnetic resonance images (MRIs) were obtained 30 minutes and 2 hours after embolization. Qualitative and quantitative analyses of the MRIs were performed via the presence and degree of contrast enhancement and the contrast enhancement ratios (CERs), respectively. An electron microscopy (EM) study was subsequently performed, using a lanthanum tracer, to correlate with the MRI results. RESULTS: Contrast enhancement of the testis was observed in both groups and at both time points, but was more prominent in the embolic group. The CERs in the embolic group were significantly higher than those in the control group (p=0.0001). In each group, the CERs at 2 hours were significantly lower than those at 30 minutes (p=0.006). In the EM study, the entry of lanthanum was markedly increased at 30 mins, but recovered at 2 hours after embolization compared to the control. CONCLUSIONS: Intra-arterial infusion of triolein emulsion transiently increased the permeability of the BTB. This result may be useful in future studies for a chemotherapy delivery system to the testis.

7.Clinical Observation on Urolithiasis in Childhood.

A Clinical observation was made on 10 cases of childhood urolithiasis under 15 year-old who were admitted to the department of urology, Hanyang University Hospital during the period from May, 1972 to Dec, 1983. The results were as follows: 1. of 3482 cases hospitalized, 10 cases were urolithiasis in childhood giving ratio of 0.29%. 2. The favorable age was absent, but showed even distribution. 3. Congenital anomalies 2, metabolic cause 1, idiopathic cause 7 were recognized as etiologic causes. 4. Incidence of lower urinary stone was high, 70%. 5. The most frequent symptoms and signs were voiding difficulty, 4 cases. Hematuria, flank pain and bladder irritative symptoms were also noted. 6.The urine cultures were showed Klebsiella pneumonia in 1 case, Enterobacter aerogens in 1 case, Enterococci in 1 case and combined infection of E. coli and Enterococci in 1 case. 7. Stone analysis was showed calcium oxalate in 3 cases, combined stone of calcium oxalate and ammonium phosphate in 1 case and fibrinoid calculi in 1 case. 8. Recurred rate cannot be estimated because of the patient was not revisited to out-patient-department at all after discharge.

8.A Case of Schizophrenia with Periventricular Leukomalacia on Magnetic Resonance Imaging.

Schizophrenia was once known as a functional psychosis, a disorder with no structural basis. However, nowadays research has identified and confirmed many structural abnormalities in schizophrenia. One prominent such feature is the combination of ventricular enlargement and decreased cerebral volume. Periventricular leukomalacia causes cerebral palsy, cognitive impairment, visual impairment, and seizures. Recent studies reveal that periventricular leukomalacia causes ventricular enlargement and many brain circuit disruptions. Ventricular enlargement caused by periventricular leukomalacia is the most common pathological marker of schizophrenia. In this study, we report a case with periventricular leukomalacia on MRI findings in a 28-year-old schizophrenia patient. This patient showed auditory hallucinations and somatic delusions. We highlight the structural abnormalities, especially periventricular leukomalacia, in this case of schizophrenia.

9.The Effects of Achievement Self Discrepancy and Evaluative Threat Stress on Performance Anxiety.

OBJECTIVES: The purpose of this study to examine if the levels of anxiety(emotional, cognitive, and behavioral measures) during performance are influenced by interaction of both achievement related self discrepancy(=psychological vulnerability variable) and evaluative threat stress(=stressor) based on vulnerabilitystress model in order to identify the mechanism of development of performance anxiety and furthermore to suggest psychological intervention strategies to reduce and prevent it effectively. METHOD: High and low self discrepancy subjects consisted of students from the upper(n=40) and lower 15%(n=40) on achievement related self discrepancy scale, respectively. They were randomly assigned to either evaluative threat condition or neutral condition, and then all subjects took intellective task(verbal reasoning test and digit symbol test). RESULTS: The results were as follows; 1) High self discrepancy subjects under evaluative threat reported significantly higher level of state anxiety during performance than low self discrepancy subjects under any condition and also experienced higher degreee of state anxiety with marginal significance than high self discrepancy subjects under neutral condition, whereas there were no significant effects on the anxiety level of group variable under neutral condition or of stress variable in low self discrepancy subjects. 2) High self discrepancy subjects reported more cognitive interference during performance than low self discrepancy subjects regardless of the level of stress. 3) High self discrepancy subjects performed marginally significantly more poorly in digit symbol test than low self discrepancy subjects regardless of the level of stress, whereas there were no significant effects on verbal reasoning test. CONCLUSION: The hypothesis that performance anxiety could be affected by achievement related self discrepancy and stress, and specially by interaction between self discrepancy and stress was relatively strongly supported by the results on emotional measure of performance anxiety. In addition, this hypothesis received partial support by the results on cognitive and behavioral measures. We might conclude that it is important to consider achievement-related self discrepancy and evaluative threat stress together to understand the mechanism of development of performance anxiety and to reduce or prevent this anxiety effectively.

Multiple methods and materials are available for bone defect reconstruction. Bone graft substitute is one of the materials used for reconstruction of bone defect and have been widely used recently. This report describes some cases about complications related to GeneX(R) which is introduced as mixture of calcium sulfate and beta-tricalcium phosphate at manufacturer's official web site. It informed of 3 patients who suffered wound inflammation, serous cyst after using GeneX(R) for reconstructing skull defect.
